Multimodal Sentiment Analysis(MSA)integrates diverse modalities to identify emotional states,yet performance often suffers in scenarios with missing data.In this situation,despite the promising results of recent metho...Multimodal Sentiment Analysis(MSA)integrates diverse modalities to identify emotional states,yet performance often suffers in scenarios with missing data.In this situation,despite the promising results of recent methods,the failure of part methods to fully exploit the latent valid information contained in incomplete modalities may degrade predictive performance.Besides,to address the oversight of varying contributions across modalities to sentiment understanding,the score-based weighting schemes in the exhibited methods remain overly sensitive to data fluctuations,leading to unstable and unreliable predictions.To this end,we propose a novel method,Data Mining and Uncertainty-Aware with Missing Modalities for Multimodal Sentiment Analysis(DUDF-MSA).In this method,the Common Feature Extraction(CFE)mechanism is introduced to learn common features across modalities,thereby guiding attention toward critical cues in the missing modalities.In parallel,the specific feature uncertainty-aware dynamic adjustment(SFUA)scheme is designed to,in addition to extracting modality-specific features,adaptively assess each modality’s contribution by quantifying its class uncertainty based on the probability distribution of its features.According to these weights,our method can effectively mitigate the negative impact of ambiguous sentiment cues from unreliable modalities during the following feature aggregation stage.Finally,the common features and the adjusted modality-specific features are jointly learned to predict sentiment intensity.The experiments conducted on benchmark datasets indicate the superior performance of DUDF-MSA,which yielding 34.29%Acc-7(1.062 MAE)on MOSI and 35.66%Acc-5(0.505 MAE)on SIMS.展开更多

Objective This study examined the associations between multidimensional body composition modalities and brain aging in Chinese adults.Methods Brain age was estimated using ridge regression based on 24 head computed to...Objective This study examined the associations between multidimensional body composition modalities and brain aging in Chinese adults.Methods Brain age was estimated using ridge regression based on 24 head computed tomographyderived neuroanatomical indicators in a Chinese cohort(n=557).Brain age gap(BAG),the deviation between the predicted brain age and chronological age(CA),was categorized into brain age acceleration(BAG>0)and deceleration(BAG<0)groups.Principal component analysis of 22 correlationindependent body composition indicators identified different body composition modalities.Logistic regression was used to examine the associations between these modalities and the BAG groups.Results The mean absolute error of brain age in predicting CA was 6.41 years.Three body composition modalities were identified:fat mass dominant(characterized by high loading coefficients of body fat mass,fat mass index,visceral fat level,and fat-to-lean mass ratio);fat-free mass dominant;and trunkleg contrast distribution.The fat mass dominant modality was significantly associated with brain age acceleration(odds ratio[OR]=1.40,95%confidence interval[CI]:1.15‒1.71),and the association was robust in sensitivity analyses.Conclusion The fat mass dominant modality was significantly associated with accelerated brain aging.This study suggests integrating deep body composition indicators into clinical and community health screening could aid in targeted prevention of brain aging.展开更多

Objective Traditional Chinese medicine(TCM)incorporates traditional diagnostic methods and several major treatment modalities including Chinese herbal medicine,Chinese patent medicine,and non-pharmacological methods s...Objective Traditional Chinese medicine(TCM)incorporates traditional diagnostic methods and several major treatment modalities including Chinese herbal medicine,Chinese patent medicine,and non-pharmacological methods such as acupuncture and tuina.Even though TCM is used daily by more than 70,000 healthcare facilities and over 700,000 clinical practitioners in China,there is a poor understanding of the extent to which TCM diagnostic methods are used,how different treatment modalities are deployed in general,and what major factors may affect the integration of TCM and Western medicine.This study aimed to fill this void in the literature.Methods In the 2021 National Healthcare Improvement Evaluation Survey,we included three questions gauging the perception and practices of TCM amongst physicians working in TCM-related facilities,investigating the frequency of their deployment of TCM diagnostic methods,and predominant TCM treatment methods.Our empirical analysis included descriptive statistics,intergroup chi-square analysis,and binary logistic regression to examine the association between different types of facilities and individual characteristics and TCM utilization patterns.Results A total of 7618 clinical physicians comprised our study sample.Among them,84.27%have integrated TCM and Western medicine in their clinical practice,and 80.77%of TCM practitioners used the 4 diagnostic methods as a tool in their clinical practice.Chinese herbal medicine was the most widely utilized modality by Chinese TCM physicians(used by 88.49%of respondents),compared with the Chinese patent medicine and non-pharmacological TCM methods,which were used by 73.14%,and 69.39%,respectively.Herbal tea as an out-of-pocket health-maintenance intervention is also a notable practice,recommended by 29.43%of physicians.Significant variations exist across certain institutions,departments,and individual practitioners.Conclusion Given that most of the surveyed physicians integrated TCM with Western medicine in their clinical practices,the practice of“pure TCM”appears to be obsolete in China’s tertiary healthcare institutions.Notably,remarkable variation exists in the use of different TCM modalities across institutions and among individuals,which might be related to and thus limited by the practitioners’experience.Future research focusing on the efficacy and safety of TCM interventions for specific diseases,the development of standardized clinical guidelines,and the enhancement of TCM education and training are called for to optimize TCM-Western medicine integration.展开更多

