Objective:This study aimed to develop and validate a temporal radiomics model based on pre-and post-treatment CT scans for the preoperative prediction of pathologic complete response(pCR)in patients with resectable non-small cell lung cancer(NSCLC)undergoing neoadjuvant chemoimmunotherapy(NCI).Methods:Data from 263 patients with resectable NSCLC who underwent NCI followed by curative surgery and had both pre-and post-treatment CT scans were retrospectively collected.Patients from one hospital were randomly divided into training and internal test sets at a 7:3 ratio,while patients from two other hospitals served as the external test set.Radiomics features were extracted from the CT scans at both timepoints and delta features capturing the temporal changes were calculated.Radiomics models based on different features were developed using the least absolute shrinkage and selection operator for feature selection,followed by logistic regression.Model performance was evaluated using the area under the curve(AUC).Results:The radiomics model based on delta features yielded AUCs of 0.85,0.76,and 0.72 in the training,internal test,and external test sets,respectively,which were superior to the radiomics models based on pre-treatment features(0.74,0.66,and 0.62,respectively)and post-treatment features(0.80,0.76,and 0.65,respectively).By integrating the optimal features from all three feature sources,the combined model achieved further improvements in performance,with AUCs of 0.89,0.85,and 0.78,respectively,across the three sets.Conclusions:A CT-based radiomics model incorporating temporal features from pre-and post-treatment scans shows favorable performance for the non-invasive preoperative estimation of pCR to NCI in patients with NSCLC. 展开更多

Objective:To characterize placental morphologic features in Moroccan women with adverse outcomes,across different clinical contexts,based on the Amsterdam consensus classification.Methods:A prospective analysis was conducted on placentas with umbilical cords collected fresh between March 1,2024 and July 15,2024 from women with adverse pregnancy outcomes.Clinical data(age,parity,gravidity,complications)were retrieved.Macroscopic parameters(weight,dimensions,cord insertion,membranes,lesions)were assessed,followed by systematic sampling.Tissue was processed by standard histology(formalin fixation,paraffin embedding,hematoxylin and eosin staining),and lesions were classified per Amsterdam criteria.Results:16 placentas from patients with adverse pregnancy outcomes were included.The median maternal age was 30 years.Adverse conditions included placental abruption(50%),intrauterine growth restriction(IUGR,38%),intrauterine fetal death(IUFD,31%),pre-eclampsia/eclampsia(19%),premature rupture of membranes(13%),and oligohydramnios(13%).Several placentas were associated with more than one adverse condition.Histopathology revealed maternal vascular malperfusion lesions in 94%,particularly in pre-eclampsia,IUGR,and IUFD.Fetal vascular malperfusion was found in 88%,mainly in IUGR and IUFD.Inflammatory lesions,dominated by acute maternal and fetal responses stage 3(necrotizing chorioamnionitis and funisitis),were primarily linked to IUFD.Conclusions:Placental examination enhances understanding of the pathophysiology underlying adverse pregnancy outcomes,supports diagnostic confirmation,and guides preventive strategies for recurrence.This study highlights the prevalence of maternal vascular malperfusion in Moroccan women and emphasizes the importance of systematic placental histopathology in obstetric care. 展开更多

Objective To develop a prognostic prediction model for early-stage triple-negative breast cancer(TNBC)using H&E-stained pathological images and to investigate its underlying biological interpretability.Methods A deep learning model was trained on 340 WSIs and externally validated using 81 TCGA cases.Image-derived features extracted through convolutional neural networks were integrated with clinicopathological variables.Model performance was assessed using ROC curve analysis,and interpretability was evaluated by correlating image features with mRNA-seq data and characteristics of the immune microenvironment.Results The model achieved AUCs of 0.86 and 0.75 in the training and validation cohorts,respectively.Analysis using HoVer-Net indicated that lymphocyte abundance was associated with recurrence risk.Texture-related features showed significant correlations with immune cell infiltration and prognostic gene expression profiles.Conclusion This study demonstrates that deep learning can enable accurate prognostic prediction in early-stage TNBC,with interpretable image features that reflect the tumor immune microenvironment and gene expression profiles. 展开更多

