Background:Advances in intensive care have markedly improved survival after severe brain injury,leading to a growing population of patients with prolonged disorders of consciousness(pDOC).Current management of pDOC re...Background:Advances in intensive care have markedly improved survival after severe brain injury,leading to a growing population of patients with prolonged disorders of consciousness(pDOC).Current management of pDOC remains largely supportive,and evidence-based neuromodulatory interventions are limited;moreover,existing guidelines provide insufficiently explicit recommendations regarding mechanisms of action and objective biomarkers of treatment response.Transcutaneous auricular vagus nerve stimulation(taVNS)has emerged as a potential noninvasive intervention;however,its modulatory effects on brain function in pDOC are not yet well characterized,and the paucity of integrative mechanistic evidence has constrained its translation into routine clinical practice.Objectives:Within a multimodal assessment framework,this study aims to systematically elucidate the neurobiological mechanisms by which taVNS modulates brain function and autonomic activity in patients with pDOC,and to evaluate its clinical potential to enhance levels of consciousness.Methods:In this randomized controlled trial,60 patients with vegetative state/minimally conscious state will be enrolled and randomly allocated to a taVNS group,a transcutaneous nonauricular vagus nerve stimulation group(sham),or a control group(n=20 per group)for a 4-week intervention.The primary outcome will be changes in the Coma Recovery Scale-Revised scores from baseline to weeks 1,2,and 4 of treatment.Secondary outcomes will include functional brain activity assessed by electroencephalography and functional near-infrared spectroscopy,as well as autonomic modulation indexed by heart rate variability.Functional prognosis will be evaluated using the Glasgow Outcome Scale-Extended at the end of treatment and at a 6-month follow-up.Safety will be assessed by continuous monitoring and documentation of adverse events throughout the study period.Results and Discussion:By integrating electroencephalography–functional near-infrared spectroscopy with heart rate variability,this study will characterize the effects of taVNS on functional brain networks and consciousness recovery in pDOC across complementary behavioral,electrophysiological,hemodynamic,and autonomic domains,while interrogating potential sources of clinical and neurobiological heterogeneity.The findings are expected to provide a mechanistic and evidence-based foundation for the mechanism-driven clinical implementation of taVNS and the optimization of stimulation protocols in pDOC.Clinical Trial Registration:International Traditional Medicine Clinical Trial Registry,ITMCTR20250021041,http://gffzz299adcccaa8d4ab3snk6wwp6qoxxc6q6c.ffgz.tsg.suse.edu.cn.展开更多
Liver transplantation(LT)remains the definitive treatment for end-stage liver disease,yet early detection of graft dysfunction and rejection is still challenging.Conventional blood-based markers provide systemic infor...Liver transplantation(LT)remains the definitive treatment for end-stage liver disease,yet early detection of graft dysfunction and rejection is still challenging.Conventional blood-based markers provide systemic information but lack hepatic specificity.Bile,directly secreted by hepatocytes and cholangiocytes,represents an organ-specific biofluid with diagnostic potential for assessing graft viability and early complications.This minireview examined the biochemical,immunological,and molecular features of bile as biomarkers in LT,focusing on pH,bicarbonate,glucose,lactate,bile acids,and proteomic/lipidomic profiles in relation to ischemia-reperfusion injury,early allograft dysfunction,and ischemic-type biliary lesions.The integration of bile-based parameters into ex situ perfusion and post-transplant monitoring,supported by omics technologies and predictive modeling,was also discussed.Building on these insights,we designed a single-center prospective study(ClinicalTrials.gov:NCT03882164)evaluating biliary tacrolimus(TACbile)as a predictor of acute rejection after LT.Paired daily TACbile and plasma tacrolimus levels are measured in recipients with Kehr T-tubes to define a blood-bile ratio of tacrolimus.The primary endpoint was the predictive accuracy of blood-bile ratio of tacrolimus for biopsy-proven rejection;secondary outcomes include associations with early allograft dysfunction.Bile-based biomarkers,particularly TACbile,may revolutionize graft monitoring and personalize immunological surveillance after LT.展开更多

Message structure reconstruction is a critical task in protocol reverse engineering,aiming to recover protocol field structures without access to source code.It enables important applications in network security,inclu...Message structure reconstruction is a critical task in protocol reverse engineering,aiming to recover protocol field structures without access to source code.It enables important applications in network security,including malware analysis and protocol fuzzing.However,existing methods suffer from inaccurate field boundary delineation and lack hierarchical relationship recovery,resulting in imprecise and incomplete reconstructions.In this paper,we propose ProRE,a novel method for reconstructing protocol field structures based on program execution slice embedding.ProRE extracts code slices from protocol parsing at runtime,converts them into embedding vectors using a data flow-sensitive assembly language model,and performs hierarchical clustering to recover complete protocol field structures.Evaluation on two datasets containing 12 protocols shows that ProRE achieves an average F1 score of 0.85 and a cophenetic correlation coefficient of 0.189,improving by 19%and 0.126%respectively over state-of-the-art methods(including BinPRE,Tupni,Netlifter,and QwQ-32B-preview),demonstrating significant superiority in both accuracy and completeness of field structure recovery.Case studies further validate the effectiveness of ProRE in practical malware analysis scenarios.展开更多

