BACKGROUND Schizophrenia presents complex challenges in older patients due to cognitive and social decline.Existing drug treatments offer limited benefits and pose risks,while aerobic exercise shows promise as a nonin...BACKGROUND Schizophrenia presents complex challenges in older patients due to cognitive and social decline.Existing drug treatments offer limited benefits and pose risks,while aerobic exercise shows promise as a noninvasive intervention whose impact remains underexplored.AIM To investigate the effects of aerobic exercise on cognitive and social function in older patients with schizophrenia.METHODS A retrospective study was conducted in 158 older patients with schizophrenia treated at The First Affiliated Hospital of Chongqing Medical and Pharmaceutical College(June 2023 and December 2024).The patients were divided into an observation group(n=86),which received 3 months of aerobic rehabilitation alongside routine treatment,and a control group(n=72),which received routine treatment alone.Cognitive function was assessed using the Measurement and Treatment Research to Improve Cognition in Schizophrenia Consensus Cognitive Battery,social function using the Inpatient Psychiatric Rehabilitation Outcome Scale(IPROS),and psychotic symptoms using the Positive and Negative Syndrome Scale.RESULTS Post-treatment,Positive and Negative Syndrome Scale total scores were lower in the observation group than in the controls(P<0.001),with significant improvements in positive,negative,and general symptoms.Measurement and Treatment Research to Improve Cognition in Schizophrenia Consensus Cognitive Battery domain scores were improved in processing speed,working memory,verbal/visual learning,and executive function(all P<0.05).IPROS total scores were decreased(P<0.001),indicating better social functioning.Longer exercise duration correlated with greater cognitive gains and lower social function deficits.Body mass index declined(P<0.001),and adverse events were mild and transient(9.3%).Multivariate linear regression confirmed that aerobic exercise was independently associated with a significant reduction in IPROS scores(Adjustedβ=-4.57,95%confidence intervals:-6.08 to-3.05,P<0.001).CONCLUSION Aerobic exercise effectively improves cognitive function,social function,and psychotic symptoms in older patients with schizophrenia,is safe,and serves as a valuable adjunctive intervention.展开更多
Vascular cognitive impairment and dementia is a debilitating neurological disorder caused by chronic cerebral hypoperfusion,for which no effective causative treatments are currently available.Intermittent hypoxia has ...Vascular cognitive impairment and dementia is a debilitating neurological disorder caused by chronic cerebral hypoperfusion,for which no effective causative treatments are currently available.Intermittent hypoxia has been shown to enhance cerebral blood flow in mice,but its efficacy in a model of vascular cognitive impairment and dementia remains unclear.In this study,we established a mouse model of vascular cognitive impairment and dementia by bilateral carotid artery stenosis.Intermittent hypoxia was induced before and after this stenosis.We found that intermittent hypoxia increased cerebral blood flow,oxygen saturation,and microcirculation in the prefrontal cortex and hippocampus in the model mice,without causing neurovascular damage.Additionally,intermittent hypoxia significantly improved cognitive function in the mouse model of vascular cognitive impairment and dementia,with perconditioning showing greater efficacy than preconditioning.Improvements in cerebral microcirculation and blood flow were positively correlated with cognitive recovery.Even in a mouse model of vascular cognitive impairment and dementia with comorbidities induced by a high-fat,high-fructose diet,intermittent hypoxic perconditioning demonstrated protective effects on cognitive function.Proteomic analysis indicated that mitochondrial protection is a key mechanism,particularly through upregulating NDUFB8 expression and increasing the activity of mitochondrial complex I.These findings suggest that intermittent hypoxia is a potential non-invasive strategy for the prevention and treatment of vascular cognitive impairment and dementia.展开更多
BACKGROUND While several studies have explored the relationship among physical activity(PA),depressive symptoms,and cognitive health,the distinct effects of PA performed for occupational vs recreational purposes remai...BACKGROUND While several studies have explored the relationship among physical activity(PA),depressive symptoms,and cognitive health,the distinct effects of PA performed for occupational vs recreational purposes remain underexplored.AIM To investigate the differential impacts of job-related vs leisure-related physical activities on depressive symptoms and cognitive function among adults aged 45 years and older in China.METHODS Data were extracted from the China Health and Retirement Longitudinal Study,encompassing 16476 participants.Depressive symptoms were assessed using the 10-item Center for Epidemiologic Studies Depression Scale,and cognitive function was evaluated using the Mini-Mental State Examination.PA was categorized by purpose(job-related vs leisure-related)and intensity(vigorous,moderate,and low),with frequency and duration measured in metabolic equivalent hours per day.An external validation using an independent clinical sample(n=200)was conducted.RESULTS Vigorous-intensity PA for job demands(JVPA)was significantly associated with increased depressive symptoms[P=0.011,odds ratio(OR)=1.003],indicating that high-intensity job-related activities may exacerbate mental health issues.Moderate-intensity PA for entertainment or exercise(EMPA)was inversely associated with depressive symptoms(P=0.030,OR=0.999),suggesting that moderate-intensity leisure activities can reduce depressive symptoms.For cognitive function,the total PA for job demands was correlated with cognitive decline(P=0.004,OR=1.008),with the frequency of JVPA showing a positive association.However,EMPA was linked to reduced cognitive decline(P=0.018,OR=0.998).Clinical validation results were consistent with those obtained from the database,further supporting the reliability of the findings.CONCLUSION JVPA exacerbates depressive symptoms and cognitive impairment,whereas EMPA mitigates depression and supports cognitive health.Targeted interventions promoting leisure-related PA may enhance mental and cognitive well-being in older adults.展开更多