Intelligent speech technology,rooted in the ancient medical practice of“auscultation and interrogation”,is emerging as a transformative tool in modern pathological healthcare.By analyzing acoustic biomarkers within speech,voice,cough,breath sounds,and heart sounds,it offers a non-invasive,cost-effective avenue for early screening,auxiliary diagnosis,monitoring,and rehabilitation assessment across a wide spectrum of conditions,including mental disorders,neurodegenerative diseases,respiratory illnesses,cardiovascular diseases,and laryngeal or vocal tract pathologies.This study comprehensively reviews the research progress and prevailing trends in this interdisciplinary field.It begins by elucidating the physiological basis of pathological acoustics,including neural dysregulation,airway and pulmonary abnormalities,hemodynamic disturbances,and laryngeal or vocal tract dysfunction,and then discusses its integration with AI-driven diagnostics.The core of the review systematically details advances in two pillars:data resource construction(encompassing datasets for various diseases and standardization efforts)and methodological innovation(tracking the paradigm shift from feature-based machine learning to deep learning,self-supervised models,and multimodal large language models).Furthermore,it explores the development of intelligent speech-driven intervention systems for mental health.The analysis identifies key dynamic trends:the evolution from single-modality to multimodal analysis,the shift from strong to weak/self-supervised learning,the transition from controlled lab settings to naturalistic scenarios,the growing priority of model interpretability,and the move towards multi-disease coexistence modeling.Despite promising clinical potential,significant challenges persist,including data scarcity,algorithmic robustness,and clinical integration bottlenecks.The study concludes by outlining critical future directions:fostering federated learning and multi-center validation,enhancing explainable AI fused with medical knowledge,improving cross-device and cross-environment robustness through hardware-software co-design,refining human-AI collaborative diagnostic paradigms,and establishing comprehensive standardization and regulatory frameworks.Overcoming these hurdles through concerted interdisciplinary efforts is essential to realize a full-cycle intelligent health ecosystem,advancing precision medicine and equitable healthcare delivery. 展开更多

BACKGROUND Combined hepatocellular-cholangiocarcinoma(cHCC-CCA)is a rare and aggressive primary liver cancer characterized by the presence of both hepatocellular and cholangiocellular differentiation within the same tumor.Its diagnostic complexity and low incidence have resulted in a scarcity of well-established prognostic pathological parameters to guide clinical management.Given the rising global prevalence of metabolic dysfunction-associated steatotic liver disease and alcohol-related liver disease,and their close association with the occurrence of primary liver cancer,it is plausible that hepatic steatosis influences the prognosis of cHCC-CCA.AIM To investigate the specific clinicopathological role of background hepatic steatosis in patients with cHCC-CCA who underwent curative-intent liver resection.METHODS This multicenter study analyzed 310 patients with cHCC-CCA who underwent curative-intent hepatectomy between 2013 and 2017.Hepatic steatosis in liver tissue was graded pathologically:Grade 0-1(2 years)RFS.RESULTS Patients with severe hepatic steatosis had a significantly prolonged RFS compared to those with negligible steatosis(P=0.035),with 5-year RFS rates of 28.8% vs 15.7%.This benefit was primarily observed in early RFS(P=0.024)and was more pronounced in non-cirrhotic patients.In contrast,no statistically significant difference was found in overall survival(P=0.586)or late RFS(P=0.931)between the two groups.Multivariate logistic regression analysis showed higher body mass index and lymphocyte count,and lower lactate dehydrogenase were independent predictors for the presence of severe steatosis.Furthermore,multivariate Cox regression analysis confirmed severe hepatic steatosis as an independent protective factor for RFS(hazard ratio=0.614)and early RFS(hazard ratio=0.577).CONCLUSION Severe hepatic steatosis is a protective factor associated with RFS and early RFS in patients with cHCC-CCA who underwent hepatectomy,which is especially significant in the absence of liver cirrhosis. 展开更多