Objective:To evaluate the efficacy and safety of Ruyi Zhenbao tablet(RYZBT),a traditional Tibetan medicinal formula,for the treatment of lumbar disc herniation(LDH),that is frequently associated with lower back pain(L...Objective:To evaluate the efficacy and safety of Ruyi Zhenbao tablet(RYZBT),a traditional Tibetan medicinal formula,for the treatment of lumbar disc herniation(LDH),that is frequently associated with lower back pain(LBP),through rigorous randomized controlled trials(RCTs).Methods:This multicenter,randomized,positive-controlled trial study will include 400 participants diagnosed with LDH,and include an enrollment period,a 4-week treatment period,and a 4-week follow-up period.Participants will be randomly assigned at a 1:1 ratio for treatment with either RYZBT or diclofenac sodium sustained-release tablet(DSSRT)for 4 weeks.The time points of visits are set at baseline,during treatment(weeks 1,2,and 4),and during follow-up(weeks 2 and 4 post-treatment).The relevant outcome will be recorded at each timepoint.The primary outcomes include the Visual Analog Scale(VAS)scores for LBP and leg pain,as well as the Japanese Orthopaedic Association scores for functional assessment.Secondary outcomes comprise the VAS scores for numbness,Oswestry Disability Index,rescue treatments,patient health education,and safety assessments.Discussion:This study has been designed to provide robust and reliable evidence of the efficacy and safety of RYZBT in alleviating LDH symptoms,with the goal of supporting evidence-based clinical applications.By using DSSRT as a positive control,the placebo effect will be effectively minimized,enabling a more accurate assessment of RYZBT efficacy.展开更多
Hyperpolarization of nuclear spins is crucial for advancing nuclear magnetic resonance and quantum information technologies,as nuclear spins typically exhibit extremely low polarization at room temperature due to thei...Hyperpolarization of nuclear spins is crucial for advancing nuclear magnetic resonance and quantum information technologies,as nuclear spins typically exhibit extremely low polarization at room temperature due to their small gyromagnetic ratios.A promising approach to achieving high nuclear spin polarization is transferring the polarization of electrons to nuclear spins.The nitrogen-vacancy(NV)center in diamond has emerged as a highly effective medium for this purpose,and various hyperpolarization protocols have been developed.Among these,the pulsed polarization(PulsePol)method has been extensively studied due to its robustness against static energy shifts of the electron spin.In this work,we present a novel polarization protocol and uncover a family of magic sequences for hyperpolarizing nuclear spins,with PulsePol emerging as a special case of our general approach.Notably,we demonstrate that some of these magic sequences exhibit significantly greater robustness compared to the PulsePol protocol in the presence of finite halfpulse duration of the protocol,Rabi and detuning errors.This enhanced robustness positions our protocol as a more suitable candidate for hyper-polarizing nuclear spins species with large gyromagnetic ratios and also ensures better compatibility with high-efficiency readout techniques at high magnetic fields.Additionally,the generality of our protocol allows for its direct application to other solid-state quantum systems beyond the NV center.展开更多
With the rapid proliferation of the Industrial Internet of Things(IIoT),Building Automation Systems(BAS)and Industrial Control Systems(ICS)are increasingly exposed to sophisticated cyber threats.Conventional Intrusion...With the rapid proliferation of the Industrial Internet of Things(IIoT),Building Automation Systems(BAS)and Industrial Control Systems(ICS)are increasingly exposed to sophisticated cyber threats.Conventional Intrusion Detection Systems(IDS)often encounter significant limitations when addressing emerging or hybrid attack patterns,primarily due to delayed signature updates and high false-positive rates.Meanwhile,existing anomaly detection approaches frequently lack sufficient awareness of the physical domain,making them ineffective in identifying falsification attacks that comply with communication protocol specifications while violating underlying physical laws.To address these challenges,this study proposes a hybrid threat detection architecture that integrates Retrieval-Augmented Generation(RAG)with a Physics Rule Engine.The proposed approach leverages the semantic reasoning capabilities of Large Language Models(LLMs)to enhance protocol-level threat interpretation,while incorporating physical constraints to validate system behavior at the cyber–physical level.The core contribution of this work lies in employing an LLM to transform unstructured BACnet packet data into structured reasoning representations using the DSPy framework.These representations are subsequently examined through physics-based validation rules derived from thermodynamics and fluid mechanics,enabling the detection of attacks that are logically valid at the protocol layer but implausible in the physical domain.This layered verification process effectively reduces spurious alerts and improves detection reliability.Experimental evaluations conducted under various BACnet attack scenarios demonstrate that the proposed system,implemented with the Mistral-7B model,achieves an accuracy of 95.12%and an F1-score of 96.0%.Compared with a baseline LLM-only approach without physical validation,the proposed method significantly lowers the false-positive rate.Moreover,the system is capable of automatically generating evidence chains that support explainable security forensics,thereby enhancing situational awareness for security operators.The results of this study suggest that the integration of domain knowledge with generative AI constitutes a promising and effective strategy for strengthening the resilience of critical cyber–physical infrastructures.展开更多