BACKGROUND Ovarian cancer patients often face complex treatment processes and psychological challenges,with different treatment modalities potentially affecting patients’psychological adjustment abilities.AIM To expl...BACKGROUND Ovarian cancer patients often face complex treatment processes and psychological challenges,with different treatment modalities potentially affecting patients’psychological adjustment abilities.AIM To explore the differences in psychological adjustment patterns among ovarian cancer patients receiving surgery,chemotherapy,targeted therapy,and combined therapy,and to analyze their relationship with clinical outcomes.METHODS A retrospective analysis was conducted on the clinical data of 286 ovarian cancer patients who received different treatment modalities from January 2020 to December 2023.Patients were divided into surgery group(n=78),chemotherapy group(n=65),targeted therapy group(n=61),and combined therapy group(n=82).The Self-Rating Anxiety Scale,Self-Rating Depression Scale,and Psychological Adjustment to Cancer Scale were used to assess psychological status,while quality of life,treatment adherence,and two-year survival rate data were collected.Some patients(n=76)received systematic psychological intervention,and the intervention effects were evaluated.RESULTS Patients in the combined therapy group had significantly higher Self-Rating Anxiety Scale(56.3±7.2)and Self-Rating Depression Scale(58.4±6.9)scores than other groups,with the highest incidence of anxiety(58.5%)and depression(62.2%);the targeted therapy group scored highest in the positive coping dimension(28.5±3.6)and had the lowest incidence of anxiety and depression(29.5%/31.1%).Logistic regression analysis showed that positive coping(odds ratio=2.86,95%confidence interval:1.75-4.68)and utilization of social support(odds ratio=2.13,95%confidence interval:1.42-3.56)were protective factors for good treatment adherence.Longitudinal assessment showed that although all patients experienced increased anxiety and depression symptoms at 3 months of treatment,the targeted therapy group and surgery group showed significant improvement at 6 months(P<0.05),while the combined therapy group showed no significant improvement.Psychological intervention effectively improved patients’treatment adherence(by 22.7%)and quality of life(by 15.6 points),with the best effect in the combined therapy group(anxiety incidence decreased by 30.5%,P<0.001).CONCLUSION Different treatment modalities significantly affect the psychological adjustment abilities of ovarian cancer patients,with combined therapy patients facing greater psychological challenges,while targeted therapy patients exhibit healthier psychological adjustment patterns.展开更多
Background:Post-liver transplantation(LT)hepatocellular carcinoma(HCC)recurrence still occurs in approximately 20%of patients and drastically affects their survival.This study aimed to evaluate the efficacy of various...Background:Post-liver transplantation(LT)hepatocellular carcinoma(HCC)recurrence still occurs in approximately 20%of patients and drastically affects their survival.This study aimed to evaluate the efficacy of various treatments for recurrent HCC after LT in a Chinese population.Methods:A total of 64 HCC patients with tumor recurrence after LT were enrolled in this study.Univariate and multivariate analyses were performed to identify factors affecting post-recurrence survival.Results:Of the 64 patients with recurrent HCC after LT,those who received radical resection followed by nonsurgical therapy had a median overall survival(OS)of 20.9 months after HCC recurrence,significantly superior to patients who received only nonsurgical therapy(9.4 months)or best supportive care(2.4 months).The one-and two-year OS following recurrence was favorable for patients receiving radical resection followed by nonsurgical therapy(93.8%,52.6%),poor for patients receiving only nonsurgical therapy(30.8%,10.8%),and dismal for patients receiving best supportive care(0%,0%;overall P<0.001).Median OS in sorafenib-tolerant patients treated with lenvatinib was 19.5 months,far surpassing the patients that discontinued sorafenib or were treated with regorafenib after sorafenib failure(12 months,P<0.001).Compared with tacrolimus-based immunosuppressive therapy,OS was significantly increased with sirolimus-based therapy at one and two years after HCC recurrence(P=0.035).Multivariate analysis showed radical resection combined with nonsurgical therapy for recurrent HCC and sorafenib-lenvatinib sequential therapy were independent favorable factors for post-recurrence survival.Conclusions:Aggressive surgical intervention in well-selected patients significantly improves OS after recurrence.A multidisciplinary treatment approach is required to slow down disease progression for patients with unresectable recurrent HCC.展开更多