BACKGROUND Helicobacter pylori(H.pylori)is widely present in the human gastric mucosa and is closely associated with a variety of gastric diseases.Recent studies have found that H.pylori infection is closely associated with diabetic patients and may adversely affect their glucose metabolism and organ function.However,the effect of H.pylori infection on pathological changes in the body during a diabetic state remains unclear.AIM To investigate the effects of H.pylori infection on the physiology and pathological changes in the organs of diabetic mice.METHODS The diabetic mice models were established using streptozotocin(STZ).The mice were infected with H.pylori through oral gavage,with their fasting blood glucose(FBG)and body weight monitored dynamically over a period of 1 to 13 months after infection.Pathological changes in major organs(including the pancreas,stomach,liver,and kidneys)were assessed,along with apoptosis levels in these tissues.The expression of H.pylori virulence factors in the liver and alterations in the intestinal microbiota were also analyzed.RESULTS H.pylori infection led to significant fluctuations in FBG in diabetic mice from the 1st to 9th month,with FBG levels remaining consistently elevated.Body weight increased gradually but remained significantly lower than that of both uninfected diabetic mice and non-diabetic controls.Pancreatic islet cell numbers decreased,accompanied by persistent inflammation and tissue damage for over 9 months.H.pylori colonized the stomach for at least 7 months,causing irreversible gastric mucosal inflammation;by the 13th month,diffuse dense inflammatory infiltration occupying the entire submucosal layer was observed.Progressive damage was observed in liver and kidney tissues,with marked expression of H.pylori virulence factors in the liver by the 9th month.Additionally,significant gut microbiota dysbiosis was observed.CONCLUSION The STZ-induced diabetic mouse model with H.pylori infection can significantly prolong the colonization time of H.pylori in the stomach and exacerbate the degree of damage to the stomach,liver,and kidney organs. 展开更多

Knee osteoarthritis(KOA)is a chronic degenerative disease.Monosodium iodoac-etate(MIA)induction is the most commonly used therapeutic effect evaluation and mechanism of action research model;we observed a lack of standardization and uni-formity in current model building methods,which led us to conduct this study.Background:The aim was to investigate the time-and dose-related changes in the behavioral and pathological characteristics in the MIA-induced KOA model rat.Methods:MIA(40,50,and 60 mg/mL)was injected into the left joint of male Sprague-Dawley rats.After 2 weeks,the changes in the KOA rat model were observed by be-havioral evaluation,imaging-level evaluation,and histological-level evaluation.The changes were also compared after 40-mg/mL MIA injection for 2 and 6 weeks.Results:MIA-induced bone surface defects,osteophyte hyperplasia around the artic-ular rim,increased subchondral bone density,thinning of the sparse trabecular bone,structural disorder,and local clustering were observed.The degree of injury gradually increased with the increase in MIA concentration.After 6 weeks,subchondral bone density and sparse trabecular bone increased in the KOA model.Conclusions:The severity of the model also increased significantly with the changes in dose and time.In dose-dependent experiments,this study revealed that 40 mg/mL was the optimal dose to induce significant pathological changes without causing undue discomfort or death in animals.This dose may induce pathological changes stably and is suitable for long-term observation. 展开更多

BACKGROUND Inappropriate selection of patients with early gastric cancer(EGC)for endoscopic submucosal dissection(ESD)may lead to non-curative resection,necessitating additional gastrectomy.Conversely,inappropriate selection for gastrectomy may result in overtreatment,adversely affecting patients’quality of life.Few have systematically evaluated the concordance between therapeutic indications under current Japanese guidelines and pathological criteria in EGC.To minimize noncurative resection risks while sparing unnecessary surgery for low-risk patients’,we specifically assess the suitability of Japanese guidelines in non-Japanese populations.This work aims to optimize clinical practice by refining endoscopic treatment criteria for adoption beyond Japan.AIM To evaluate EGC clinical decision accuracy by comparing therapeutic indication with postoperative pathological criteria and analyzing factors influencing discrepancies.METHODS A retrospective analysis was conducted on 796 EGC cases diagnosed at Peking University Third Hospital between January 2010 and December 2022.Cases were categorized into three groups:Same-estimated(preoperative therapeutic indication with postoperative pathological criteria matched),underestimated(preoperative ESD indication but postoperative surgical criteria),and overestimated(preoperative surgical indication but postoperative ESD criteria).The rate of discrepancy and associated risk factors were assessed.RESULTS The accuracy rates of preoperative evaluation for ESD and gastrectomy indications were 73.0%(321/430)and 76.0%(278/366),respectively.The overall discrepancy rate was 25.6%(204/796).Multivariate analysis identified tumor location in the upper-third stomach(odds ratio=2.158,95%confidence interval:1.373-3.390,P=0.001)was significantly associated with a higher likelihood of being underestimated and undifferentiated histologic type on preoperative biopsy(odds ratio=2.005,95%confidence interval:1.036-3.879,P=0.039)was more likely to be overestimated.Significant differences were observed in tumor diameter(P<0.001),depth of infiltration(P<0.001),ulcerative findings(P<0.001),and histologic type(P<0.001)between preoperative and postoperative evaluations.CONCLUSION The accuracy of preoperative EGC indications is 74.4%.Upper-third stomach and undifferentiated histology are primary discrepancy predictors.Upper-third tumors are prone to underestimation,while undifferentiated tumors are prone to overestimation. 展开更多