Vanadium redox flow batteries(VRFBs)are among the most promising technologies for large-scale stationary energy storage because of their scalability,long cycle life,operational safety,flexible design and environmental...Vanadium redox flow batteries(VRFBs)are among the most promising technologies for large-scale stationary energy storage because of their scalability,long cycle life,operational safety,flexible design and environmentally friendly operation.However,the sluggish redox kinetics and long-term instability of conventional carbon-based electrodes remain major challenges that limit their commercial deployment.Although many electrode modifi-cation strategies have been reported,direct comparison between studies is often difficult due to variations in experimental conditions,operating parameters and electrochemical evaluation methods.In many cases,modified electrodes show excellent catalytic activity in half-cell or three-electrode measurements,but these improvements do not always translate into stable long-term performance in practical full-cell systems.This limitation highlights the urgent need for standardized testing and characterization protocols in VRFB research.Recent advances in electrode engineering,including thermal and microwave treatment,chemical and electrochemical modification,defect engineering,plasma processing,heteroatom doping,incorporation of carbon nanomaterials or metal-based catalysts and ionic liquid-derived carbon materials,have demonstrated strong potential to improve cat-alytic activity,wettability,conductivity and cycling stability.In addition,advanced in-situ and operando tech-niques,such as electrochemical impedance spectroscopy(EIS),UV-Vis spectroscopy,Raman spectroscopy,X-ray computed tomography and synchrotron-based characterization,provide deeper understanding of electrode behavior under realistic flow battery operating conditions.This review critically summarizes recent progress in carbon electrode modification strategies,structural and electrochemical characterization methods,operando diagnostic approaches and full-cell evaluation protocols for VRFBs.A novel three-tier standardized reporting framework is proposed to improve reproducibility and enable meaningful comparison across different studies.Emphasis is placed on reporting essential operational parameters,including electrolyte composition,flow rate,current density,compression ratio and state-of-charge range.Establishing standardized testing protocols and advanced operando evaluation methods will support the rational design of durable and high-performance electrodes,thereby accelerating the practical deployment of VRFBs for large-scale renewable energy integration.展开更多
BACKGROUND Histological findings of microscopic colitis(MC)are patchy,with potential for missed diagnoses.While guidelines recommend biopsies from the right and left sides of the colon,the optimal approach remains unc...BACKGROUND Histological findings of microscopic colitis(MC)are patchy,with potential for missed diagnoses.While guidelines recommend biopsies from the right and left sides of the colon,the optimal approach remains uncertain.AIM To interrogate diagnostic biopsies in a cohort of patients with confirmed MC with a view to improving biopsy protocols.METHODS We conducted a retrospective study of index colonoscopies in patients with MC over a 14-year period.We recorded demographics,symptoms,total number and location of biopsies,and whether they revealed features of lymphocytic or collagenous colitis.RESULTS A total of 228 index colonoscopies were included:64%(145/228)lymphocytic colitis and 36%(83/228)collagenous colitis.The median age at diagnosis was 60(interquartile range:48-72)years,with 27%(61/228)aged<50 years.Overall,71%(162/228)of the patients were female,similar across subtypes.Males were older at diagnosis than females(62 vs 56,P=0.03).Symptoms were known in 86%of patients(196/228),of whom 98%(192/196)had diarrhea.Among 150 cases with biopsies from the right colon,MC features were present in the right-colon specimens in 100%,with a patchy distribution in only 2%(3/150).In contrast,among 158 cases with biopsies from the left colon,MC features were present in the left-colon specimens in 92%.Among cases with paired right and left biopsies,10%(13/136)had positive right-sided biopsies with negative left-sided biopsies;the converse was never observed.Regarding specimen containers,those from the left colon were more likely to be negative[9%(17/199)vs 2%(4/209),odds ratio:4.79,P=0.0025].CONCLUSION In the era of green endoscopy,our data support a protocol of taking biopsies from at least two sites in the right colon only,processed in a single container.展开更多

BACKGROUND Nursing-led peri-endoscopic management protocols play an important role in reducing the risk of short-term rebleeding in patients with peptic ulcer bleeding and may also contribute to shorter hospital stays...BACKGROUND Nursing-led peri-endoscopic management protocols play an important role in reducing the risk of short-term rebleeding in patients with peptic ulcer bleeding and may also contribute to shorter hospital stays.AIM To evaluate the impact of a nursing-led peri-endoscopic management protocol on outcomes in peptic ulcer bleeding treated with therapeutic endoscopy.METHODS This prospective randomized controlled trial included 130 patients with peptic ulcer bleeding who underwent therapeutic endoscopy at our hospital,between March 2023 and March 2025.Patients were randomly assigned to an observation group(n=65)or a control group(n=65).The observation group received a nursing-led peri-endoscopic management protocol,whereas the control group received routine endoscopic nursing care.Outcomes included rebleeding,hospital stay,blood transfusion volume,complications,and patient satisfaction.RESULTS Baseline characteristics were comparable between groups(P>0.05).Rebleeding rates in the observation group were lower than in the control group at both time points:6.15%(4/65)vs 18.46%(12/65)on day 7 and 9.23%(6/65)vs 24.62%(16/65)on day 30(all P<0.05).Mean hospital stay was shorter in the observation group (6.8 ± 2.1 days vs 8.1 ± 2.6 days;P = 0.002). Mean blood transfusion volume was also lower (1.3 ± 0.9 U vs 1.9± 1.1 U;P < 0.05). The incidence of endoscopic complications was lower in the observation group (P < 0.05). Patientsatisfaction rate was higher in the observation group, 92.31% (60/65) vs 78.46% (51/65) in the control group (P <0.05).CONCLUSIONNursing-led peri-endoscopic management reduces short-term rebleeding in patients with peptic ulcer bleedingafter therapeutic endoscopy, shortens hospital stay, decreases blood transfusion requirements, and improves patientexperience, indicating substantial clinical value.展开更多
