目的:挖掘赵珂主任医师治疗月经后期的用药组方规律,为临床治疗该疾病提供参考。方法:整理赵珂主任2022年5月~2023年6月期间治疗月经后期的用药处方,录入Excel、SPSS Modeler 18.0平台和IBM SPSS statistics 25,采用关联规则、复杂系统...目的:挖掘赵珂主任医师治疗月经后期的用药组方规律,为临床治疗该疾病提供参考。方法:整理赵珂主任2022年5月~2023年6月期间治疗月经后期的用药处方,录入Excel、SPSS Modeler 18.0平台和IBM SPSS statistics 25,采用关联规则、复杂系统聚类、聚类分析等统计方法对整理好的处方药物进行用药规律分析。结果:共收集处方210首,涉及中药198味。中药使用频次前十位依次为白芍、菟丝子、香附、山茱萸、甘草、川芎、党参、熟地黄、当归、肉苁蓉。常用药对有白芍–紫石英、香附–延胡索、川芎–桃仁、白芍–杜仲、白芍–北沙参、白芍–淫羊藿、白芍–黄精、川芎–红花、菟丝子–鹿角霜、白芍–鹿角霜等。通过高频药物聚类分析得到可能的新方组合4首。结论:利用数据挖掘技术,可初步揭示赵珂主任临床诊疗月经后期疾病的规律:从肝、脾、肾论治,多用白芍、菟丝子、山茱萸等补虚药以滋经血之源,在此基础上加以辨证,配伍香附、川芎、延胡索等药物理气活血调经,从而治愈疾病。本研究亦可用于中药新药候选处方的筛选。Objective: To explore the medication rules of Director Zhao Ke’s treatment of late menstruation period, and provide reference for the clinical treatment of this disease. Methods: The prescriptions of Director Zhao Ke’s treatment of late menstruation from May 2022 to June 2023 were collected, and recorded into Excel, SPSS Modeler 18.0 platform and IBM SPSS statistics 25. The medication rules of the collected prescriptions were analyzed by association rules, complex system clustering, cluster analysis and other statistical methods. Results: A total of 210 prescriptions were collected, involving 198 flavors of traditional Chinese medicine. The top ten frequency of traditional Chinese medicine were Baishao (Paeoniae Radix Alba), Tusizi (Cuscutae Semen), Xiangfu (Cyperus Rotundus), Shanzhuyu (Corni Fructus), Gancao (Radix Rhizoma Glycyrrhizae), Chuanxiong (Chuanxiong Rhizoma), Dangshen (Codonopsis Radix), Shudihuang (Prepared Rehmannia Root), Danggui (Angelicae Sinensis Radix), Roucongrong (Cistanches Herba). The commonly used drug pairs were Baishao (Paeoniae Radix Alba)-Zishiying (Fluoritum), Xiangfu (Cyperus Rotundus)-Yanhusuo (Corydalis Rhizoma), Chuanxiong (Chuanxiong Rhizoma)-Taoren (Persicae Semen), Baishao (Paeoniae Radix Alba)-Duzhong (Eucommiae Cortex), Baishao (Paeoniae Radix Alba)-Beishashen (Glehniae Radix), Baishao (Paeoniae Radix Alba)-Yinyanghuo (Epimedii Folium), Beishashen (Glehniae Radix)-Huangjing (Polygonati Rhizoma), Chuanxiong (Chuanxiong Rhizoma)-Honghua (Carthami Flos), Tusizi (Cuscutae Semen)-Lujiaoshuang (Cornu Cervi Degelatinatum), Baishao (Paeoniae Radix Alba)-Lujiaoshuang (Cornu Cervi Degelatinatum), etc. Four compositions possible new combinations were obtained through high-frequency drug cluster analysis. Conclusions: Using data mining technology, we can preliminarily reveal the rules of clinical diagnosis and treatment of diseases in the late menstrual period by Director Zhao Ke: from the treatment of liver, spleen and kidney, the use of Baishao (Paeoniae Radix Alba), Tusizi (Cuscutae Semen), Shanzhuyu (Corni Fructus) and other tonifying drugs to nourish the source of menstrual blood, on this basis, to be differentiated, compatibility with Xiangfu (Cyperus Rotundus), Chuanxiong (Chuanxiong Rhizoma), Yanhusuo (Corydalis Rhizoma) and other drugs to activate Qi and blood circulation, so as to cure diseases. Methods: Information from 215 Chinese herbal prescriptions for TE-induced hypomenorrhea from Director Zhao Ke’s outpatient clinic was collected. The constituent herbs were organized and analyzed scientifically, entered