In recent years, given the accumulating evidence that prediabetes is also related to increasing risk of CAD including cardiovascular events, a new guideline has been proposed for the treatment of blood cholesterolfor primary prevention of cardiovascular events. This guideline recommends aggressive lipid-lowering statin therapy for primary prevention in diabetes and other patients. The ultimate goal of patient management is to inhibit progression of systemic atherosclerosis and prevent fatal cardiovascular events such as acute coronary syndrome(ACS). Because disruption of atherosclerotic coronary plaques is a trigger of ACS, the high-risk atheroma is called a vulnerable plaque. Several types of novel diagnostic imaging technologies have been developed for identifying the characteristics of coronary atherosclerosis before the onset of ACS, especially vulnerable plaques. According to coronary angioscopic evaluation, atherosclerosis severity and plaque vulnerability were more advanced in prediabetic than in nondiabetic patients and comparable to that in diabetic patients. In addition, pharmacological intervention by statin therapy changed plaque color and complexity, and the dynamic changes in plaque features are considered plaque stabilization. In this article, we review the findings of atherosclerosis in prediabetes, detected by intravascular imaging modalities, and the therapeutic implications.展开更多

Indeed, human activity recognition pays more attention on the extraction of relevant and discriminative features whose the implementation facilitates the seamless monitoring of functional inhabitant abilities with the involvement of sensing technology in the smart home envi- ronments. However, despite the exponential efforts made by individual standard machine learning techniques, and recently by the remarkable breakthrough of deep learning methods, designing robust activity recognition architecture remains a major challenge in term of performance, due to a high degree of uncertainty and complexity caused by inherent behavior of human actions. For the former case, the drawbacks are essentially composed of heuristic and hand- crafted methods for features extraction, shallow features learning, and learning of low amount of well-labeled data. While the latter suffers from imbalanced datasets and problematic data quality in real-life datasets. In addition, the choice of suitable sensor types is also critical for successful human activity recognition. This paper proposes an ensemble of deep classifier techniques based on hybrid sensor types composed of wearable and environment interactive sensors to improve the prediction and recognition performance of activities of daily living in smart home environments. Indeed, this ensemble is designed by combining the both automatically learned features and hand-crafted features from Denoising Stacked Autoencoders (DSAE) and Random Forest (RF) algorithm respectively. Specifically, the combination involves both the features and out- puts of the two techniques using stacking learning. The use of two public benchmark datasets has enabled to evaluate our approach. Furthermore, the experimental results show the accuracy improvement of the ensembles of deep autoencoders classifiers compared to denoising stacked autoencoder networks and random forest algorithm performed individually. Hence, our approach adaptability to ubiquitous environments and its effectiveness in the recognition of human activity applications.展开更多

The study aimed to describe the baseline clinical-pathological profile and treatment modalities for primary brain tumors in adults treated at the Ocean Road Cancer Institute (ORCI) from 2017 to 2020. Materials and Methods: This was a retrospective study conducted at ORCI by reviewing 61 medical records of patients with primary brain tumors over the age of 15 from January 2017 to December 2020. A structured questionnaire was used to retrieve information on sociodemographic, clinical-pathological characteristics, and treatment modalities. The 2007 WHO classification system and the International Classification of Cancer Diseases (ICD-0-3) were used for classification and diagnosis. The X2 test and Fisher’s exact test were used to compare the proportions and an independent t-test was used to compare the means. A P-value less than 0.05 was deemed statistically significant. The Results: The mean age of the females was 41.8 years and the mean age of males was 42.9 years. Overall M: F ratio was 1:1.2. Meningioma was the only tumor that was more commonly found in women with M:F of 1:2.1. The most prevalent symptom was headache (57.4%). Glioblastoma (GBM) was the most common tumor among adults (38%), followed by astrocytomas (23%) and meningioma (18%). Approximately 91.8% of all tumors occurred in the supratentorial region. The Frontal lobe was the most common site (29.5%). Approximately 81.9% of patients received surgery. The gross tumor resection (GTR) rate was 26.2%, and the subtotal tumor resection (STR) rate was 55.7%. Roughly 18% of the tumors were inoperable. An estimated 80.3% of respondents received radiation therapy. The radiotherapy technique was 3DCRT in two-thirds of the patients and the rest received conventional 2D radiotherapy. The mean equivalent dose in the 2 Gy fractions (EQD2) was 43.9 Gy. Respondents with low-grade intracranial tumors were treated with a mean EQD2 of 47.3 Gy, while those with high-grade intracranial tumors were treated with a mean EQD2 of 44.3 Gy and the difference was statistically significant. Only half of the patients who received adjuvant radiotherapy received it concurrently with chemotherapy. Temozolomide was the most widely used cytotoxic medication. Conclusion: Mean age of the patients was 41 years old. Most tumors were in the supratentorial area and GBM was the most common tumor. Only meningioma was a bit more common amongst females. Overall, radiotherapy doses and the gross tumor resection rates were low. Concurrent chemotherapy with radiotherapy was given to a few patients.展开更多