Sport-related concussion(SRC)and its potential neurological sequela represent an emerging global health concern,requiring improved recovery management and strategies for return-to-play(RTP)to enhance brain health in a...Sport-related concussion(SRC)and its potential neurological sequela represent an emerging global health concern,requiring improved recovery management and strategies for return-to-play(RTP)to enhance brain health in athletes.Given the dynamic and multifaceted nature of SRC recovery,the purpose of this review is to synthesize existing literature on post-SRC outcomes in adult athletes,and to outline the temporal trajectories of key recovery indicators(symptoms,cognitive function,blood biomarkers)across distinct recovery phases until resolution.In the acute phase of SRC(first 48 h),symptom scores and brain damage markers peaked immediately,while cognitive impairments and neuroinflammation emerged with a slight delay.Following the initial rise,brain damage marker concentrations rapidly dropped below baseline levels at approximately 48 h following SRC injury.During the early recovery phase,neuroinflammation and most cognitive alterations resolved after 3–5 days,though symptom burden and attention deficits persisted for up to 7 days.Despite prolonged alterations reported in some individuals,recovery markers typically returned to pre-injury levels in the transition phase(≤2 weeks),though mild attention deficits were detected up to 3 weeks,and TNF-α concentrations remained elevated throughout late recovery(>2 weeks).These results reveal distinct temporal discrepancies across recovery markers and emphasize that physiological disturbances can outlast symptom resolution,underscoring the need for both multimodal assessments and appropriately timed evaluations to accurately track recovery progression.Incorporating structured follow-ups at key time points,particularly beyond symptom resolution,may improve RTP decision-making and reduce the risk of premature return and long-term neurological consequences.展开更多
Background:Health benefits have been reported for many physical activity(PA)interventions for improving fundamental movement skills(FMS)and cognitive function(CF),but the most effective type of PA interventions for em...Background:Health benefits have been reported for many physical activity(PA)interventions for improving fundamental movement skills(FMS)and cognitive function(CF),but the most effective type of PA interventions for emhancing FMS and CF in early childhood remain unknown.Thus,the study aimed to determine the effects of PA interventions in enhancing FMS and CF among young children and to establish the optimal types of PA interventions.Methods:Six electronic databases(PubMed,OVID,SPORTDiscus,Scopus,Web of Science,and Cochrane)were searched for studies from inception to March 17,2024.Randomized controlled trials(RCTs)were included in this study if they reported outcomes related to FMS,CF,or both associated with PA interventions.Effect sizes were calculated and performed as Hedges'g.The hierarchy of competing interventions was established using the surface under the cumulative ranking curve(SUCRA).Risk of bias was independently assessed using the Cochrane Riskof-Bias 2.Results:This analysis included 38 studies with 5237 young children,with sample sizes ranging from 32 to 897 participants.The types of PA interventions analyzed included active play/free play/unstructured PA(AP),general structured PA(GSPA),FMS-targeted PA programs(FMSprograms),cognitively-engaging PA programs(CPA),multilevel PA interventions(MPA),and exergaming.PA interventions had a large,pooled effect size for total FMS(g=0.96;95%CI:0.45-1.46;p<0.01;I2=94%).For CF,a small-to-moderate pooled effect size was found(g=0.39;95%CI:0.18-0.60;p<0.01;I2=88%).PA interventions longer than 3 months showed fewer benefits for FMS(p<0.01).The network meta-analysis showed that FMS-programs(standardized mean difference((SMD)=1.55,95%CI:0.98-2.11,SUCRA=98.3%)and GSPA(SMD=0.94,95%CI:0.05-1.85,SUCRA=69.8%)significantly improved total FMS compared to AP.For locomotor skills(LMS),exergaming ranked highest(SUCRA=79.3%),followed by FMS-programs(75.9%)and GSPA(61.6%).However,despite its top ranking,exergaming's effect estimate was not statistically significant(SMD=1.38,95%CI:-0.08 to 2.85).For object control skills(OCS),exergaming again ranked highest(SUCRA=91.9%)and showed the largest significant effect(SMD=2.38,95%CI:0.96-3.80),followed by FMS-programs(SUCRA=78.5%)and GSPA(SUCRA=53.7%).FMS-programs,GSPA,MPA,and UC also significantly improved OCS compared to AP.While no significant differences were observed across PA interventions for most CF domains,exergaming had a significant positive effect on working memory(SMD=1.41,95%CI:0.07-2.75).The certainty of evidence varied from low to moderate.Conclusion:These findings emphasize the importance of PA interventions in improving FMS and CF in early childhood.FMS-programs and GSPA appear to be the most effective approaches for enhancing total FMS,while exergaming showed the highest ranking for LMS and OCS,with a significant impact on OCS but uncertainty in LMS improvements.Additionally,exergaming had a positive effect on working memory,suggesting its potential cognitive benefits.展开更多
Multivitamins were widely used health supplements that replenished essential nutrients in the human body.Despite their popularity,the impact of multivitamins on the cognitive function of older adults remained unclear ...Multivitamins were widely used health supplements that replenished essential nutrients in the human body.Despite their popularity,the impact of multivitamins on the cognitive function of older adults remained unclear and contentious.This study offered a comprehensive review and meta-analysis of research published until June 2024,analyzing the effects of multivitamins on various cognitive functions in individuals aged 65 and older.We included ten randomized controlled trials encompassing 13,600 participants from multiple databases.These studies evaluated the impact of multivitamins on reasoning,memory,learning,visual perception,idea production,cognitive speed,psychomotor abilities,and higher cognitive functions.Our meta-analysis revealed that multivitamins significantly enhanced delayed free recall (standardized mean difference(SMD)=0.09,95%confidence interval(CI)=[0.05,0.13],P<0.0001).However,they had no substantial effects on immediate free recall(SMD=0.85,95%CI=[-0.18,1.90],P=0.11),idea production(SMD=0.00,95%CI=[-0.04,0.03],P=0.86),or cognitive speed(SMD=0.34,95%CI=[-0.07,0.74],P=0.11).Thus,while multivitamins facilitated delayed free recall,they did not significantly improve other cognitive functions in older adults.展开更多
Aims To investigate the efficacy and safety of transcutaneous auricular vagus nerve stimulation(taVNS)in improving cognitive function in older inpatients with schizophrenia.Design Randomised controlled trial.Setting C...Aims To investigate the efficacy and safety of transcutaneous auricular vagus nerve stimulation(taVNS)in improving cognitive function in older inpatients with schizophrenia.Design Randomised controlled trial.Setting Cognitive improvement in hospitalised older patients with schizophrenia in Shanghai.Participants Fifty-one older inpatients with schizophrenia in Shanghai Mental Health Center.Intervention The taVNS electrodes were placed on the skin with vagus nerve distribution in the taVNS group,whereas the electrodes were placed on the skin without vagus nerve distribution in the sham taVNS group.taVNS parameters included 25 Hz,the maximum tolerated current intensity(apparatus maximum load current ≤6 mA) and one 30-min session per day for 2 weeks,for a total of 10 interventions.Measurements The primary outcome was a neurocognitive assessment.Safety was the secondary outcome measure.After each treatment session,adverse reactions were assessed using the self-administered Adverse Reaction Assessment Scale.Four subtests of the Cambridge Neuropsychological Test Automated Battery were further considered:reaction time(RTI),paired associate learning(PAL),multitasking test(MTT) and spatial working memory(SWM).Results The time and interaction effects of betweengroup search errors were statistically significant(F=9.51,p <0.001).Compared to the sham taVNS,taVNS improved SWM performance in older patients with schizophrenia.Furthermore,the SWM performance after a single session of taVNS showed fewer errors in the taVNS group,whereas no such improvement was observed in the sham taVNS group.Unlike in SWM,there were no significant between-group differences in RTI,PAL or MTT after either a single stimulation or a session of 10 stimulations.Conclusions taVNS may have the potential to improve SWM in older patients with schizophrenia.展开更多
