In previous studies, researchers have used singing to treat respiratory function in patients with spinal cord injury. However, few studies have examined the way in which vocal training affects respiratory neural plasticity in patients with spinal cord injury.Vocal respiratory training(VRT)is a type of vocal muscle-related treatment that is often a component of music therapy(MT)and focuses on strengthening respiratory muscles and improving lung function. In this randomized controlled study,we analyzed the therapeutic effects of VRT on respiratory dysfunction at 3 months after cervical spinal cord injury.Of an initial group of 37 patients,26 completed the music therapy intervention, which comprised five 30-minute sessions per week for 12 weeks. The intervention group(n=13)received VRT training delivered by professional certified music therapists. The control group(n=13)received respiratory physical therapy delivered by professional physical therapists.Compared with the control group,we observed a substantial increase in respiratory function in the intervention group after the 12-week intervention.Further, the nerve fiber bundles in the respiratory center in the medulla exhibited a trend towards increased diversification, with an increased number, path length, thickness, and density of nerve fiber bundles. These findings provide strong evidence for the effect of music therapeutic VRT on neural plasticity. This study was approved by the Ethics Committee of China Rehabilitation Research Center(approval No.2020-013-1)on April 1,2020,and was registered with the Chinese Clinical Trial Registry(registration No.Chi CTR2000037871)on September 2,2020. 展开更多

INTRODUCTION: Gait asymmetry can become very pronounced in patients who have suffered a stroke. The impairment of trunk function in some stroke patients can restrict thorax mobility and cause respiratory muscle weakness. Trunk and neck dysfunction are believed to affect the gait in stroke patients. OBJECTIVE: This study aimed to investigate the relationship between gait asymmetry and respiratory function in stroke patients by measuring the step time and trunk acceleration. METHODS: This study employed a cross-sec- tional design. Thirty stroke patients participated in this study. The symmetry index (SI) and Lissajous index (LI) were used to evaluate asymmetry during walking. The respiratory function and respiratory muscle strength were evaluated by spirometry. We examined the relation between SI or LI and the respiratory functionespiratory muscle strength in patients with stroke. RE- SULTS: The results of our analysis demonstrated that the SI was significantly correlated with the inspiratory and expiratory muscle strength and the LI was significantly correlated with the percentage of the predicted vital capacity (respectively, r = ﹣0.386, r = ﹣0.392, r = ﹣0.446;p CONCLUSION: The present study is the first to indicate a relationship between gait asymmetry and respiratory function in stroke patients. 展开更多

Chronic obstructive pulmonary disease(COPD),a disease responsible for early mortality worldwide,is well accepted to be associated with periodontitis epidemiologically.Although both of the diseases are the multi-microbial inflammatory disease, the precise underlying mechanisms by which periodontitis influences the progression of COPD remains largely unknown. Here,we established COPD accompanied with periodontitis mouse models and observed the pronounced progress in pulmonary symptoms and histopathology, cha racterized by poorer respiratory function, thicke ned bronchial walls, and increased neutrophils infiltration in lung tissue.Mechanistically, periodontitis pathogen Porphyromonas gingivalis(P.gingivalis)relocated in the lung through the respiratory tract and LPS from P.gingivalis promoted the secretion of chemokines CXCL2 and G-CSF of alveolar epithelial cells through NF-κB and p38 MAPK pathways to recruit neutrophils.Furthermore, exposure to P.gingivalis of infiltrated neutrophils released matrix metallopeptidase-8(MMP-8)and neutrophil elastase(NE),which aggravated airway inflammation and tissue damage. These findings indicated that periodontitis could exacerbate COPD via its pathogen P.gingivalis, which translocated in the lung and stimulated neutrophil chemotaxis and activation in the lung. 展开更多

BACKGROUND Dopaminergic neurons are lost in Parkinson’s disease(PD),a degenerative neurological condition. While the effects of PD on motor functions are well understood, its impact on respiratory function is less clear. In PD patients with suspicious pulmonary symptoms, the presence and degree of impairment in lung function often cannot be optimally assessed through pulmonary function tests(PFTs)due to the patients'neurological symptoms.AIM To examine the relationship between diaphragmatic thickness as determined by ultrasonography(US)and conventional PFTs in PD patients.Additionally,we evaluated the potential use of diaphragmatic thickness as an alternative measure for assessing respiratory function in PD patients who are unable to perform PFTs.METHODS A total of 43 PD patients who presented to the clinics between June 15,2021,and June 30,2022,were included.Patients with coronavirus disease 2019 infection history, conditions affecting diaphragmatic motility, recent thoracic or abdominal surgery,or imaging showing restrictive masses were excluded.Demographic data, disease duration,Hoehn-Yahr stage, and medication use were recorded.PFTs were used to evaluate forced vital capacity, forced expiratory volume in the first second, and other parameters, while US was used to measure diaphragmatic thickness.Breathlessness was assessed using the modified medical research council dyspnea scale.RESULTS Significant correlations were found between peak expiratory flow(PEF)and diaphragm fraction(P=0.019),as well as between PEF percentage and diaphragmatic thickness during expiration(P=0.023).In early-stage PD,diaphragmatic thickness during inspiration correlated with oxygen saturation(P=0.048).In advanced-stage PD,PEF and PEF%were significantly correlated with diaphragm thickness during expiration[r=0.405,P=0.049(for PEF);r=0.463,P=0.023(for PEF%)]and diaphragm fraction[r=-0.476,P=0.019(for PEF);r=-0.617,P=0.001(for PEF%)].CONCLUSION Present study assessed the relationship between PFTs and diaphragmatic thickness in PD patients.Diaphragmatic thickness measured by US was found to correlate with certain PFT parameters, suggesting it could be used as an alternative measure for assessing respiratory function in Parkinson’s patients who have difficulty performing PFTs. 展开更多