Competencies have been classified in education according to two approaches--behaviorism and constructivism and a polemic arises when trying to determine which of them prevails in higher education. The Mexican Ministry of Education (SEP) asserts that the constructivist approach prevails in Mexico, while the one behaviorist has considerably diminished in the face-to-face, e-learning, and b-learning modalities. A non-experimental, quantitative, descriptive, and simple cross-sectional research was designed with the aim to analyze the effectiveness of the different educational modalities to develop competencies at the undergraduate schools of the business area of UPAEP. Animals communicate information within their environments via visual,chemical,auditory,and/or tactile modalities.The use of each modalityis generally linked to particular brain regions,but it is not yet known whether ...Animals communicate information within their environments via visual,chemical,auditory,and/or tactile modalities.The use of each modalityis generally linked to particular brain regions,but it is not yet known whether the cellular morphology of neurons in these regions has evolved in association with the relative use of a modality.We investigated relationships between the behavioral use of communication modalities and neural morphologies in six lizard species.Two of these species(Anolis carol#nensis and Leiocephalus carinatus)primarily use visual signals to communicate with conspecifics and detect potential prey,and two(Aspidoscelis gularis and Scincella lateralis)communicate and forage primarily using chemical signals.Two other species(Hemidactylus turcicus and Sceloporus olivaceus)use both visual and chemical signals.For each species,we performed beha-vioral observations and quantified rates of visual and chemical behaviors.We then cryosectioned brain tissues from 9-10 males of each species and measured the soma size and density of neurons in two brain regions associated with visual behaviors(the lat-eral geniculate nucleus and the nucleus rotundus)and one region associated with chemical behaviors(the nucleus sphericus).With analyses conducted in a phylogenetic context,we found that species that performed higher rates of visual displays had a denser lateral geniculate nucleus,and species that used a higher proportion of chemical displays had larger somas in the nucleus sphericus.These relationships suggest that neural morphologies in the brain have evolved convergently in species with similar communication behaviors.展开更多

Prognosis depends on the extent of invasion of adjacent tissues whereby multimodality treatment including surgery with or without adjuvant chemoradiotherapy is the preferred approach for locally advanced thymomas. For metastatic thymomas, only few chemotherapeutic options are available. We report 2 cases of patients with metastatic thymic malignancies with a dramatic response on pemetrexed treatment. The choice for this antifolate therapy is based upon a small series. Because metastatic thymic neoplasm is a rare disease, large randomised trials are not feasible. Case reports on the treatment of these malignancies are very important and can provide readers with the opportunity to deal with rare dis- eases.展开更多
Impaired functional regeneration following tendon-to-bone interface(TBI)injury is a major challenge in sports medicine.The rigidity and limited efficacy of existing rehabilitation strategies remain significant constra...Impaired functional regeneration following tendon-to-bone interface(TBI)injury is a major challenge in sports medicine.The rigidity and limited efficacy of existing rehabilitation strategies remain significant constraints.Physical modalities,leveraging advantages such as non-invasiveness,spatiotemporal controllability,and low immunogenicity,offer effective intervention options for the long-term management of TBI healing.However,conventional physical modalities struggle to address the complex pathological microenvironment involved in TBI healing.In contrast,therapeutic strategies utilizing physical energy-responsive biomaterials enable program-mable,precise,and dynamic regulation,potentially integrating these advantages while overcoming inherent limitations,thereby opening new and effective therapeutic avenues for TBI healing.This review systematically examines the benefits,current applications,and shortcomings of physical modalities for TBI injuries,with particular focus on parameter-response relationships and underlying biological effector mechanisms.Further-more,it summarizes the design strategies and application progress of energy-responsive biomaterials in TBI healing and discusses future directions and promising prospects,aiming to address the core therapeutic chal-lenges in achieving robust TBI healing.展开更多

摘要Multimodal Sentiment Analysis(MSA)integrates diverse modalities to identify emotional states,yet performance often suffers in scenarios with missing data.In this situation,despite the promising results of recent methods,the failure of part methods to fully exploit the latent valid information contained in incomplete modalities may degrade predictive performance.Besides,to address the oversight of varying contributions across modalities to sentiment understanding,the score-based weighting schemes in the exhibited methods remain overly sensitive to data fluctuations,leading to unstable and unreliable predictions.To this end,we propose a novel method,Data Mining and Uncertainty-Aware with Missing Modalities for Multimodal Sentiment Analysis(DUDF-MSA).In this method,the Common Feature Extraction(CFE)mechanism is introduced to learn common features across modalities,thereby guiding attention toward critical cues in the missing modalities.In parallel,the specific feature uncertainty-aware dynamic adjustment(SFUA)scheme is designed to,in addition to extracting modality-specific features,adaptively assess each modality’s contribution by quantifying its class uncertainty based on the probability distribution of its features.According to these weights,our method can effectively mitigate the negative impact of ambiguous sentiment cues from unreliable modalities during the following feature aggregation stage.Finally,the common features and the adjusted modality-specific features are jointly learned to predict sentiment intensity.The experiments conducted on benchmark datasets indicate the superior performance of DUDF-MSA,which yielding 34.29%Acc-7(1.062 MAE)on MOSI and 35.66%Acc-5(0.505 MAE)on SIMS.