BACKGROUND Depression is a highly prevalent comorbidity in Alzheimer’s disease(AD),exacerbating cognitive decline and worsening prognosis.Adiponectin(APN),an adipokine with anti-inflammatory and neuroprotective prope...BACKGROUND Depression is a highly prevalent comorbidity in Alzheimer’s disease(AD),exacerbating cognitive decline and worsening prognosis.Adiponectin(APN),an adipokine with anti-inflammatory and neuroprotective properties,has been implicated in both conditions,yet its role in the comorbid state remains unclear.We hypothesized that serum APN levels would be significantly reduced in these patients and would independently correlate with the severity of cognitive impairment.AIM To investigate the association between serum APN levels and cognitive function in AD patients with comorbid depression.METHODS This observational study enrolled 60 AD patients with depression and 60 healthy controls from a hospital neurology department.The observation group was subclassified into mild,moderate,and severe groups using the Clinical Dementia Rating Scale.Cognitive function was evaluated using the Montreal Cognitive Assessment(MoCA)and the Mini-Mental State Examination(MMSE).Serum APN levels were determined via ELISA,and metabolic/inflammatory markers were also assessed.Data were analyzed using t-tests,ANOVA,Pearson correlation,and multiple linear regression.RESULTS In the observation group serum APN levels(6.67±1.99μg/mL)and MoCA/MMSE scores were considerably lower than in control group(12.53±2.87μg/mL;all P<0.001).Within the observation group,APN levels and cognitive scores decreased progressively with worsening disease severity(all P<0.001).Serum APN was positively associated with MoCA(r=0.781,P<0.001)and MMSE scores(r=0.643,P<0.001).After adjusting for metabolic/inflammatory factors,APN remained an independent predictor of MoCA scores(t=8.009,P<0.001).CONCLUSION Serum APN levels are markedly reduced in AD patients with depression and closely correlate with impaired cognition.APN may represent a potential biomarker for assessing cognitive status in this population.展开更多

Traumatic brain injury involves complex pathophysiological mechanisms,among which oxidative stress significantly contributes to the occurrence of secondary injury.In this study,we evaluated hypidone hydrochloride(YL-0...Traumatic brain injury involves complex pathophysiological mechanisms,among which oxidative stress significantly contributes to the occurrence of secondary injury.In this study,we evaluated hypidone hydrochloride(YL-0919),a self-developed antidepressant with selective sigma-1 receptor agonist properties,and its associated mechanisms and targets in traumatic brain injury.Behavioral experiments to assess functional deficits were followed by assessment of neuronal damage through histological analyses and examination of blood-brain barrier permeability and brain edema.Next,we investigated the antioxidative effects of YL-0919 by assessing the levels of traditional markers of oxidative stress in vivo in mice and in vitro in HT22 cells.Finally,the targeted action of YL-0919 was verified by employing a sigma-1 receptor antagonist(BD-1047).Our findings demonstrated that YL-0919 markedly improved deficits in motor function and spatial cognition on day 3 post traumatic brain injury,while also decreasing neuronal mortality and reversing blood-brain barrier disruption and brain edema.Furthermore,YL-0919 effectively combated oxidative stress both in vivo and in vitro.The protective effects of YL-0919 were partially inhibited by BD-1047.These results indicated that YL-0919 relieved impairments in motor and spatial cognition by restraining oxidative stress,a neuroprotective effect that was partially reversed by the sigma-1 receptor antagonist BD-1047.YL-0919 may have potential as a new treatment for traumatic brain injury.展开更多
BACKGROUND Increasing evidence has shown that hippocampal damage serves as a marker of early cognitive decline in patients with type 2 diabetes mellitus(T2DM);however,the association between hippocampal subregion volu...BACKGROUND Increasing evidence has shown that hippocampal damage serves as a marker of early cognitive decline in patients with type 2 diabetes mellitus(T2DM);however,the association between hippocampal subregion volume changes and cognitive decline in different dimensions remains unclear.AIM To investigate changes in hippocampal subregion volumes in patients with T2DM and their relationship with cognitive function impairment.METHODS Sixty patients with T2DM and 32 healthy controls were recruited.All participants underwent a 3.0 T magnetic resonance scan and a series of clinical assessments.Hippocampal subfield volumes were determined using FreeSurfer 7.4.1.A two-sample t-test was used to evaluate group differences.Partial correlation analysis was performed to assess the relationship between hippocampal subregion volumes and cognitive function.aP<0.05 was considered statistically significant.RESULTS Compared with controls,the volume of right hippocampus-amygdala transition area(t=-3.053,P=0.003)in patients with T2DM was significantly reduced,which was negatively correlated with the required time of the Trail Making Test(TMT)-A(r=-0.331,P=0.028)and TMT-B(r=-0.402,P=0.007)and positively correlated with the scores of Symbol Digit Modalities Test(r=0.381,P=0.011),Auditory Verbal Learning Test(AVLT)-N7(r=0.309,P=0.041),and Digital Span Test(r=0.300,P=0.048).The volume of the right molecular layer(t=-2.998,P=0.004)was also significantly reduced,which was positively associated with the scores of AVLT-N7(r=0.311,P=0.045).In addition,the left hippocampal fissure volume(t=3.617,P=0.002)was significantly increased in patients with T2DM.CONCLUSION Declines in cognitive performance,especially memory and executive function,are linked to changes in the volumes of the right hippocampus-amygdala transition area and right molecular layer in patients with T2DM.展开更多
BACKGROUND Surgery is a common treatment for gastrointestinal tumors.General anesthesia(GA),while effective,can cause oxidative stress reactions and neuroinflammation,potentially leading to postoperative cognitive dys...BACKGROUND Surgery is a common treatment for gastrointestinal tumors.General anesthesia(GA),while effective,can cause oxidative stress reactions and neuroinflammation,potentially leading to postoperative cognitive dysfunction and gastrointestinal dysfunction.The stellate ganglion block(SGB)can reduce sympathetic excitability and stress responses.This study aims to investigate whether combining SGB with GA can mitigate these adverse effects in patients undergoing gastrointestinal surgery.AIM To analyze the effects of SGB plus GA on hemodynamic stability,oxidative stress,neuroinflammation,cognitive function,and gastrointestinal function in patients undergoing gastrointestinal surgery.METHODS Patients undergoing gastrointestinal surgery between October 2022 and December 2024 were divided into two groups:A single GA group and an SGB combined with GA group(40 patients each).Hemodynamics,oxidative stress response,laboratory indices,cognitive function,and gastrointestinal function were compared preoperatively and 24 hours postoperatively between the two groups.Pain levels and complications were also recorded.RESULTS Before anesthesia induction,no significant differences were found in various indexes(including hemodynamics,oxidative stress indicators,laboratory indices,cognitive function scores,and gastrointestinal function indicators)between the two groups(P>0.05).At tracheal intubation,3 minutes after,and extubation,the GA-only group had significantly higher mean arterial pressure and heart rate postoperatively than preoperatively and compared to the SGB-GA combined group(P0.05).CONCLUSION Combining SGB with GA can maintain perioperative hemodynamic stability,reduce oxidative stress and neuroinflammatory injury,and attenuate postoperative cognitive decline and gastrointestinal dysfunction in patients undergoing gastrointestinal surgery.展开更多