Neoadjuvant therapy(NAT)has become the standard treatment for patients with locally advanced breast cancer and stage II-III HER2-positive(HER2+)or triple-negative breast cancer(TNBC)1,2.It is essential to accurately mark the primary breast tumor and positive axillary lymph nodes(ALNs)prior to NAT to ensure precise surgical excision,guide axillary downstaging,and guarantee reliable lesion retrieval for pathologic evaluation3.The false-negative rate of sentinel lymph node biopsy(SLNB)after NAT can be reduced to<10%by applying modalities,such as the identification of≥3 sentinel lymph nodes(SLNs)with dual-mapping techniques or removal of the marked lymph node with target axillary dissection(TAD)according to the ASCO,NCCN,and CBCS guidelines3-5.However,there is a lack of consensus regarding the optimal methods and materials for accurate marking6,7.Conventional techniques include clip placement,guidewire localization,and carbon or ink tattooing,whereas wireless technologies,such as MagseedR,radiofrequency identification tags,SAVI SCOUTR,and radioactive iodine-125(125I)seeds,have also been adopted.Traditional marking techniques have a localization failure rate of approximately 10%.In contrast,the use of 125I seeds(with a radiation dose of 0.1-0.3 mCi)has significantly improved localization accuracy8,9.Nevertheless,owing to radioactive properties,concerns have been raised regarding the potential impact of 125I seed marking on assessing the pathologic complete response(pCR)after NAT10.Moreover,whether the influence of 125I seed marking on pCR could lead to suboptimal adjuvant treatment decisions and potentially compromise long-term oncologic outcomes has not been established.To investigate the potential impact of 125I seed placement on the pCR rate and long-term outcomes in breast cancer patients receiving NAT,we conducted a retrospective cohort study utilizing propensity score matching(PSM). 展开更多

BACKGROUND Monomorphic epitheliotropic intestinal T-cell lymphoma(MEITL)is a rare,aggressive T-cell lymphoma involving the gastrointestinal tract,which was previously classified as enteropathy-associated T-cell lymphoma type II(EATL II).AIM To improve the understanding and diagnostic accuracy of MEITL,this study analyzed its clinical,endoscopic,and pathological characteristics.METHODS Patients who were diagnosed with MEITL or EATL II at Peking Union Medical College Hospital between August 2012 and June 2025 were retrospectively enrolled.Additionally,cases with confirmed diagnoses of EATL II or MEITL were retrieved from PubMed and Web of Science.Clinical symptoms,medical history,auxiliary examinations,endoscopic and pathological findings were collected for all included patients.RESULTS Among the 26 MEITL patients in Peking Union Medical College Hospital,the most common symptoms were weight loss(73%)and abdominal pain(65%),with 96%showing small intestinal involvement.Endoscopic findings in 10 patients showed mucosal edema(100%),roughness(100%),and villous blunting(90%).A review of 130 published MEITL cases revealed similar clinical profiles but heterogeneous endoscopic features.Ulceration(66%)was the most frequent in 64 cases with available data.Integrated analysis of 156 patients,stratified by a prediagnostic surgical history,showed nonsurgical group more frequently presented with diarrhea and weight loss(P<0.001);surgical group had higher incidence of bowel perforation(P<0.001)and higher expression of T-cell intracellular antigen 1 and granzyme B(P<0.05),indicating aggressiveness.CONCLUSION Recognizing the characteristic symptoms and endoscopic features of MEITL is essential for improving the diagnostic accuracy and early identification of this rare and aggressive lymphoma. 展开更多