摘要In China,the incidence and mortality rates of hepatocellular carcinoma(commonly referred to as liver cancer)are relatively high.To further standardize the surgical treatment of liver cancer,the Liver Surgery Group of Chinese Society of Surgery took the lead in 2000 in formulating and releasing the selection of surgical treatment modalities for primary hepatocellular carcinoma,which was subsequently revised three times in 2004,2009,and 2016.In recent years,with the rapid advancement of medical science and technology,new concepts and treatment modalities have continuously emerged.Based on this,the Liver Surgery Group of Chinese Society of Surgery convened all committee members of the group,along with other leading domestic experts in liver surgery,to revise the fourth edition of the Expert Consensus on the Selection of Surgical Treatment Modalities for Hepatocellular Carcinoma,incorporating the latest advances in surgical treatment of liver cancer,ultimately resulting in the fifth edition of the Expert Consensus on the Selection of Surgical Treatment Modalities for Hepatocellular Carcinoma,intended for reference by physicians engaged in the diagnosis and treatment of liver cancer.
摘要Attempts in the 1970s to outline Ockham’s modalities are helpful as they point out the temporal character of Ockham’s theory of modalities.However,this important insight has been neglected for several decades.Falessi and Schang(2023)is one of the few recent serious attempts to work out a semantic for Ockham’s modalities,resorts to possible worlds semantics and largely disregards temporal aspects.This paper begins with a synopsis of important modern approaches to semantics for Ockham’s modalities,and proceeds with a discussion on methodological problems concerning a representation of medieval logic and a clarification of Ockham’s modal terms.After that,it examines Falessi and Schang’s approach in detail,and assesses it against Ockham’s own texts and understandings.In doing so,we identify what is inadequate in Falessi and Schang’s reconstruction,enabling a return to Ockham’s own texts and with the aid of modern logical tools the development of a new approach to represent Ockham’s understanding of modalities.We ultimately arrive at a time-based relational semantics without branching time for the representation of several key features of Ockham’s modalities.

摘要Objective This study examined the associations between multidimensional body composition modalities and brain aging in Chinese adults.Methods Brain age was estimated using ridge regression based on 24 head computed tomographyderived neuroanatomical indicators in a Chinese cohort(n=557).Brain age gap(BAG),the deviation between the predicted brain age and chronological age(CA),was categorized into brain age acceleration(BAG>0)and deceleration(BAG<0)groups.Principal component analysis of 22 correlationindependent body composition indicators identified different body composition modalities.Logistic regression was used to examine the associations between these modalities and the BAG groups.Results The mean absolute error of brain age in predicting CA was 6.41 years.Three body composition modalities were identified:fat mass dominant(characterized by high loading coefficients of body fat mass,fat mass index,visceral fat level,and fat-to-lean mass ratio);fat-free mass dominant;and trunkleg contrast distribution.The fat mass dominant modality was significantly associated with brain age acceleration(odds ratio[OR]=1.40,95%confidence interval[CI]:1.15‒1.71),and the association was robust in sensitivity analyses.Conclusion The fat mass dominant modality was significantly associated with accelerated brain aging.This study suggests integrating deep body composition indicators into clinical and community health screening could aid in targeted prevention of brain aging.

摘要Multimodal MRI(magnetic resonance imaging)provides critical complementary information for accurate brain tumor segmentation.However,conventional methods struggle when certain modalities are missing due to issues like image quality,protocol inconsistencies,patient allergies,or financial constraints.To address this,we propose a robust single-modality parallel processing framework that achieves high segmentation accuracy even with incomplete modalities.Leveraging Hölder divergence and mutual information,our model maintains modality-specific features while dynamically adjusting network parameters based on available inputs.By using these divergence and information-based loss functions,the framework effectively quantifies discrepancies between predictions and ground-truth labels,resulting in consistently accurate segmentation.Extensive evaluations on the Bra TS 2018 and Bra TS 2020 datasets demonstrate superior performance over existing methods in handling missing modalities,with ablation studies validating each component's contribution to the framework.

摘要Objective Traditional Chinese medicine(TCM)incorporates traditional diagnostic methods and several major treatment modalities including Chinese herbal medicine,Chinese patent medicine,and non-pharmacological methods such as acupuncture and tuina.Even though TCM is used daily by more than 70,000 healthcare facilities and over 700,000 clinical practitioners in China,there is a poor understanding of the extent to which TCM diagnostic methods are used,how different treatment modalities are deployed in general,and what major factors may affect the integration of TCM and Western medicine.This study aimed to fill this void in the literature.Methods In the 2021 National Healthcare Improvement Evaluation Survey,we included three questions gauging the perception and practices of TCM amongst physicians working in TCM-related facilities,investigating the frequency of their deployment of TCM diagnostic methods,and predominant TCM treatment methods.Our empirical analysis included descriptive statistics,intergroup chi-square analysis,and binary logistic regression to examine the association between different types of facilities and individual characteristics and TCM utilization patterns.Results A total of 7618 clinical physicians comprised our study sample.Among them,84.27%have integrated TCM and Western medicine in their clinical practice,and 80.77%of TCM practitioners used the 4 diagnostic methods as a tool in their clinical practice.Chinese herbal medicine was the most widely utilized modality by Chinese TCM physicians(used by 88.49%of respondents),compared with the Chinese patent medicine and non-pharmacological TCM methods,which were used by 73.14%,and 69.39%,respectively.Herbal tea as an out-of-pocket health-maintenance intervention is also a notable practice,recommended by 29.43%of physicians.Significant variations exist across certain institutions,departments,and individual practitioners.Conclusion Given that most of the surveyed physicians integrated TCM with Western medicine in their clinical practices,the practice of“pure TCM”appears to be obsolete in China’s tertiary healthcare institutions.Notably,remarkable variation exists in the use of different TCM modalities across institutions and among individuals,which might be related to and thus limited by the practitioners’experience.Future research focusing on the efficacy and safety of TCM interventions for specific diseases,the development of standardized clinical guidelines,and the enhancement of TCM education and training are called for to optimize TCM-Western medicine integration.