BACKGROUND Ischemic stroke(IS)survivors face an elevated risk of epileptic seizures,and recurrent seizures following the first episode often signal worsening functional outcomes.AIM To investigate risk factors associa...BACKGROUND Ischemic stroke(IS)survivors face an elevated risk of epileptic seizures,and recurrent seizures following the first episode often signal worsening functional outcomes.AIM To investigate risk factors associated with seizure recurrence after a first episode in patients with IS and explore their associations with cognitive function,anxiety,and depression.METHODS A total of 100 patients with IS admitted to Shaanxi Provincial People’s Hospital between January 2017 and January 2024 were enrolled in this study.After a 1-5-year follow-up,patients were categorized into recurrence(n=43)and nonrecurrence(n=57)groups.Their medical records were collected and analyzed using univariate and multivariate analyses to determine potential predictors of seizure recurrence.Variables with statistical significance in the univariate analysis were incorporated into a binary logistic regression model for multivariate analysis.The risk model’s predictive performance was evaluated using the receiver operating characteristic curve.How independent risk factors,identified in multivariate analysis,related to cognitive[Montreal Cognitive Assessment(MoCA)]and emotional[Self-Rating Anxiety Scale(SAS)/Self-Rating Depression Scale(SDS)]outcomes,were assessed.RESULTS Recurrent seizures were significantly associated with age,stroke severity(National Institutes of Health Stroke Scale),late-onset seizures,electroenceph-alogram abnormalities,cortical involvement,hemorrhagic infarction,and extensive cerebral infarctions,with lateonset seizures,cortical involvement,and hemorrhagic infarction serving as independent predictors.The risk model revealed an area under the curve of 0.732,with 88.37%specificity and 42.11%sensitivity.These three were also correlated with lower MoCA scores and higher SAS and SDS scores.CONCLUSION In patients with IS,recurrent seizures after the first episode can be independently predicted by late-onset seizures,cortical involvement,and hemorrhagic cerebral infarction-factors also correlating with cognitive impairment and emotional distress.These findings underscore the need for close clinical monitoring and targeted interventions(e.g.,cognitive rehabilitation and psychological support)to mitigate seizure recurrence in high-risk individuals.展开更多
This article explored the application of dexmedetomidine(Dex),a highly selective alpha-2 agonist,in managing postoperative cognitive dysfunction(POCD)in elderly patients undergoing radical colon cancer surgery.Aging i...This article explored the application of dexmedetomidine(Dex),a highly selective alpha-2 agonist,in managing postoperative cognitive dysfunction(POCD)in elderly patients undergoing radical colon cancer surgery.Aging is associated with a progressive decline in physiological functions and an increased risk of adverse surgical outcomes,including POCD,which encompasses many neurocognitive disorders that manifest during the perioperative period.The aging population is at a higher risk for POCD,which can lead to prolonged hospital stays,delayed recovery,and increased healthcare costs.Dex has neuroprotective,opioid-sparing,and sympatholytic properties,which reduces the incidence and severity of POCD.Dex was introduced for sedation in patients receiving mechanical ventilation but has since been adopted in anesthesia due to its multifaceted benefits.Its appli-cation extends to sedation,analgesia,maintenance of anesthesia,and controlling delirium.Its neuroprotective and anti-inflammatory effects have been explored in managing POCD.This article discussed the broad range of patient and procedure-related risk factors for POCD.Early identification and intervention are crucial to prevent the progression of POCD,which can have severe physical,psychological,and economic consequences.The article underscored the importance of a mul-tidisciplinary approach in managing POCD,involving the optimization of comor-bidities,depth of anesthesia monitoring,hemodynamic stability,and cerebral oxygenation monitoring.展开更多

Objective:To identify key factors influencing cognitive function in the elderly,including traditional Chinese medicine(TCM)constitutional classification,and to rank their relative importance.Methods:We used cross-sect...Objective:To identify key factors influencing cognitive function in the elderly,including traditional Chinese medicine(TCM)constitutional classification,and to rank their relative importance.Methods:We used cross-sectional data from seven geographical regions across Chinese mainland.The Changsha version of the Montreal Cognitive Assessment was used to assess cognitive function.A“least absolute shrinkage and selection operator”(LASSO)model,multivariate linear regression analysis,and random forest(RF)model were used.Subgroup analyses were performed to examine the correlation between key TCM constitution types and cognitive function in different population subgroups.Results:A total of 24803 individuals aged 60 and above were included in the study.We selected 18 influential factors using the LASSO model.Higher education,being married,and having insurance were positively correlated with cognitive function in the elderly(all P<0.05).In contrast,poor sleep,vision impairment,hearing impairment,basic activities of daily living disability,instrumental activities of daily living disability,depression,hypertension,coronary heart disease,diabetes,stroke,yang-deficiency constitution(YADC),yin-deficiency constitution(YIDC),qi deficiency constitution(QDC),and blood stasis constitution(BSC)were negatively correlated with cognitive function(all P<0.05).YIDC and BSC affected all dimensions of cognitive function(all P<0.05).YADC mainly affected attention,language,abstraction(verbal analogies),memory,and orientation to time and place dimensions(P<0.001).QDC mainly affected language and abstraction(verbal analogies)dimensions(P<0.05).The negative correlations between BSC,YADC,YIDC,and QDC scores and cognitive function revealed statistically significant dif-ferences across most subgroups.The RF model identified education,BSC,and poor sleep quality as the three most influential factors in our study.Conclusion:BSC,YADC,YIDC,and QDC were associated with cognitive decline in the elderly.Our findings provide new perspectives and significant references for interventions for early-stage cognitive disorders.展开更多

