Objective In 2017,China launched a new round of medical reform(NMR)to address the inaccessibility of high-priced drugs for patients with serious diseases.This study explored the impact of the NMR on the accessibility and affordability of high-priced monoclonal antibodies(mAbs),and the effective promotion policies after the NMR.Methods We used a standard method developed by the World Health Organization to conduct two surveys on the availability of mAbs and their prices before and after the NMR in the public hospitals in Hubei province,China.By interviewing hospital pharmacy experts,we identified the potential value of the current NMR in improving the access to therapeutic mAbs.Results The average availability of 13 mAbs increased by 8.1%in the surveyed hospitals of Hubei province after the NMR.The median unit price of 10 mAbs dropped by 34.3%.The average affordability of a treatment cycle of 10 mAbs dropped from 680 days to 298 days of the disposable daily income for a middle-income resident(56.2%reduction).The drug price negotiation of medical insurance inclusion and the promotion of consistent evaluation of generic and original drugs could effectively promote the accessibility of mAbs.However,the zero markup of drug pricing and the limit on the proportion of drug revenues in public hospitals showed certain negative effects on the availability of mAbs.Conclusion Not all current NMR policies play a positive role in promoting the accessibility of mAbs.To further improve the accessibility of mAbs in the future in China,it is therefore critical to increase the investment in independent research and development of high-quality mAbs,establish localized guidelines for the rational use of mAbs in clinical practice,and have a cost-sharing mechanism for high-priced drugs with multiple stakeholders. 展开更多

Methods: the hospital pharmacy department implemented the traditional management mode ... Objective: to explore the transformation of drug management mode in pharmacy department under the situation of new medical reform. Methods: the hospital pharmacy department implemented the traditional management mode from March to October 2018, changed the management mode in November 2018, and selected November 2018 to June 2019 as the later implementation period. Results: after the implementation, the coverage rate of rechecked prescriptions, administration rate, monthly per capita prescription quantity of pharmacists, clear destination rate and drug use rate were higher than those before the implementation (P < 0.05). After the implementation, the drug expiration rate, drug abuse rate, drug damage rate and audit error rate were lower than those before the implementation (P < 0.05). Conclusion: under the new medical reform situation, the transformation of drug management mode has become an inevitable trend. The new management mode can further strengthen the management of pharmacy department, improve the professional quality and professional ability of pharmacy staff, and provide medication services for patients. 展开更多

Objective:Taking the reform of the Diagnosis-Related Groups(DRG)payment system for medical insurance as an opportunity,we aim to construct a closed-loop management system for lean hospital operations under the trusteeship of urban medical groups,thereby promoting high-quality hospital development.Methods:Led by the Medical Insurance DRG Management Committee,we established a closed-loop MDT-DRG management system,data analysis system,pharmaceutical and consumable management system,discipline construction management system,and standardized medical record front page filling management system.These measures enabled refined operational management,fostering high-quality hospital development.Results:After the trusteeship of Qingdao Municipal Hospital from 2024 to 2025,leveraging the reform of the medical insurance DRG payment system,combined with the integration of medical disciplines and improved performance management,lean operational management effectively achieved six improvements(15.25%year-on-year increase in outpatient and emergency visits,7.62%increase in discharges,50.08%increase in surgeries,71.89%increase in Level IV surgeries,10.76%increase in Case Mix Index,and 45.99%increase in cases with weights>2),two reductions(41.18%decrease in average inpatient drug costs and 28.69%decrease in average outpatient drug costs),and one innovation(78.92%year-on-year increase in Level IV surgeries for national examinations).Conclusion:This effectively promoted the overall operational efficiency and competitiveness of the hospital,laying a solid foundation for the establishment of a tertiary hospital and achieving the goal of satisfaction for the government,hospital,medical insurance,and patients. 展开更多

This paper outlines the internal control management of hospital accounting and finance,introducing its concept,importance,and objectives.It then analyzes the problems existing in the internal control management of hospital accounting and finance,including an unsound management system,insufficient system implementation,inadequate risk assessment and control capabilities,poor information communication,weak internal supervision,and uneven personnel quality.The reasons for these problems are also analyzed,including outdated management concepts,irrational organizational structures,imperfect training and incentive mechanisms,low levels of information technology,and changes in the external environment.This paper proposes improvement measures for the internal control management of hospital accounting,including enhancing the management system,strengthening system implementation,improving risk assessment and control capabilities,enhancing the information communication mechanism,strengthening internal supervision,improving personnel quality,advancing information technology,and adapting to changes in the external environment. 展开更多