摘要Aging is a persistent topic of interest,with skin aging as its most visible manifestation,characterized by a reduction in intact collagen and elastic fibers in the dermis.Hyaluronic acid,a vital component of the extracellular matrix present in the skin,has become a mainstream method for skin rejuvenation through injections.However,the rapid degradation of pure hyaluronic acid,combined with insufficient maintenance duration and often limited therapeutic effects,presents significant challenges for injectable treatments.Additionally,low patient compliance due to discomfort from needles penetrating the dermal layer further complicates its use.In this review,we summarize and compare existing interventions for skin aging,focusing on strategies to prolong the degradation cycle of hyaluronic acid,including variations in cross-linking modalities and injection techniques.We conclude that the injection of cross-linked modified hyaluronic acid via microneedles represents a promising approach to extend the degradation cycle,offering valuable insights for current therapeutic strategies.
摘要BACKGROUND Ovarian cancer patients often face complex treatment processes and psychological challenges,with different treatment modalities potentially affecting patients’psychological adjustment abilities.AIM To explore the differences in psychological adjustment patterns among ovarian cancer patients receiving surgery,chemotherapy,targeted therapy,and combined therapy,and to analyze their relationship with clinical outcomes.METHODS A retrospective analysis was conducted on the clinical data of 286 ovarian cancer patients who received different treatment modalities from January 2020 to December 2023.Patients were divided into surgery group(n=78),chemotherapy group(n=65),targeted therapy group(n=61),and combined therapy group(n=82).The Self-Rating Anxiety Scale,Self-Rating Depression Scale,and Psychological Adjustment to Cancer Scale were used to assess psychological status,while quality of life,treatment adherence,and two-year survival rate data were collected.Some patients(n=76)received systematic psychological intervention,and the intervention effects were evaluated.RESULTS Patients in the combined therapy group had significantly higher Self-Rating Anxiety Scale(56.3±7.2)and Self-Rating Depression Scale(58.4±6.9)scores than other groups,with the highest incidence of anxiety(58.5%)and depression(62.2%);the targeted therapy group scored highest in the positive coping dimension(28.5±3.6)and had the lowest incidence of anxiety and depression(29.5%/31.1%).Logistic regression analysis showed that positive coping(odds ratio=2.86,95%confidence interval:1.75-4.68)and utilization of social support(odds ratio=2.13,95%confidence interval:1.42-3.56)were protective factors for good treatment adherence.Longitudinal assessment showed that although all patients experienced increased anxiety and depression symptoms at 3 months of treatment,the targeted therapy group and surgery group showed significant improvement at 6 months(P<0.05),while the combined therapy group showed no significant improvement.Psychological intervention effectively improved patients’treatment adherence(by 22.7%)and quality of life(by 15.6 points),with the best effect in the combined therapy group(anxiety incidence decreased by 30.5%,P<0.001).CONCLUSION Different treatment modalities significantly affect the psychological adjustment abilities of ovarian cancer patients,with combined therapy patients facing greater psychological challenges,while targeted therapy patients exhibit healthier psychological adjustment patterns.