More recently, ecological demands have informed such investigations, most notably with respect to foraging in birds. In parallel, behavioural ecologists, primarily concerned with functional optimization, have begun to consider the role of mechanistic factors, including cognition, to explain apparent deviations from optimal predictions. In the present paper we discuss the application of laboratory-based constructs and paradigms of cognition to the real-world challenges faced by avian foragers. We argue that such applications have been handicapped by what we term the 'paradigmatic assumption' - the assumption that a given laboratory paradigm maps well enough onto a congruent cognitive mechanism (or cognitive ability) to justify confiation of the two. We present evidence against the paradigmatic assumption and suggest that to achieve a profitable integration between function and mechanism, with respect to animal cognition, a new conceptualization of cognitive mechanisms - functional cogni- tion - is required. This new conceptualization should define cognitive mechanisms based on the informational properties of the animal's environment and the adaptive challenges faced. Cognitive mechanisms must be examined in settings that mimic the im- portant aspects of the natural environment, using customized tasks designed to probe defined aspects of the mechanisms' opera- tion. We suggest that this approach will facilitate investigations of the functional and evolutionary relevance of cognitive mecha- nisms, as well as the patterns of divergence, convergence and specialization of cognitive mechanisms within and between species .展开更多
BACKGROUND Cognitive impairment is one of the common clinical manifestations of depression,causing negative distress to patients.Elevated homocysteine(Hcy)concentrations and gut microbiome dysfunction may be observed ...BACKGROUND Cognitive impairment is one of the common clinical manifestations of depression,causing negative distress to patients.Elevated homocysteine(Hcy)concentrations and gut microbiome dysfunction may be observed in patients with depression.AIM To investigate the relationship between Hcy,microbiome,and cognition in depressive patients.METHODS We recruited 67 patients with major depressive disorder(MDD)(MDD group)and 94 healthy controls(HCs)individuals(HCs group).Serum Hcy levels were determined using the enzyme circulation method.16s rRNA sequencing was used to classify and identify the fecal bacteria.17 Hamilton depression rating scale and MATRICS consensus cognitive battery were used to evaluate mood states and cognition in patients with MDD. Correlation analysis was performed to explore the correlation between fecal flora,Hcy, and depressive cognitive function.RESULTSElevated serum levels of Hcy were seen in patients with MDD compared to healthy individuals. Patients withMDD indicated significant decreases in cognitive scores (P < 0.001) in six modules: Speed of processing, workingmemory, visual learning, reasoning and problem-solving, social cognition, and total scores. Hcy levels showed anegative correlation with processing speed, social cognition, and total MDD scores (P < 0.05). Hcy was alsosignificantly negatively correlated with Alistipes, Ruminococcae, Tenericides, and Porphyromonas (P < 0.05).CONCLUSIONOur results highlight that Hcy was correlated with cognition and gut microbiome in MDD. This interaction may berelated to the physiological and pathological mechanisms underlying cognitive deficits in depression.展开更多

摘要BACKGROUND Schizophrenia presents complex challenges in older patients due to cognitive and social decline.Existing drug treatments offer limited benefits and pose risks,while aerobic exercise shows promise as a noninvasive intervention whose impact remains underexplored.AIM To investigate the effects of aerobic exercise on cognitive and social function in older patients with schizophrenia.METHODS A retrospective study was conducted in 158 older patients with schizophrenia treated at The First Affiliated Hospital of Chongqing Medical and Pharmaceutical College(June 2023 and December 2024).The patients were divided into an observation group(n=86),which received 3 months of aerobic rehabilitation alongside routine treatment,and a control group(n=72),which received routine treatment alone.Cognitive function was assessed using the Measurement and Treatment Research to Improve Cognition in Schizophrenia Consensus Cognitive Battery,social function using the Inpatient Psychiatric Rehabilitation Outcome Scale(IPROS),and psychotic symptoms using the Positive and Negative Syndrome Scale.RESULTS Post-treatment,Positive and Negative Syndrome Scale total scores were lower in the observation group than in the controls(P<0.001),with significant improvements in positive,negative,and general symptoms.Measurement and Treatment Research to Improve Cognition in Schizophrenia Consensus Cognitive Battery domain scores were improved in processing speed,working memory,verbal/visual learning,and executive function(all P<0.05).IPROS total scores were decreased(P<0.001),indicating better social functioning.Longer exercise duration correlated with greater cognitive gains and lower social function deficits.Body mass index declined(P<0.001),and adverse events were mild and transient(9.3%).Multivariate linear regression confirmed that aerobic exercise was independently associated with a significant reduction in IPROS scores(Adjustedβ=-4.57,95%confidence intervals:-6.08 to-3.05,P<0.001).CONCLUSION Aerobic exercise effectively improves cognitive function,social function,and psychotic symptoms in older patients with schizophrenia,is safe,and serves as a valuable adjunctive intervention.

摘要Vascular cognitive impairment and dementia is a debilitating neurological disorder caused by chronic cerebral hypoperfusion,for which no effective causative treatments are currently available.Intermittent hypoxia has been shown to enhance cerebral blood flow in mice,but its efficacy in a model of vascular cognitive impairment and dementia remains unclear.In this study,we established a mouse model of vascular cognitive impairment and dementia by bilateral carotid artery stenosis.Intermittent hypoxia was induced before and after this stenosis.We found that intermittent hypoxia increased cerebral blood flow,oxygen saturation,and microcirculation in the prefrontal cortex and hippocampus in the model mice,without causing neurovascular damage.Additionally,intermittent hypoxia significantly improved cognitive function in the mouse model of vascular cognitive impairment and dementia,with perconditioning showing greater efficacy than preconditioning.Improvements in cerebral microcirculation and blood flow were positively correlated with cognitive recovery.Even in a mouse model of vascular cognitive impairment and dementia with comorbidities induced by a high-fat,high-fructose diet,intermittent hypoxic perconditioning demonstrated protective effects on cognitive function.Proteomic analysis indicated that mitochondrial protection is a key mechanism,particularly through upregulating NDUFB8 expression and increasing the activity of mitochondrial complex I.These findings suggest that intermittent hypoxia is a potential non-invasive strategy for the prevention and treatment of vascular cognitive impairment and dementia.