BACKGROUND Intraoperative device-related pressure injuries are common in gastrointestinal surgery,and conventional nursing has limited preventive efficacy.A comprehensive perioperative strategy is urgently needed.AIM ...BACKGROUND Intraoperative device-related pressure injuries are common in gastrointestinal surgery,and conventional nursing has limited preventive efficacy.A comprehensive perioperative strategy is urgently needed.AIM To evaluate the efficacy of a whole-process management protocol in preventing device-related pressure injuries during gastrointestinal surgery.METHODS This retrospective cohort study included 104 patients undergoing gastrointestinal surgery.The observation group(n=53)received a whole-process management protocol,while the control group(n=51)received standard care.The primary outcome was incidence of intraoperative pressure injuries.Secondary outcomes included postoperative complications,recovery indices,inflammatory markers,anxiety,sleep quality,and quality of life.Between-group comparisons were performed using theχ2 test,independent t-test,or Mann-Whitney U test,as appropriate.Repeated measures were analyzed using mixed-effects models.RESULTS The observation group had significantly lower pressure injury incidence at 1 day postoperatively(5.66%vs 27.45%,P=0.003)and fewer total complications(50.94%vs 80.39%,P=0.002).Times to flatus,regular diet,and wound healing were shorter in the observation group(all P<0.05).Mixed-effects models revealed significant group-by-time interactions favoring the observation group for highsensitivity C-reactive protein(P=0.004),anxiety scores(P=0.008),and all domains of quality of life(P<0.01).CONCLUSION The whole-process management protocol significantly reduces device-related pressure injuries and improves postoperative recovery,inflammation,anxiety,sleep,and quality of life in patients undergoing gastrointestinal surgery.展开更多

Postoperative recovery-related indicators, pain scores, and complication rates were compared between the two groups. Results: The standardized nursing group demonstrated shorter postoperative recovery times, lower pain scores, and a reduced complication rate compared to the conventional nursing group (P<0.05). Conclusion: The standardized nursing protocol effectively accelerates postoperative recovery in patients with acute appendicitis, alleviates pain, and reduces the likelihood of complications, thereby contributing positively to postoperative recovery.展开更多
Objective:This study aims to design and establish a comprehensive intervention protocol that combines oral care with swallowing stimulation for comatose patients with cerebral hemorrhage.The core objectives are to sys...Objective:This study aims to design and establish a comprehensive intervention protocol that combines oral care with swallowing stimulation for comatose patients with cerebral hemorrhage.The core objectives are to systematically control the risk of aspiration pneumonia,improve the oral health environment of patients,and explore its potential supportive role in the early awakening process.Methods:The study was conducted using a mixed-methods approach.The first phase involved integrating systematic literature reviews with Delphi expert consultations to establish the basic components and specific operational norms of the protocol.The second phase implemented a non-randomized concurrent controlled clinical trial,enrolling 112 eligible comatose patients with cerebral hemorrhage,evenly divided into an intervention group and a control group,with 56 patients in each group.The intervention group received a newly constructed integrated protocol of oral care and swallowing stimulation for four weeks,while the control group maintained standard oral care routines in the neurology department.The core observational indicators included the Clinical Pulmonary Infection Score(CPIS),Oral Assessment Scale(OAS)scores,changes in the Glasgow Coma Scale(GCS)scores,and analysis of microbial colony counts in saliva.Data were analyzed using SPSS 25.0 software,with continuous variables conforming to a normal distribution presented as mean±standard deviation.Comparisons between groups were made using independent sample t-tests or non-parametric Mann-Whitney U tests,and repeated measures analysis of variance was used for repeated measurement indicators.Results:After the four-week intervention period,data analysis revealed that the CPIS values in the intervention group were significantly lower than those in the control group,with statistical significance.In terms of oral health assessment,the intervention group showed significantly greater improvement than the control group.The intervention group also demonstrated a more pronounced upward trend in consciousness level scores.Microbiological test results confirmed that the survival numbers of opportunistic pathogens in the saliva of the intervention group were effectively controlled.Conclusion:The integrated protocol of oral care combined with swallowing stimulation constructed and preliminarily validated in this study has been shown to effectively reduce the clinical probability of aspiration pneumonia in comatose patients with cerebral hemorrhage,optimize oral microecological balance,and potentially promote improvement in consciousness status.This protocol holds potential value for translation into routine clinical practice.展开更多
摘要This paper concentrates on the study of passivity-based synchronization of inertial neural networks including Markov jump parameters.The second-order differential equations are converted into first-order differential equations using the variable transformation method.To make effective use of network bandwidth resources and to optimize the Markov jump inertial neural networks(MJINNs)performance,an adaptive event-driven protocol controller is studied.To achieve synchronization,an appropriate Lyapunov-Krasovskii functional(LKF)is constructed,which includes double integral terms that capture the information of time-varying delay terms.Some sufficient conditions are obtained in terms of linear matrix inequalities(LMIs)using Reciprocal convex combination lemma(RCCL).Then,a numerical simulation and an application of image encryption are carried out to illustrate the effectiveness of the proposed method.