The core lethal complications of liver cirrhosis,particularly portal hypertension and hepatic decompensation,are closely linked to dysregulated pathological angiogenesis.Aberrant neovascularization is primarily driven by the hypoxiainducible factor-vascular endothelial growth factor(VEGF)axis.Activated hepatic stellate cells release key factors like VEGF and platelet-derived growth factor,promoting endothelial cell proliferation.Inflammatory-oxidative stress signals(e.g.,tumor necrosis factor-alpha/interleukin-6 and non-alcoholic fatty liver disease oxidase-derived reactive oxygen species)amplify this process via the nuclear factor kappa-B pathway.Activation of the local renin-angiotensin system also exacerbates vascular leakage and malformation through pathways like angiotensin II.Preclinical studies demonstrate that drugs targeting these pathways(e.g.,the anti-VEGF agent bevacizumab,the multi-kinase inhibitor sorafenib)can effectively inhibit pathological angiogenesis.However,their clinical translation faces three major challenges:(1)Intrahepatic vascular heterogeneity limits targeting efficiency;(2)Activation of compensatory pathways(e.g.,FGF/Notch)leads to treatment resistance;and(3)Systemic administration carries off-target risks.Future breakthroughs hinge on developing biomarkers based on single-cell sequencing,applying novel models like the“hepatic hypertension-chip”,and implementing multi-target combination therapies. 展开更多

The management of small rectal neuroendocrine tumors(NETs)with positive vertical margins after endoscopic resection remains controversial.In light of the recent study by Qiao et al published in World Journal of Clinical Oncology,which reported excellent oncologic outcomes following both endoscopic submucosal dissection(ESD)and hybrid ESD for stage 1 rectal NETs,we provide a pathology-centered reinterpretation of margin positivity.Despite multiple non-R0 resections,predominantly due to positive vertical margins,no patients developed local recurrence or metastasis.From a histopathological perspective,apparent margin involvement is frequently attributable to technical artifacts such as thermal injury,tissue distortion,tangential sectioning,or specimen fragmentation rather than true residual viable tumor.Moreover,the intrinsic biological indolence of small,well-differentiated(G1)rectal NETs further explains the low risk of recurrence.Together,these observations support a risk-stratified,surveillance-oriented management strategy and underscore the importance of nuanced pathological interpretation to avoid unnecessary salvage surgery while maintaining oncologic safety. 展开更多

Blood-tissue barriers are specialized interfaces that safeguard organ homeostasis by restricting pathogen dissemination.Zika virus(ZIKV),an emerging flavivirus of global concern,exhibits an exceptional ability to breach multiple barriers-including the blood-brain,blood-placental,blood-testis,and blood-retinal barriers-enabling neuroinvasion,vertical and sexual transmission,and ocular disease.ZIKV employs diverse strategies to cross these barriers:receptor-mediated entry,disruption of tight junctions,and hijacking immune cells or extracellular vesicles as viral carriers.Adaptive mutations further refine tissue tropism and enhance barrier traversal efficiency.Insights from cell culture,organoid,animal,and ex vivo tissue models reveal not only the conserved and tissue-specific mechanisms of barrier penetration but also the downstream pathological consequences in the affected organs.Understanding how ZIKV breaches these interfaces and induces organ-specific pathology deepens our knowledge of host-pathogen interactions and provides a framework for designing barrier-protective and disease-mitigating strategies against ZIKV and other pathogens that breach blood-tissue barriers. 展开更多