摘要Background:Post-liver transplantation(LT)hepatocellular carcinoma(HCC)recurrence still occurs in approximately 20%of patients and drastically affects their survival.This study aimed to evaluate the efficacy of various treatments for recurrent HCC after LT in a Chinese population.Methods:A total of 64 HCC patients with tumor recurrence after LT were enrolled in this study.Univariate and multivariate analyses were performed to identify factors affecting post-recurrence survival.Results:Of the 64 patients with recurrent HCC after LT,those who received radical resection followed by nonsurgical therapy had a median overall survival(OS)of 20.9 months after HCC recurrence,significantly superior to patients who received only nonsurgical therapy(9.4 months)or best supportive care(2.4 months).The one-and two-year OS following recurrence was favorable for patients receiving radical resection followed by nonsurgical therapy(93.8%,52.6%),poor for patients receiving only nonsurgical therapy(30.8%,10.8%),and dismal for patients receiving best supportive care(0%,0%;overall P<0.001).Median OS in sorafenib-tolerant patients treated with lenvatinib was 19.5 months,far surpassing the patients that discontinued sorafenib or were treated with regorafenib after sorafenib failure(12 months,P<0.001).Compared with tacrolimus-based immunosuppressive therapy,OS was significantly increased with sirolimus-based therapy at one and two years after HCC recurrence(P=0.035).Multivariate analysis showed radical resection combined with nonsurgical therapy for recurrent HCC and sorafenib-lenvatinib sequential therapy were independent favorable factors for post-recurrence survival.Conclusions:Aggressive surgical intervention in well-selected patients significantly improves OS after recurrence.A multidisciplinary treatment approach is required to slow down disease progression for patients with unresectable recurrent HCC.
摘要Diabetes mellitus is a powerful risk factor of coronary artery disease(CAD), leading to death and disability. In recent years, given the accumulating evidence that prediabetes is also related to increasing risk of CAD including cardiovascular events, a new guideline has been proposed for the treatment of blood cholesterolfor primary prevention of cardiovascular events. This guideline recommends aggressive lipid-lowering statin therapy for primary prevention in diabetes and other patients. The ultimate goal of patient management is to inhibit progression of systemic atherosclerosis and prevent fatal cardiovascular events such as acute coronary syndrome(ACS). Because disruption of atherosclerotic coronary plaques is a trigger of ACS, the high-risk atheroma is called a vulnerable plaque. Several types of novel diagnostic imaging technologies have been developed for identifying the characteristics of coronary atherosclerosis before the onset of ACS, especially vulnerable plaques. According to coronary angioscopic evaluation, atherosclerosis severity and plaque vulnerability were more advanced in prediabetic than in nondiabetic patients and comparable to that in diabetic patients. In addition, pharmacological intervention by statin therapy changed plaque color and complexity, and the dynamic changes in plaque features are considered plaque stabilization. In this article, we review the findings of atherosclerosis in prediabetes, detected by intravascular imaging modalities, and the therapeutic implications.

摘要Over the past few years, a particular interest has been focused toward activity recognition domain. Indeed, human activity recognition pays more attention on the extraction of relevant and discriminative features whose the implementation facilitates the seamless monitoring of functional inhabitant abilities with the involvement of sensing technology in the smart home envi- ronments. However, despite the exponential efforts made by individual standard machine learning techniques, and recently by the remarkable breakthrough of deep learning methods, designing robust activity recognition architecture remains a major challenge in term of performance, due to a high degree of uncertainty and complexity caused by inherent behavior of human actions. For the former case, the drawbacks are essentially composed of heuristic and hand- crafted methods for features extraction, shallow features learning, and learning of low amount of well-labeled data. While the latter suffers from imbalanced datasets and problematic data quality in real-life datasets. In addition, the choice of suitable sensor types is also critical for successful human activity recognition. This paper proposes an ensemble of deep classifier techniques based on hybrid sensor types composed of wearable and environment interactive sensors to improve the prediction and recognition performance of activities of daily living in smart home environments. Indeed, this ensemble is designed by combining the both automatically learned features and hand-crafted features from Denoising Stacked Autoencoders (DSAE) and Random Forest (RF) algorithm respectively. Specifically, the combination involves both the features and out- puts of the two techniques using stacking learning. The use of two public benchmark datasets has enabled to evaluate our approach. Furthermore, the experimental results show the accuracy improvement of the ensembles of deep autoencoders classifiers compared to denoising stacked autoencoder networks and random forest algorithm performed individually. Hence, our approach adaptability to ubiquitous environments and its effectiveness in the recognition of human activity applications.
摘要Background: The profile of primary brain tumors and treatment modalities employed in Tanzania remains largely unknown. The study aimed to describe the baseline clinical-pathological profile and treatment modalities for primary brain tumors in adults treated at the Ocean Road Cancer Institute (ORCI) from 2017 to 2020. Materials and Methods: This was a retrospective study conducted at ORCI by reviewing 61 medical records of patients with primary brain tumors over the age of 15 from January 2017 to December 2020. A structured questionnaire was used to retrieve information on sociodemographic, clinical-pathological characteristics, and treatment modalities. The 2007 WHO classification system and the International Classification of Cancer Diseases (ICD-0-3) were used for classification and diagnosis. The X2 test and Fisher’s exact test were used to compare the proportions and an independent t-test was used to compare the means. A P-value less than 0.05 was deemed statistically significant. The Results: The mean age of the females was 41.8 years and the mean age of males was 42.9 years. Overall M: F ratio was 1:1.2. Meningioma was the only tumor that was more commonly found in women with M:F of 1:2.1. The most prevalent symptom was headache (57.4%). Glioblastoma (GBM) was the most common tumor among adults (38%), followed by astrocytomas (23%) and meningioma (18%). Approximately 91.8% of all tumors occurred in the supratentorial region. The Frontal lobe was the most common site (29.5%). Approximately 81.9% of patients received surgery. The gross tumor resection (GTR) rate was 26.2%, and the subtotal tumor resection (STR) rate was 55.7%. Roughly 18% of the tumors were inoperable. An estimated 80.3% of respondents received radiation therapy. The radiotherapy technique was 3DCRT in two-thirds of the patients and the rest received conventional 2D radiotherapy. The mean equivalent dose in the 2 Gy fractions (EQD2) was 43.9 Gy. Respondents with low-grade intracranial tumors were treated with a mean EQD2 of 47.3 Gy, while those with high-grade intracranial tumors were treated with a mean EQD2 of 44.3 Gy and the difference was statistically significant. Only half of the patients who received adjuvant radiotherapy received it concurrently with chemotherapy. Temozolomide was the most widely used cytotoxic medication. Conclusion: Mean age of the patients was 41 years old. Most tumors were in the supratentorial area and GBM was the most common tumor. Only meningioma was a bit more common amongst females. Overall, radiotherapy doses and the gross tumor resection rates were low. Concurrent chemotherapy with radiotherapy was given to a few patients.
摘要This paper firstly reviews the characteristics and current situation of business English teaching,especially pointing out the four main problems in practical teaching.Then it introduces the theory of Multimodal Discourse Analysis and its important influence on modern multimedia foreign language teaching.By analyzing a teaching case of the integrated business English course,the author discusses the synergy of different modalities to achieve the teaching goal in the 10 stages of the actual teaching.Finally,it tries to construct a multimodal teaching model of integrated business English from four aspects:expanding multimodal teaching content,exploring multimodal teaching approach,carrying out multimodal practice activity,and improving multimodal evaluation system.
摘要In a highly complex, competitive, and innovative globalized world, the topic of competencies has emerged as means to link universities with the real needs of the firms. But this topic far from being clear is confusing and ambiguous in the specialized literature. Competencies have been classified in education according to two approaches--behaviorism and constructivism and a polemic arises when trying to determine which of them prevails in higher education. The Mexican Ministry of Education (SEP) asserts that the constructivist approach prevails in Mexico, while the one behaviorist has considerably diminished in the face-to-face, e-learning, and b-learning modalities. A non-experimental, quantitative, descriptive, and simple cross-sectional research was designed with the aim to analyze the effectiveness of the different educational modalities to develop competencies at the undergraduate schools of the business area of UPAEP. The results show that a behaviorist approach prevails even if the discourse is constructivist.