摘要BACKGROUND While several studies have explored the relationship among physical activity(PA),depressive symptoms,and cognitive health,the distinct effects of PA performed for occupational vs recreational purposes remain underexplored.AIM To investigate the differential impacts of job-related vs leisure-related physical activities on depressive symptoms and cognitive function among adults aged 45 years and older in China.METHODS Data were extracted from the China Health and Retirement Longitudinal Study,encompassing 16476 participants.Depressive symptoms were assessed using the 10-item Center for Epidemiologic Studies Depression Scale,and cognitive function was evaluated using the Mini-Mental State Examination.PA was categorized by purpose(job-related vs leisure-related)and intensity(vigorous,moderate,and low),with frequency and duration measured in metabolic equivalent hours per day.An external validation using an independent clinical sample(n=200)was conducted.RESULTS Vigorous-intensity PA for job demands(JVPA)was significantly associated with increased depressive symptoms[P=0.011,odds ratio(OR)=1.003],indicating that high-intensity job-related activities may exacerbate mental health issues.Moderate-intensity PA for entertainment or exercise(EMPA)was inversely associated with depressive symptoms(P=0.030,OR=0.999),suggesting that moderate-intensity leisure activities can reduce depressive symptoms.For cognitive function,the total PA for job demands was correlated with cognitive decline(P=0.004,OR=1.008),with the frequency of JVPA showing a positive association.However,EMPA was linked to reduced cognitive decline(P=0.018,OR=0.998).Clinical validation results were consistent with those obtained from the database,further supporting the reliability of the findings.CONCLUSION JVPA exacerbates depressive symptoms and cognitive impairment,whereas EMPA mitigates depression and supports cognitive health.Targeted interventions promoting leisure-related PA may enhance mental and cognitive well-being in older adults.

摘要Sport-related concussion(SRC)and its potential neurological sequela represent an emerging global health concern,requiring improved recovery management and strategies for return-to-play(RTP)to enhance brain health in athletes.Given the dynamic and multifaceted nature of SRC recovery,the purpose of this review is to synthesize existing literature on post-SRC outcomes in adult athletes,and to outline the temporal trajectories of key recovery indicators(symptoms,cognitive function,blood biomarkers)across distinct recovery phases until resolution.In the acute phase of SRC(first 48 h),symptom scores and brain damage markers peaked immediately,while cognitive impairments and neuroinflammation emerged with a slight delay.Following the initial rise,brain damage marker concentrations rapidly dropped below baseline levels at approximately 48 h following SRC injury.During the early recovery phase,neuroinflammation and most cognitive alterations resolved after 3–5 days,though symptom burden and attention deficits persisted for up to 7 days.Despite prolonged alterations reported in some individuals,recovery markers typically returned to pre-injury levels in the transition phase(≤2 weeks),though mild attention deficits were detected up to 3 weeks,and TNF-α concentrations remained elevated throughout late recovery(>2 weeks).These results reveal distinct temporal discrepancies across recovery markers and emphasize that physiological disturbances can outlast symptom resolution,underscoring the need for both multimodal assessments and appropriately timed evaluations to accurately track recovery progression.Incorporating structured follow-ups at key time points,particularly beyond symptom resolution,may improve RTP decision-making and reduce the risk of premature return and long-term neurological consequences.
摘要Background:Health benefits have been reported for many physical activity(PA)interventions for improving fundamental movement skills(FMS)and cognitive function(CF),but the most effective type of PA interventions for emhancing FMS and CF in early childhood remain unknown.Thus,the study aimed to determine the effects of PA interventions in enhancing FMS and CF among young children and to establish the optimal types of PA interventions.Methods:Six electronic databases(PubMed,OVID,SPORTDiscus,Scopus,Web of Science,and Cochrane)were searched for studies from inception to March 17,2024.Randomized controlled trials(RCTs)were included in this study if they reported outcomes related to FMS,CF,or both associated with PA interventions.Effect sizes were calculated and performed as Hedges'g.The hierarchy of competing interventions was established using the surface under the cumulative ranking curve(SUCRA).Risk of bias was independently assessed using the Cochrane Riskof-Bias 2.Results:This analysis included 38 studies with 5237 young children,with sample sizes ranging from 32 to 897 participants.The types of PA interventions analyzed included active play/free play/unstructured PA(AP),general structured PA(GSPA),FMS-targeted PA programs(FMSprograms),cognitively-engaging PA programs(CPA),multilevel PA interventions(MPA),and exergaming.PA interventions had a large,pooled effect size for total FMS(g=0.96;95%CI:0.45-1.46;p<0.01;I2=94%).For CF,a small-to-moderate pooled effect size was found(g=0.39;95%CI:0.18-0.60;p<0.01;I2=88%).PA interventions longer than 3 months showed fewer benefits for FMS(p<0.01).The network meta-analysis showed that FMS-programs(standardized mean difference((SMD)=1.55,95%CI:0.98-2.11,SUCRA=98.3%)and GSPA(SMD=0.94,95%CI:0.05-1.85,SUCRA=69.8%)significantly improved total FMS compared to AP.For locomotor skills(LMS),exergaming ranked highest(SUCRA=79.3%),followed by FMS-programs(75.9%)and GSPA(61.6%).However,despite its top ranking,exergaming's effect estimate was not statistically significant(SMD=1.38,95%CI:-0.08 to 2.85).For object control skills(OCS),exergaming again ranked highest(SUCRA=91.9%)and showed the largest significant effect(SMD=2.38,95%CI:0.96-3.80),followed by FMS-programs(SUCRA=78.5%)and GSPA(SUCRA=53.7%).FMS-programs,GSPA,MPA,and UC also significantly improved OCS compared to AP.While no significant differences were observed across PA interventions for most CF domains,exergaming had a significant positive effect on working memory(SMD=1.41,95%CI:0.07-2.75).The certainty of evidence varied from low to moderate.Conclusion:These findings emphasize the importance of PA interventions in improving FMS and CF in early childhood.FMS-programs and GSPA appear to be the most effective approaches for enhancing total FMS,while exergaming showed the highest ranking for LMS and OCS,with a significant impact on OCS but uncertainty in LMS improvements.Additionally,exergaming had a positive effect on working memory,suggesting its potential cognitive benefits.