Neuromyelitis optica spectrum disorder-related optic neuritis involves various cellular responses to inflammation and degeneration.In most patients,the primary mechanism underlying neuromyelitis optica spectrum disorder-related optic neuritis is the interaction of aquaporin-4 antibodies with the aquaporin-4 protein present on astrocytes within posterior optic nerve.This binding subsequently initiates a cascade of events leading to secondary demyelination of the optic nerve,ultimately culminating in optic nerve degeneration.Earlier studies on this disorder primarily used systemic-induced animal models,which often require prior activation of a systemic immune response.This can result in primary demyelination of the optic nerve,complicating the interpretation of experimental results.Such methodologies hinder the ability to isolate immune responses triggered by specific antibodies.Additionally,the lack of a detailed profile of disease progression over time limits our capacity to identify potential intervention windows.Therefore,constructing a targeted optic neuritis animal model induced by specific antibodies and elucidate the disease progression arecrucial for exploring the mechanisms underlying neuromyelitis optica spectrum disorder-related optic neuritis.In this study,specific antibodies against aquaporin-4 were precisely injected into the retrobulbar optic nerve of mice to induce a targeted inflammatory response in the posterior optic nerve,resulting in a more representative mouse model of neuromyelitis optica spectrum disorder-related optic neuritis than current models.The progression of the disease was then dynamically observed from both histological and functional perspectives over the course of 1 month following the induction of inflammation.By the first week,astrocytes were damaged,as evidenced by the loss of aquaporin-4 and glial fibrillary acidic protein,the activation of microglia,and the upregulation of microglia-related cytokines,including tumor necrosis factor,interleukin-6,interleukin-1β,C-X-C motif ligand 10,and brain-derived neurotrophic factor.Starting from the second week,there were signs of optic nerve demyelination and significant damage to axonal fibers and retinal ganglion cell bodies.Visual-evoked potentials and dark adaptation threshold responses in electroretinogram both indicated dysfunction in the visual pathway and retina,while optical coherence tomography revealed thinning of the retinal nerve fiber layer in live mice.In summary,in this study we conducted a dynamic exploration of the occurrence and progression of neuromyelitis optica spectrum disorder-related optic neuritis triggered by specific antibodies.Our results show pathological changes at various stages and correlate histological and molecular alterations with in vivo structural and functional deterioration.The findings from this study lay an important foundation for further research on neuromyelitis optica spectrum disorder-related optic neuritis. 展开更多

●AIM:To investigate macular changes characteristics among adults with high myopia in Egypt,and to evaluate the relationship between the degree of myopia and the extent of macular alterations.●METHODS:A series of hig... ●AIM:To investigate macular changes characteristics among adults with high myopia in Egypt,and to evaluate the relationship between the degree of myopia and the extent of macular alterations.●METHODS:A series of highly myopic eyes with axial length(AL)≥26.0 mm and/or spherical equivalent(SE)≤−6.0 D were included.All subjects underwent automated refraction,correction of refractive error,AL measurement,and optical coherence tomography(OCT)for macular scanning.Myopic maculopathy(MM)for each eye was recorded,and their association with age,refractive error,and AL was determined.●RESULTS:Among the 427 eyes(mean SE:-11.11±4.34 D,mean AL:27.84±1.66 mm)of 250 subjects(77 males,mean age 56.84±12.19y),all had tigroid fundus(A1,ATN classification of MM),associated macular lesions were chorioretinal atrophy[C-R atrophy;8.47%diffuse C-R atrophy(A2),11.0%patchy C-R atrophy(A3)].Neovascular MM was:lacquer cracks in(N1)1.64%,myopic choroidal neovascularization[mCNV;6.09%active CNV(N2a),19.20%choroidal neovascular membrane(N2b)],myopic traction maculopathy(MTM)in 11.48%,macular hole(MH;3.98%lamellar MH,2.34%full thickness MH).Older age was associated with MTM,MH,and lacquer cracks(P=0.004,P=0.025,P=0.013).More myopic SE was associated with increased odds of atrophic MM,neovascular MM,and MTM(P<0.001,P<0.001,P=0.002).Increased AL was associated with increased odds of atrophic MM,neovascular MM,and dome shape macula(P=0.049,P=0.037,P=0.001).In the multivariable linear mixed model with adjustment of age,gender and SE:the presence of atrophic MM(P<0.001),neovascular MM(P<0.001)were significantly associated with poorer best corrected visual acuity(BCVA).●CONCLUSION:Among adult Egyptian patients with high myopia:age,more myopic SE and longer AL are associated with increased odds of MM.Atrophic or neovascular MM is associated with poorer visual acuity. 展开更多