摘要Animals communicate information within their environments via visual,chemical,auditory,and/or tactile modalities.The use of each modalityis generally linked to particular brain regions,but it is not yet known whether the cellular morphology of neurons in these regions has evolved in association with the relative use of a modality.We investigated relationships between the behavioral use of communication modalities and neural morphologies in six lizard species.Two of these species(Anolis carol#nensis and Leiocephalus carinatus)primarily use visual signals to communicate with conspecifics and detect potential prey,and two(Aspidoscelis gularis and Scincella lateralis)communicate and forage primarily using chemical signals.Two other species(Hemidactylus turcicus and Sceloporus olivaceus)use both visual and chemical signals.For each species,we performed beha-vioral observations and quantified rates of visual and chemical behaviors.We then cryosectioned brain tissues from 9-10 males of each species and measured the soma size and density of neurons in two brain regions associated with visual behaviors(the lat-eral geniculate nucleus and the nucleus rotundus)and one region associated with chemical behaviors(the nucleus sphericus).With analyses conducted in a phylogenetic context,we found that species that performed higher rates of visual displays had a denser lateral geniculate nucleus,and species that used a higher proportion of chemical displays had larger somas in the nucleus sphericus.These relationships suggest that neural morphologies in the brain have evolved convergently in species with similar communication behaviors.
摘要Thymomas are rare and usually slowly growing tumors, originating from the epithelial layer of the thymus. Prognosis depends on the extent of invasion of adjacent tissues whereby multimodality treatment including surgery with or without adjuvant chemoradiotherapy is the preferred approach for locally advanced thymomas. For metastatic thymomas, only few chemotherapeutic options are available. We report 2 cases of patients with metastatic thymic malignancies with a dramatic response on pemetrexed treatment. The choice for this antifolate therapy is based upon a small series. Because metastatic thymic neoplasm is a rare disease, large randomised trials are not feasible. Case reports on the treatment of these malignancies are very important and can provide readers with the opportunity to deal with rare dis- eases.

摘要Impaired functional regeneration following tendon-to-bone interface(TBI)injury is a major challenge in sports medicine.The rigidity and limited efficacy of existing rehabilitation strategies remain significant constraints.Physical modalities,leveraging advantages such as non-invasiveness,spatiotemporal controllability,and low immunogenicity,offer effective intervention options for the long-term management of TBI healing.However,conventional physical modalities struggle to address the complex pathological microenvironment involved in TBI healing.In contrast,therapeutic strategies utilizing physical energy-responsive biomaterials enable program-mable,precise,and dynamic regulation,potentially integrating these advantages while overcoming inherent limitations,thereby opening new and effective therapeutic avenues for TBI healing.This review systematically examines the benefits,current applications,and shortcomings of physical modalities for TBI injuries,with particular focus on parameter-response relationships and underlying biological effector mechanisms.Further-more,it summarizes the design strategies and application progress of energy-responsive biomaterials in TBI healing and discusses future directions and promising prospects,aiming to address the core therapeutic chal-lenges in achieving robust TBI healing.