摘要Background:Advances in intensive care have markedly improved survival after severe brain injury,leading to a growing population of patients with prolonged disorders of consciousness(pDOC).Current management of pDOC remains largely supportive,and evidence-based neuromodulatory interventions are limited;moreover,existing guidelines provide insufficiently explicit recommendations regarding mechanisms of action and objective biomarkers of treatment response.Transcutaneous auricular vagus nerve stimulation(taVNS)has emerged as a potential noninvasive intervention;however,its modulatory effects on brain function in pDOC are not yet well characterized,and the paucity of integrative mechanistic evidence has constrained its translation into routine clinical practice.Objectives:Within a multimodal assessment framework,this study aims to systematically elucidate the neurobiological mechanisms by which taVNS modulates brain function and autonomic activity in patients with pDOC,and to evaluate its clinical potential to enhance levels of consciousness.Methods:In this randomized controlled trial,60 patients with vegetative state/minimally conscious state will be enrolled and randomly allocated to a taVNS group,a transcutaneous nonauricular vagus nerve stimulation group(sham),or a control group(n=20 per group)for a 4-week intervention.The primary outcome will be changes in the Coma Recovery Scale-Revised scores from baseline to weeks 1,2,and 4 of treatment.Secondary outcomes will include functional brain activity assessed by electroencephalography and functional near-infrared spectroscopy,as well as autonomic modulation indexed by heart rate variability.Functional prognosis will be evaluated using the Glasgow Outcome Scale-Extended at the end of treatment and at a 6-month follow-up.Safety will be assessed by continuous monitoring and documentation of adverse events throughout the study period.Results and Discussion:By integrating electroencephalography–functional near-infrared spectroscopy with heart rate variability,this study will characterize the effects of taVNS on functional brain networks and consciousness recovery in pDOC across complementary behavioral,electrophysiological,hemodynamic,and autonomic domains,while interrogating potential sources of clinical and neurobiological heterogeneity.The findings are expected to provide a mechanistic and evidence-based foundation for the mechanism-driven clinical implementation of taVNS and the optimization of stimulation protocols in pDOC.Clinical Trial Registration:International Traditional Medicine Clinical Trial Registry,ITMCTR20250021041,http://gffzz299adcccaa8d4ab3snk6wwp6qoxxc6q6c.ffgz.tsg.suse.edu.cn.
摘要Liver transplantation(LT)remains the definitive treatment for end-stage liver disease,yet early detection of graft dysfunction and rejection is still challenging.Conventional blood-based markers provide systemic information but lack hepatic specificity.Bile,directly secreted by hepatocytes and cholangiocytes,represents an organ-specific biofluid with diagnostic potential for assessing graft viability and early complications.This minireview examined the biochemical,immunological,and molecular features of bile as biomarkers in LT,focusing on pH,bicarbonate,glucose,lactate,bile acids,and proteomic/lipidomic profiles in relation to ischemia-reperfusion injury,early allograft dysfunction,and ischemic-type biliary lesions.The integration of bile-based parameters into ex situ perfusion and post-transplant monitoring,supported by omics technologies and predictive modeling,was also discussed.Building on these insights,we designed a single-center prospective study(ClinicalTrials.gov:NCT03882164)evaluating biliary tacrolimus(TACbile)as a predictor of acute rejection after LT.Paired daily TACbile and plasma tacrolimus levels are measured in recipients with Kehr T-tubes to define a blood-bile ratio of tacrolimus.The primary endpoint was the predictive accuracy of blood-bile ratio of tacrolimus for biopsy-proven rejection;secondary outcomes include associations with early allograft dysfunction.Bile-based biomarkers,particularly TACbile,may revolutionize graft monitoring and personalize immunological surveillance after LT.

摘要In this paper,a zonotopic distributed fusion estimation problem is investigated for a class of 2-D nonlinear systems subject to unknown-but-bounded noises over a binary sensor network.An auxiliary innovation is constructed to reduce the influence of the less measured information,and Flex Ray protocol is employed to schedule the innovation transmission between nodes.By resorting to the set-membership filtering,a variable-independent zonotope is achieved to constrain local estimation error,and gain parameters are obtained by minimizing the upper bound of zonotope in the F-radius sense.Subsequently,a zonotopic distributed fusion scheme is implemented through the matrixweighted fusion criteria,and an optimized weighted matrix is obtained using the Lagrange Multiple method.Furthermore,the monotonicity of the local zonotope is analyzed with the gain-constraint parameter.Finally,a numerical example is considered to verify the effectiveness of the developed fusion algorithm.
摘要Message structure reconstruction is a critical task in protocol reverse engineering,aiming to recover protocol field structures without access to source code.It enables important applications in network security,including malware analysis and protocol fuzzing.However,existing methods suffer from inaccurate field boundary delineation and lack hierarchical relationship recovery,resulting in imprecise and incomplete reconstructions.In this paper,we propose ProRE,a novel method for reconstructing protocol field structures based on program execution slice embedding.ProRE extracts code slices from protocol parsing at runtime,converts them into embedding vectors using a data flow-sensitive assembly language model,and performs hierarchical clustering to recover complete protocol field structures.Evaluation on two datasets containing 12 protocols shows that ProRE achieves an average F1 score of 0.85 and a cophenetic correlation coefficient of 0.189,improving by 19%and 0.126%respectively over state-of-the-art methods(including BinPRE,Tupni,Netlifter,and QwQ-32B-preview),demonstrating significant superiority in both accuracy and completeness of field structure recovery.Case studies further validate the effectiveness of ProRE in practical malware analysis scenarios.

摘要Both large-scale prospective randomized controlled trials(RCTs)and smaller investigator-initiated trials are essential for evaluating the efficacy and safety of medical interventions.Robust protocols and statistical designs ensure the reliability of trial outcomes and improve the credibility of research findings.By reviewing the statistical approaches used in the TORCHLIGHT,NCC2167,and NeoTENNIS trials,this article illustrates the principles underlying large-sample confirmatory RCTs,small-sample exploratory adaptive designs,and single-arm two-stage designs.This discussion is aimed at helping researchers apply these design methods more effectively,to increase the likelihood of success in clinical studies.