BACKGROUND Soft tissue sarcomas(STSs)of the lower extremities are rare malignancies increasingly treated with limb-sparing surgery combined with radiotherapy(RT).While this approach provides excellent oncologic control and preserves function,it is associated with late skeletal complications.Postoperative pathologic fractures(PPFs)represent a serious and potentially disabling outcome,often requiring additional surgical intervention and impairing quality of life.Reported fracture rates and associated risk factors vary widely,and contemporary data incorporating modern surgical and radiotherapeutic techniques remain limited.AIM To identify independent predictors of PPFs following limb-sparing surgery for lower extremity STS.METHODS This retrospective cohort study included 266 patients with intermediate-or highgrade lower extremity STS treated with limb-sparing surgery between 2010 and 2020.Clinical,tumor-related,surgical,and treatment variables were collected.PPF was defined as a low-energy fracture occurring within the surgical field.Cumulative fracture incidence was estimated using Kaplan-Meier analysis.Risk factors were assessed using univariable analyses and multivariable Cox regression with Firth’s penalized likelihood.RESULTS During a median follow-up of 48.9 months,23 postoperative pathological fractures occurred.The cumulative fracture incidence was 1.8%at 1 year,13.0%at 5 years,and 20.6%at 10 years.Fractures most frequently involved the femur.In univariable analyses,older age,arterial hypertension,larger tumor size,RT,and bone-involving surgical procedures were associated with increased fracture risk.In multivariable analysis,bone resection and osteoporosis remained independently associated with PPF,whereas RT,age,and arterial hypertension were not statistically significant.Tumor size was not included in the multivariable model due to lack of significance in univariable analysis but remains clinically relevant.CONCLUSION PPFs are a relevant late complication following limb-sparing surgery for STS.Fracture risk is primarily associated with bone-involving surgical procedures and osteoporosis,whereas RT shows an elevated but not statistically significant association. 展开更多

Correlation of systemic immune-inflammation and prognostic nutritional indices with pathological characteristics and prognosis in gastric cancer 认领 引用

BACKGROUND An ongoing need persists to identify reliable biological markers for gastric cancer(GC)prognosis,clinicopathological stratification,and survival prediction.AIM To determine the correlation of the Systemic Immune-Inflammation Index(SII)and Prognostic Nutritional Index(PNI)with clinicopathological characteristics and prognosis in patients with GC.METHODS This study included 107 patients with GC admitted to Inner Mongolia Baogang Hospital between February 2020 and February 2022.Clinical data including SII,PNI,and three-year prognosis were collected to evaluate the potential correlation of SII and PNI with the clinicopathological characteristics in patients with GC.Among the studied patients,receiver operating characteristic curve analysis assessed the prognostic predictive value of SII and PNI,and univariate and Cox multivariate regression analyses identified independent prognostic factors.RESULTS In patients with GC,SII levels were closely associated with maximum tumor diameter,invasion depth,metastases to lymph nodes,distant metastasis,tumornode-metastasis staging,and carbohydrate antigen 199,and PNI levels were closely associated with differentiation degree,invasion depth,distant metastasis,tumor-node-metastasis staging,and carbohydrate antigen 199.The poor prognosis group(n=37)exhibited notably higher SII levels and lower PNI levels relative to the good prognosis group(n=70).Survival curve analysis revealed that in GC patients,high SII levels(≥607)were significantly correlated with lower three-year overall survival,while low PNI levels(<44.5)were significantly correlated with lower three-year overall survival.According to the receiver operating characteristic curve,the area under the curve values for predicting GC prognosis using SII and PNI were 0.816 and 0.768,respectively,and the area under the curve of the two combined indices reached 0.866.Univariate and multivariate analyses showed that metastases to lymph nodes,distant metastasis,SII,and PNI were independent factors influencing three-year prognosis in patients with GC.CONCLUSION SII and PNI were effective biomarkers for predicting GC prognosis,and the two combined markedly improved predictive efficacy. 展开更多