BACKGROUND Thoracic scoliosis causes complex three-dimensional deformities of the thoracic cage that can impair lung mechanics and airway geometry, leading to restrictive ventilatory dysfunction.Respiratory impairment differs by etiology, with nonidiopathic scoliosis often showing more severe and persistent deformity.Conventional pulmonary function testing provides global functional assessment but is frequently limited in patients with non-idiopathic scoliosis.Advances in chest computed tomography(CT)allow quantitative evaluation of lung volume and airway morphology, offering an alternative approach to assess respiratory function and the relationship to spinal deformity.AIM To investigate CT-derived airway morphology and respiratory function in thoracic scoliosis, focusing on etiologic differences and structure-function relationships.METHODS This retrospective observational study included 53 patients≤30 years of age who underwent corrective surgery for thoracic scoliosis and had preoperative and postoperative CT examinations.Patients were classified into idiopathic(n=25)or non-idiopathic scoliosis(n=28).Spinal deformity parameters, lung volumes, predicted total lung capacity percentage(TLC%),and airway dimensions were quantified using CT.Group comparisons were performed using the Wilcoxon rank-sum test, correlations were determined using Spearman’s rank correlation, and pre-and postoperative changes were determined using the Wilcoxon signed-rank test.RESULTS Patients with non-idiopathic scoliosis had significantly more severe spinal deformities and a lower TLC%compared to patients with idiopathic scoliosis(median TLC%:39%vs 64%,P<0.001).The Cobb angle was negatively correlated with TLC%(ρ=-0.52,P=0.004),lung volumes, and multiple airway parameters in patients with non-idiopathic scoliosis.Surgical correction significantly improved spinal alignment in both groups.CTderived lung volume parameters did not show significant postoperative changes. In contrast, selective improvement was observed in the left bronchus.CONCLUSION CT-derived analysis showed differences in respiratory impairment between idiopathic and non-idiopathic scoliosis.Surgical correction improved alignment without immediate lung volume improvement, with selective changes in the left bronchus. 展开更多

But treated Group A showed certain degree of recovery of respirato ry function; treated Group B showed further improvement. 2. Untreated Group, tre ated Groups A and B had different degrees of mitochondrial ultra structural dam age respectively, which could be attenuated after the treatment with magnesium s ulfate. Conclusions: The mitochondrial respiratory function decreases s ignificantly after traumatic brain injury. But it can be apparently improved aft er magnesium sulfate management along with the attenuated damage of mitochondria discovered by TEM. The longer course of treatment can obtain a better improveme nt of mitochondrial respiratory function. 展开更多

Objective: To explore the therapeutic effect of shengmai injec tion (SMI) on respiratory function in chronic obstructive pulmonary disease (COPD ).Methods: Twenty patients with COPD were continuously treated w ith SMI by intravenous drip for 14 days and their lung vital capacity (VC),for ced vital capacity (FVC),forced vital capacity of the first second (FEV1),FEV1/FVC , maximum voluntary ventilation (MVV), maximum inspiratory pressure (MIP),load respiratory time (LT), 6 minute walk distance (6 MWD), arterial blood gas an alysis, and Borg dyspnea scale were examined respectively before and after treatment a nd compared before and after treatment or with the control group. Results: After treatment all parameters in the treated group were significantly better than those in the control group or than those before treatment (P<0.05) with the exception of FVC and FEV1/FVC. Conclusion: SMI could improve respiratory function in COPD. So i t is an effective rehabilitative injection in treating COPD 展开更多

Objective Scoliosis is a prevalent spinal deformity that severely impacts the physical and psychological well being of adolescents and often requires surgical correction. This study aimed to investigate the risk factors for postoperative pulmonary complications(PPCs)following posterior spinal fusion in adolescent scoliosis, with a specific focus on evaluating the predictive value of preoperative forced vital capacity(FVC)and etiological classification. Methods This retrospective cohort study enrolled adolescents who underwent posterior spinal fusion for scoliosis correction at Peking Union Medical College Hospital between January 2018 and December 2022.The baseline demographic, clinical, and perioperative variables were collected. The primary endpoint was the occurrence of PPCs prior to hospital discharge.Univariate and multivariable binary logistic regression analyses were conducted to identify independent risk factors. Results A total of 1,234 patients were included,of whom 29(2.35%)developed PPCs and 1,205 did not.Univariate analysis revealed that 12 variables, including non-idiopathic scoliosis,a larger main thoracic curve,a decreased FVC%predicted, and increased perioperative blood transfusions, were significantly associated with PPCs(P<0.05).Multivariable logistic regression model showed that non-idiopathic scoliosis emerged as the sole independent risk factor for PPCs[adjusted odds ratio(OR)=5.478,95%confidence interval:2.017-19.148,P=0.002].Preoperative FVC%predicted(adjusted OR=0.993,P=0.497)and main thoracic curve(adjusted OR=1.002,P=0.736)did not demonstrate independent predictive value. Conclusions Non-idiopathic scoliosis, rather than preoperative pulmonary function or curve severity,is an independent risk factor for PPCs following posterior spinal fusion in adolescents.During perioperative respiratory risk assessment, particular emphasis should be placed on the etiological classification of these patients. 展开更多