摘要With the rapid advancement of generative artificial intelligence(AI)and large language model(LLM)technologies,AI agents are gradually becoming the core service units in networks,and their communication mode is evolving from local collaboration to wide-area interconnection.The construction of the Internet of Agents(IoA)faces multiple challenges,such as identity management,dynamic networking,and semantic routing,which urgently requires the design of a network protocol system that adapts to its new traffic characteristics and collaboration needs.Based on the application scenarios of agent communication,this paper systematically analyzes the management,control,and routing requirements that multi-agent collaboration imposes on IP networks,proposes a three-layer functional architecture for the IoA,and designs a protocol suite covering management,control,and routing around key issues such as agent registration and identification,service discovery,capability sensing,and cross-domain traffic assurance.By extending existing Internet protocols and introducing a semantically aware routing mechanism,this paper provides a scalable,efficient,and secure approach to implementing a protocol for end-to-end agent collaboration,thereby contributing to the construction of an open,large-scale agent collaboration ecosystem.

摘要Objective:To evaluate the efficacy and safety of Ruyi Zhenbao tablet(RYZBT),a traditional Tibetan medicinal formula,for the treatment of lumbar disc herniation(LDH),that is frequently associated with lower back pain(LBP),through rigorous randomized controlled trials(RCTs).Methods:This multicenter,randomized,positive-controlled trial study will include 400 participants diagnosed with LDH,and include an enrollment period,a 4-week treatment period,and a 4-week follow-up period.Participants will be randomly assigned at a 1:1 ratio for treatment with either RYZBT or diclofenac sodium sustained-release tablet(DSSRT)for 4 weeks.The time points of visits are set at baseline,during treatment(weeks 1,2,and 4),and during follow-up(weeks 2 and 4 post-treatment).The relevant outcome will be recorded at each timepoint.The primary outcomes include the Visual Analog Scale(VAS)scores for LBP and leg pain,as well as the Japanese Orthopaedic Association scores for functional assessment.Secondary outcomes comprise the VAS scores for numbness,Oswestry Disability Index,rescue treatments,patient health education,and safety assessments.Discussion:This study has been designed to provide robust and reliable evidence of the efficacy and safety of RYZBT in alleviating LDH symptoms,with the goal of supporting evidence-based clinical applications.By using DSSRT as a positive control,the placebo effect will be effectively minimized,enabling a more accurate assessment of RYZBT efficacy.

摘要Hyperpolarization of nuclear spins is crucial for advancing nuclear magnetic resonance and quantum information technologies,as nuclear spins typically exhibit extremely low polarization at room temperature due to their small gyromagnetic ratios.A promising approach to achieving high nuclear spin polarization is transferring the polarization of electrons to nuclear spins.The nitrogen-vacancy(NV)center in diamond has emerged as a highly effective medium for this purpose,and various hyperpolarization protocols have been developed.Among these,the pulsed polarization(PulsePol)method has been extensively studied due to its robustness against static energy shifts of the electron spin.In this work,we present a novel polarization protocol and uncover a family of magic sequences for hyperpolarizing nuclear spins,with PulsePol emerging as a special case of our general approach.Notably,we demonstrate that some of these magic sequences exhibit significantly greater robustness compared to the PulsePol protocol in the presence of finite halfpulse duration of the protocol,Rabi and detuning errors.This enhanced robustness positions our protocol as a more suitable candidate for hyper-polarizing nuclear spins species with large gyromagnetic ratios and also ensures better compatibility with high-efficiency readout techniques at high magnetic fields.Additionally,the generality of our protocol allows for its direct application to other solid-state quantum systems beyond the NV center.
摘要With the rapid proliferation of the Industrial Internet of Things(IIoT),Building Automation Systems(BAS)and Industrial Control Systems(ICS)are increasingly exposed to sophisticated cyber threats.Conventional Intrusion Detection Systems(IDS)often encounter significant limitations when addressing emerging or hybrid attack patterns,primarily due to delayed signature updates and high false-positive rates.Meanwhile,existing anomaly detection approaches frequently lack sufficient awareness of the physical domain,making them ineffective in identifying falsification attacks that comply with communication protocol specifications while violating underlying physical laws.To address these challenges,this study proposes a hybrid threat detection architecture that integrates Retrieval-Augmented Generation(RAG)with a Physics Rule Engine.The proposed approach leverages the semantic reasoning capabilities of Large Language Models(LLMs)to enhance protocol-level threat interpretation,while incorporating physical constraints to validate system behavior at the cyber–physical level.The core contribution of this work lies in employing an LLM to transform unstructured BACnet packet data into structured reasoning representations using the DSPy framework.These representations are subsequently examined through physics-based validation rules derived from thermodynamics and fluid mechanics,enabling the detection of attacks that are logically valid at the protocol layer but implausible in the physical domain.This layered verification process effectively reduces spurious alerts and improves detection reliability.Experimental evaluations conducted under various BACnet attack scenarios demonstrate that the proposed system,implemented with the Mistral-7B model,achieves an accuracy of 95.12%and an F1-score of 96.0%.Compared with a baseline LLM-only approach without physical validation,the proposed method significantly lowers the false-positive rate.Moreover,the system is capable of automatically generating evidence chains that support explainable security forensics,thereby enhancing situational awareness for security operators.The results of this study suggest that the integration of domain knowledge with generative AI constitutes a promising and effective strategy for strengthening the resilience of critical cyber–physical infrastructures.