摘要Multivitamins were widely used health supplements that replenished essential nutrients in the human body.Despite their popularity,the impact of multivitamins on the cognitive function of older adults remained unclear and contentious.This study offered a comprehensive review and meta-analysis of research published until June 2024,analyzing the effects of multivitamins on various cognitive functions in individuals aged 65 and older.We included ten randomized controlled trials encompassing 13,600 participants from multiple databases.These studies evaluated the impact of multivitamins on reasoning,memory,learning,visual perception,idea production,cognitive speed,psychomotor abilities,and higher cognitive functions.Our meta-analysis revealed that multivitamins significantly enhanced delayed free recall (standardized mean difference(SMD)=0.09,95%confidence interval(CI)=[0.05,0.13],P<0.0001).However,they had no substantial effects on immediate free recall(SMD=0.85,95%CI=[-0.18,1.90],P=0.11),idea production(SMD=0.00,95%CI=[-0.04,0.03],P=0.86),or cognitive speed(SMD=0.34,95%CI=[-0.07,0.74],P=0.11).Thus,while multivitamins facilitated delayed free recall,they did not significantly improve other cognitive functions in older adults.

摘要Aims To investigate the efficacy and safety of transcutaneous auricular vagus nerve stimulation(taVNS)in improving cognitive function in older inpatients with schizophrenia.Design Randomised controlled trial.Setting Cognitive improvement in hospitalised older patients with schizophrenia in Shanghai.Participants Fifty-one older inpatients with schizophrenia in Shanghai Mental Health Center.Intervention The taVNS electrodes were placed on the skin with vagus nerve distribution in the taVNS group,whereas the electrodes were placed on the skin without vagus nerve distribution in the sham taVNS group.taVNS parameters included 25 Hz,the maximum tolerated current intensity(apparatus maximum load current ≤6 mA) and one 30-min session per day for 2 weeks,for a total of 10 interventions.Measurements The primary outcome was a neurocognitive assessment.Safety was the secondary outcome measure.After each treatment session,adverse reactions were assessed using the self-administered Adverse Reaction Assessment Scale.Four subtests of the Cambridge Neuropsychological Test Automated Battery were further considered:reaction time(RTI),paired associate learning(PAL),multitasking test(MTT) and spatial working memory(SWM).Results The time and interaction effects of betweengroup search errors were statistically significant(F=9.51,p <0.001).Compared to the sham taVNS,taVNS improved SWM performance in older patients with schizophrenia.Furthermore,the SWM performance after a single session of taVNS showed fewer errors in the taVNS group,whereas no such improvement was observed in the sham taVNS group.Unlike in SWM,there were no significant between-group differences in RTI,PAL or MTT after either a single stimulation or a session of 10 stimulations.Conclusions taVNS may have the potential to improve SWM in older patients with schizophrenia.

摘要BACKGROUND Depression is a highly prevalent comorbidity in Alzheimer’s disease(AD),exacerbating cognitive decline and worsening prognosis.Adiponectin(APN),an adipokine with anti-inflammatory and neuroprotective properties,has been implicated in both conditions,yet its role in the comorbid state remains unclear.We hypothesized that serum APN levels would be significantly reduced in these patients and would independently correlate with the severity of cognitive impairment.AIM To investigate the association between serum APN levels and cognitive function in AD patients with comorbid depression.METHODS This observational study enrolled 60 AD patients with depression and 60 healthy controls from a hospital neurology department.The observation group was subclassified into mild,moderate,and severe groups using the Clinical Dementia Rating Scale.Cognitive function was evaluated using the Montreal Cognitive Assessment(MoCA)and the Mini-Mental State Examination(MMSE).Serum APN levels were determined via ELISA,and metabolic/inflammatory markers were also assessed.Data were analyzed using t-tests,ANOVA,Pearson correlation,and multiple linear regression.RESULTS In the observation group serum APN levels(6.67±1.99μg/mL)and MoCA/MMSE scores were considerably lower than in control group(12.53±2.87μg/mL;all P<0.001).Within the observation group,APN levels and cognitive scores decreased progressively with worsening disease severity(all P<0.001).Serum APN was positively associated with MoCA(r=0.781,P<0.001)and MMSE scores(r=0.643,P<0.001).After adjusting for metabolic/inflammatory factors,APN remained an independent predictor of MoCA scores(t=8.009,P<0.001).CONCLUSION Serum APN levels are markedly reduced in AD patients with depression and closely correlate with impaired cognition.APN may represent a potential biomarker for assessing cognitive status in this population.
摘要Alzheimer’s Disease(AD)is a degenerative disorder of the central nervous system characterized primarily by progressive cognitive decline,behavioral abnormalities,and a decrease in daily living abilities.It is predominantly observed in the elderly population,with its incidence rising annually alongside the aging demographic,posing a significant public health threat to the physical and mental well-being and quality of life of older adults.Currently,clinical treatment primarily relies on pharmacological interventions to alleviate symptoms,whereas functional exercise nursing interventions,as a core non-pharmacological approach,can effectively delay cognitive decline,improve physical motor function,enhance self-care abilities in daily life,and reduce the caregiving burden on families and society.This article systematically reviews the main types,implementation methods,influencing factors,and intervention effects of functional exercise nursing interventions for elderly patients with AD,aiming to provide theoretical foundations and practical references for the clinical implementation of standardized and individualized functional exercise nursing interventions.

摘要Traumatic brain injury involves complex pathophysiological mechanisms,among which oxidative stress significantly contributes to the occurrence of secondary injury.In this study,we evaluated hypidone hydrochloride(YL-0919),a self-developed antidepressant with selective sigma-1 receptor agonist properties,and its associated mechanisms and targets in traumatic brain injury.Behavioral experiments to assess functional deficits were followed by assessment of neuronal damage through histological analyses and examination of blood-brain barrier permeability and brain edema.Next,we investigated the antioxidative effects of YL-0919 by assessing the levels of traditional markers of oxidative stress in vivo in mice and in vitro in HT22 cells.Finally,the targeted action of YL-0919 was verified by employing a sigma-1 receptor antagonist(BD-1047).Our findings demonstrated that YL-0919 markedly improved deficits in motor function and spatial cognition on day 3 post traumatic brain injury,while also decreasing neuronal mortality and reversing blood-brain barrier disruption and brain edema.Furthermore,YL-0919 effectively combated oxidative stress both in vivo and in vitro.The protective effects of YL-0919 were partially inhibited by BD-1047.These results indicated that YL-0919 relieved impairments in motor and spatial cognition by restraining oxidative stress,a neuroprotective effect that was partially reversed by the sigma-1 receptor antagonist BD-1047.YL-0919 may have potential as a new treatment for traumatic brain injury.