into an Excel database, and a complex network system established using SPSS software was utilized to analyze the high-frequency herbs and principles of compatibility in prescriptions for TE-induced hypomenorrhea. Results: A total of 215 Chinese herbal formulas were included, comprising 165 Chinese herbs. Among them, 13 herbs were used with a frequency of ≥100, namely Dodder Seed, Polygonatum Sibiricum, Cornus Officinalis, Rehmannia Glutinosa, etc. The herbal temperature properties were mainly warm;the flavors were mainly sweet, bitter, and pungent;and the meridian affinities were primarily associated with the liver, spleen, and kidney meridians. Association rules and complex network diagrams revealed strong associations between herb combinations, such as Dodder Seed → Dipsacus Asperoides, Dodder Seed → Cornu Cervi Degelatinatum, Cornus Officinalis → Radix Pseudostellariae, and Salvia Miltiorrhiza. Cluster analysis classified the herbs into three categories: the first category was kidney-tonifying herbs, the second was blood-nourishing, spleen-invigorating, and liver-soothing herbs, and the third was blood-activating and stasis-resolving herbs. Conclusion: Director Zhao Ke’s treatment of hypomenorrhea due to TE is based on the principle of tonifying the kidney and regulating menstruation, nourishing blood and activating blood circulation, protecting the spleen and stomach, and applying both tonifying and replenishing methods. This approach aims to address the root cause of the disease while also addressing the symptoms, achieving a therapeutic effect that treats both the underlying pathology and the manifest symptoms.展开更多
目的:传统Meta分析证实运动干预可有效改善慢性腰背痛,但哪种特定的运动方式疗效更好目前证据还不够充分。鉴于此,此次研究通过网状Meta分析探讨不同运动要素改善成年人慢性腰背痛影响的差异。方法:检索PubMed、Web of Science、Embase...目的:传统Meta分析证实运动干预可有效改善慢性腰背痛,但哪种特定的运动方式疗效更好目前证据还不够充分。鉴于此,此次研究通过网状Meta分析探讨不同运动要素改善成年人慢性腰背痛影响的差异。方法:检索PubMed、Web of Science、Embase、Cochrane、中国知网、万方、维普数据库中运动干预非特异性慢性腰痛的随机对照试验,其中试验组进行任何形式的运动类型干预,对照组进行药物治疗、物理治疗等非运动干预,检索时限设为各数据库建库至2025-03-01,使用布尔逻辑运算符(AND/OR)组合关键词进行检索。由2位评价员独立完成文献筛选、信息提取及偏倚风险评估后,使用Stata 17.0软件进行网状Meta分析,利用累计排序概率图下面积(SUCRA)对运动剂量变量的影响效果进行排序。结果:①共纳入40项随机对照试验,网状Meta分析结果显示,核心稳定性训练、垫上运动、传统功法运动、组合运动、其他运动改善慢性腰背痛的效果优于对照组[SMD=-0.76,95%CI(-1.39,-0.13),P<0.05;SMD=-1.67,95%CI(-2.48,-0.86),P<0.05;SMD=-2.09,95%CI(-3.37,-0.80),P<0.05;SMD=-1.60,95%CI(-2.71,-0.49),P<0.05;SMD=-1.40,95%CI(-2.40,-0.40),P<0.05],悬吊训练改善慢性腰背痛的效果弱于传统功法运动和垫上运动[SMD=1.50,95%CI(0.05,2.95),P<0.05;SMD=1.09,95%CI(0.11,2.06),P<0.05],传统功法运动改善慢性腰背痛的效果优于核心稳定性训练[SMD=-1.32,95%CI(-2.64,-0.01),P<0.05];单次运动15-20 min、30-40 min的干预效果优于对照组[SMD=-1.96,95%CI(-3.55,-0.36),P<0.05;SMD=-1.44,95%CI(-2.12,-0.76),P<0.05],每周运动3次、六七次的干预效果优于对照组[SMD=-1.03,95%CI(-1.69,-0.37),P<0.05;SMD=-1.83,95%CI(-2.75,-0.91),P<0.05],每周运动六七次的干预效果优于每周运动一二次[SMD=-1.30,95%CI(-2.61,-0.06),P<0.05],运动4周、12-13周和≥16周的干预效果均显著优于对照组[SMD=-0.81,95%CI(-1.50,-0.12),P<0.05;SMD=-1.63,95%CI(-2.82,-0.43),P<0.05;SMD=-2.14,95%CI(-3.36,-0.92),P<0.05],运动≥16周的干预效果显著优于运动6周[SMD=-1.55,95%CI(-3.03,-0.07),P<0.05]。SUCRA结果显示,传统功法运动、单次运动15-20 min、每周运动六七次、运动≥16周在各自对应的因素中排名最高。结论:干预类型为传统功法(太极拳、气功、五禽戏、八段锦)、单次运动15-20 min、每周运动六七次、运动≥16周可能对成年人慢性腰背痛的缓解效果最佳。然而,由于纳入的研究数量有限,需要进一步的研究来提供更有力证据。展开更多