摘要Vanadium redox flow batteries(VRFBs)are among the most promising technologies for large-scale stationary energy storage because of their scalability,long cycle life,operational safety,flexible design and environmentally friendly operation.However,the sluggish redox kinetics and long-term instability of conventional carbon-based electrodes remain major challenges that limit their commercial deployment.Although many electrode modifi-cation strategies have been reported,direct comparison between studies is often difficult due to variations in experimental conditions,operating parameters and electrochemical evaluation methods.In many cases,modified electrodes show excellent catalytic activity in half-cell or three-electrode measurements,but these improvements do not always translate into stable long-term performance in practical full-cell systems.This limitation highlights the urgent need for standardized testing and characterization protocols in VRFB research.Recent advances in electrode engineering,including thermal and microwave treatment,chemical and electrochemical modification,defect engineering,plasma processing,heteroatom doping,incorporation of carbon nanomaterials or metal-based catalysts and ionic liquid-derived carbon materials,have demonstrated strong potential to improve cat-alytic activity,wettability,conductivity and cycling stability.In addition,advanced in-situ and operando tech-niques,such as electrochemical impedance spectroscopy(EIS),UV-Vis spectroscopy,Raman spectroscopy,X-ray computed tomography and synchrotron-based characterization,provide deeper understanding of electrode behavior under realistic flow battery operating conditions.This review critically summarizes recent progress in carbon electrode modification strategies,structural and electrochemical characterization methods,operando diagnostic approaches and full-cell evaluation protocols for VRFBs.A novel three-tier standardized reporting framework is proposed to improve reproducibility and enable meaningful comparison across different studies.Emphasis is placed on reporting essential operational parameters,including electrolyte composition,flow rate,current density,compression ratio and state-of-charge range.Establishing standardized testing protocols and advanced operando evaluation methods will support the rational design of durable and high-performance electrodes,thereby accelerating the practical deployment of VRFBs for large-scale renewable energy integration.
摘要BACKGROUND Histological findings of microscopic colitis(MC)are patchy,with potential for missed diagnoses.While guidelines recommend biopsies from the right and left sides of the colon,the optimal approach remains uncertain.AIM To interrogate diagnostic biopsies in a cohort of patients with confirmed MC with a view to improving biopsy protocols.METHODS We conducted a retrospective study of index colonoscopies in patients with MC over a 14-year period.We recorded demographics,symptoms,total number and location of biopsies,and whether they revealed features of lymphocytic or collagenous colitis.RESULTS A total of 228 index colonoscopies were included:64%(145/228)lymphocytic colitis and 36%(83/228)collagenous colitis.The median age at diagnosis was 60(interquartile range:48-72)years,with 27%(61/228)aged<50 years.Overall,71%(162/228)of the patients were female,similar across subtypes.Males were older at diagnosis than females(62 vs 56,P=0.03).Symptoms were known in 86%of patients(196/228),of whom 98%(192/196)had diarrhea.Among 150 cases with biopsies from the right colon,MC features were present in the right-colon specimens in 100%,with a patchy distribution in only 2%(3/150).In contrast,among 158 cases with biopsies from the left colon,MC features were present in the left-colon specimens in 92%.Among cases with paired right and left biopsies,10%(13/136)had positive right-sided biopsies with negative left-sided biopsies;the converse was never observed.Regarding specimen containers,those from the left colon were more likely to be negative[9%(17/199)vs 2%(4/209),odds ratio:4.79,P=0.0025].CONCLUSION In the era of green endoscopy,our data support a protocol of taking biopsies from at least two sites in the right colon only,processed in a single container.
摘要Protocol Reverse Engineering(PRE)is of great practical importance in Internet security-related fields such as intrusion detection,vulnerability mining,and protocol fuzzing.For unknown binary protocols having fixed-length fields,and the accurate identification of field boundaries has a great impact on the subsequent analysis and final performance.Hence,this paper proposes a new protocol segmentation method based on Information-theoretic statistical analysis for binary protocols by formulating the field segmentation of unsupervised binary protocols as a probabilistic inference problem and modeling its uncertainty.Specifically,we design four related constructions between entropy changes and protocol field segmentation,introduce random variables,and construct joint probability distributions with traffic sample observations.Probabilistic inference is then performed to identify the possible protocol segmentation points.Extensive trials on nine common public and industrial control protocols show that the proposed method yields higher-quality protocol segmentation results.

摘要Bloody tears denote a medical condition characterized by the presence of blood in the tears that flow from the eye,stemming from a variety of causes.The etiology and clinical manifestations of this condition are intricate and multifaceted,frequently intersecting with multiple disciplines and specialties.To furnish clinicians with a valuable reference for the clinical diagnosis and treatment of bloody tears,the Lacrimal Diseases Professional Committee of Ophthalmologist Branch of Chinese Medical Doctor Association,in conjunction with the Lacrimal Diseases Group of Ophthalmic Professional Committee of Chinese Association of Integrated Medicine,organized a panel of relevant experts.After thorough and serious discussion,these experts established this consensus document,which offers guiding opinions on the etiology,clinical characteristics,and diagnosis and therapeutic approaches for bloody tears based on clinical practice.
摘要BACKGROUND Nursing-led peri-endoscopic management protocols play an important role in reducing the risk of short-term rebleeding in patients with peptic ulcer bleeding and may also contribute to shorter hospital stays.AIM To evaluate the impact of a nursing-led peri-endoscopic management protocol on outcomes in peptic ulcer bleeding treated with therapeutic endoscopy.METHODS This prospective randomized controlled trial included 130 patients with peptic ulcer bleeding who underwent therapeutic endoscopy at our hospital,between March 2023 and March 2025.Patients were randomly assigned to an observation group(n=65)or a control group(n=65).The observation group received a nursing-led peri-endoscopic management protocol,whereas the control group received routine endoscopic nursing care.Outcomes included rebleeding,hospital stay,blood transfusion volume,complications,and patient satisfaction.RESULTS Baseline characteristics were comparable between groups(P>0.05).Rebleeding rates in the observation group were lower than in the control group at both time points:6.15%(4/65)vs 18.46%(12/65)on day 7 and 9.23%(6/65)vs 24.62%(16/65)on day 30(all P<0.05).Mean hospital stay was shorter in the observation group (6.8 ± 2.1 days vs 8.1 ± 2.6 days;P = 0.002). Mean blood transfusion volume was also lower (1.3 ± 0.9 U vs 1.9± 1.1 U;P < 0.05). The incidence of endoscopic complications was lower in the observation group (P < 0.05). Patientsatisfaction rate was higher in the observation group, 92.31% (60/65) vs 78.46% (51/65) in the control group (P <0.05).CONCLUSIONNursing-led peri-endoscopic management reduces short-term rebleeding in patients with peptic ulcer bleedingafter therapeutic endoscopy, shortens hospital stay, decreases blood transfusion requirements, and improves patientexperience, indicating substantial clinical value.
摘要BACKGROUND Intraoperative device-related pressure injuries are common in gastrointestinal surgery,and conventional nursing has limited preventive efficacy.A comprehensive perioperative strategy is urgently needed.AIM To evaluate the efficacy of a whole-process management protocol in preventing device-related pressure injuries during gastrointestinal surgery.METHODS This retrospective cohort study included 104 patients undergoing gastrointestinal surgery.The observation group(n=53)received a whole-process management protocol,while the control group(n=51)received standard care.The primary outcome was incidence of intraoperative pressure injuries.Secondary outcomes included postoperative complications,recovery indices,inflammatory markers,anxiety,sleep quality,and quality of life.Between-group comparisons were performed using theχ2 test,independent t-test,or Mann-Whitney U test,as appropriate.Repeated measures were analyzed using mixed-effects models.RESULTS The observation group had significantly lower pressure injury incidence at 1 day postoperatively(5.66%vs 27.45%,P=0.003)and fewer total complications(50.94%vs 80.39%,P=0.002).Times to flatus,regular diet,and wound healing were shorter in the observation group(all P<0.05).Mixed-effects models revealed significant group-by-time interactions favoring the observation group for highsensitivity C-reactive protein(P=0.004),anxiety scores(P=0.008),and all domains of quality of life(P<0.01).CONCLUSION The whole-process management protocol significantly reduces device-related pressure injuries and improves postoperative recovery,inflammation,anxiety,sleep,and quality of life in patients undergoing gastrointestinal surgery.

摘要Objective: To analyze the specific effects of standardized nursing protocols on postoperative recovery in patients undergoing acute appendicitis surgery. Methods: A total of 80 patients who underwent acute appendicitis surgery at our hospital between January 2024 and June 2025 were enrolled. These patients were randomly assigned to either the conventional nursing group or the standardized nursing group using a random number table, with 40 cases in each group. The conventional nursing group received traditional postoperative care, while the standardized nursing group received the standardized nursing protocol. Postoperative recovery-related indicators, pain scores, and complication rates were compared between the two groups. Results: The standardized nursing group demonstrated shorter postoperative recovery times, lower pain scores, and a reduced complication rate compared to the conventional nursing group (P<0.05). Conclusion: The standardized nursing protocol effectively accelerates postoperative recovery in patients with acute appendicitis, alleviates pain, and reduces the likelihood of complications, thereby contributing positively to postoperative recovery.
摘要Objective:This study aims to design and establish a comprehensive intervention protocol that combines oral care with swallowing stimulation for comatose patients with cerebral hemorrhage.The core objectives are to systematically control the risk of aspiration pneumonia,improve the oral health environment of patients,and explore its potential supportive role in the early awakening process.Methods:The study was conducted using a mixed-methods approach.The first phase involved integrating systematic literature reviews with Delphi expert consultations to establish the basic components and specific operational norms of the protocol.The second phase implemented a non-randomized concurrent controlled clinical trial,enrolling 112 eligible comatose patients with cerebral hemorrhage,evenly divided into an intervention group and a control group,with 56 patients in each group.The intervention group received a newly constructed integrated protocol of oral care and swallowing stimulation for four weeks,while the control group maintained standard oral care routines in the neurology department.The core observational indicators included the Clinical Pulmonary Infection Score(CPIS),Oral Assessment Scale(OAS)scores,changes in the Glasgow Coma Scale(GCS)scores,and analysis of microbial colony counts in saliva.Data were analyzed using SPSS 25.0 software,with continuous variables conforming to a normal distribution presented as mean±standard deviation.Comparisons between groups were made using independent sample t-tests or non-parametric Mann-Whitney U tests,and repeated measures analysis of variance was used for repeated measurement indicators.Results:After the four-week intervention period,data analysis revealed that the CPIS values in the intervention group were significantly lower than those in the control group,with statistical significance.In terms of oral health assessment,the intervention group showed significantly greater improvement than the control group.The intervention group also demonstrated a more pronounced upward trend in consciousness level scores.Microbiological test results confirmed that the survival numbers of opportunistic pathogens in the saliva of the intervention group were effectively controlled.Conclusion:The integrated protocol of oral care combined with swallowing stimulation constructed and preliminarily validated in this study has been shown to effectively reduce the clinical probability of aspiration pneumonia in comatose patients with cerebral hemorrhage,optimize oral microecological balance,and potentially promote improvement in consciousness status.This protocol holds potential value for translation into routine clinical practice.