摘要BACKGROUND Increasing evidence has shown that hippocampal damage serves as a marker of early cognitive decline in patients with type 2 diabetes mellitus(T2DM);however,the association between hippocampal subregion volume changes and cognitive decline in different dimensions remains unclear.AIM To investigate changes in hippocampal subregion volumes in patients with T2DM and their relationship with cognitive function impairment.METHODS Sixty patients with T2DM and 32 healthy controls were recruited.All participants underwent a 3.0 T magnetic resonance scan and a series of clinical assessments.Hippocampal subfield volumes were determined using FreeSurfer 7.4.1.A two-sample t-test was used to evaluate group differences.Partial correlation analysis was performed to assess the relationship between hippocampal subregion volumes and cognitive function.aP<0.05 was considered statistically significant.RESULTS Compared with controls,the volume of right hippocampus-amygdala transition area(t=-3.053,P=0.003)in patients with T2DM was significantly reduced,which was negatively correlated with the required time of the Trail Making Test(TMT)-A(r=-0.331,P=0.028)and TMT-B(r=-0.402,P=0.007)and positively correlated with the scores of Symbol Digit Modalities Test(r=0.381,P=0.011),Auditory Verbal Learning Test(AVLT)-N7(r=0.309,P=0.041),and Digital Span Test(r=0.300,P=0.048).The volume of the right molecular layer(t=-2.998,P=0.004)was also significantly reduced,which was positively associated with the scores of AVLT-N7(r=0.311,P=0.045).In addition,the left hippocampal fissure volume(t=3.617,P=0.002)was significantly increased in patients with T2DM.CONCLUSION Declines in cognitive performance,especially memory and executive function,are linked to changes in the volumes of the right hippocampus-amygdala transition area and right molecular layer in patients with T2DM.
摘要BACKGROUND Surgery is a common treatment for gastrointestinal tumors.General anesthesia(GA),while effective,can cause oxidative stress reactions and neuroinflammation,potentially leading to postoperative cognitive dysfunction and gastrointestinal dysfunction.The stellate ganglion block(SGB)can reduce sympathetic excitability and stress responses.This study aims to investigate whether combining SGB with GA can mitigate these adverse effects in patients undergoing gastrointestinal surgery.AIM To analyze the effects of SGB plus GA on hemodynamic stability,oxidative stress,neuroinflammation,cognitive function,and gastrointestinal function in patients undergoing gastrointestinal surgery.METHODS Patients undergoing gastrointestinal surgery between October 2022 and December 2024 were divided into two groups:A single GA group and an SGB combined with GA group(40 patients each).Hemodynamics,oxidative stress response,laboratory indices,cognitive function,and gastrointestinal function were compared preoperatively and 24 hours postoperatively between the two groups.Pain levels and complications were also recorded.RESULTS Before anesthesia induction,no significant differences were found in various indexes(including hemodynamics,oxidative stress indicators,laboratory indices,cognitive function scores,and gastrointestinal function indicators)between the two groups(P>0.05).At tracheal intubation,3 minutes after,and extubation,the GA-only group had significantly higher mean arterial pressure and heart rate postoperatively than preoperatively and compared to the SGB-GA combined group(P0.05).CONCLUSION Combining SGB with GA can maintain perioperative hemodynamic stability,reduce oxidative stress and neuroinflammatory injury,and attenuate postoperative cognitive decline and gastrointestinal dysfunction in patients undergoing gastrointestinal surgery.
摘要BACKGROUND Ischemic stroke(IS)survivors face an elevated risk of epileptic seizures,and recurrent seizures following the first episode often signal worsening functional outcomes.AIM To investigate risk factors associated with seizure recurrence after a first episode in patients with IS and explore their associations with cognitive function,anxiety,and depression.METHODS A total of 100 patients with IS admitted to Shaanxi Provincial People’s Hospital between January 2017 and January 2024 were enrolled in this study.After a 1-5-year follow-up,patients were categorized into recurrence(n=43)and nonrecurrence(n=57)groups.Their medical records were collected and analyzed using univariate and multivariate analyses to determine potential predictors of seizure recurrence.Variables with statistical significance in the univariate analysis were incorporated into a binary logistic regression model for multivariate analysis.The risk model’s predictive performance was evaluated using the receiver operating characteristic curve.How independent risk factors,identified in multivariate analysis,related to cognitive[Montreal Cognitive Assessment(MoCA)]and emotional[Self-Rating Anxiety Scale(SAS)/Self-Rating Depression Scale(SDS)]outcomes,were assessed.RESULTS Recurrent seizures were significantly associated with age,stroke severity(National Institutes of Health Stroke Scale),late-onset seizures,electroenceph-alogram abnormalities,cortical involvement,hemorrhagic infarction,and extensive cerebral infarctions,with lateonset seizures,cortical involvement,and hemorrhagic infarction serving as independent predictors.The risk model revealed an area under the curve of 0.732,with 88.37%specificity and 42.11%sensitivity.These three were also correlated with lower MoCA scores and higher SAS and SDS scores.CONCLUSION In patients with IS,recurrent seizures after the first episode can be independently predicted by late-onset seizures,cortical involvement,and hemorrhagic cerebral infarction-factors also correlating with cognitive impairment and emotional distress.These findings underscore the need for close clinical monitoring and targeted interventions(e.g.,cognitive rehabilitation and psychological support)to mitigate seizure recurrence in high-risk individuals.
摘要This article explored the application of dexmedetomidine(Dex),a highly selective alpha-2 agonist,in managing postoperative cognitive dysfunction(POCD)in elderly patients undergoing radical colon cancer surgery.Aging is associated with a progressive decline in physiological functions and an increased risk of adverse surgical outcomes,including POCD,which encompasses many neurocognitive disorders that manifest during the perioperative period.The aging population is at a higher risk for POCD,which can lead to prolonged hospital stays,delayed recovery,and increased healthcare costs.Dex has neuroprotective,opioid-sparing,and sympatholytic properties,which reduces the incidence and severity of POCD.Dex was introduced for sedation in patients receiving mechanical ventilation but has since been adopted in anesthesia due to its multifaceted benefits.Its appli-cation extends to sedation,analgesia,maintenance of anesthesia,and controlling delirium.Its neuroprotective and anti-inflammatory effects have been explored in managing POCD.This article discussed the broad range of patient and procedure-related risk factors for POCD.Early identification and intervention are crucial to prevent the progression of POCD,which can have severe physical,psychological,and economic consequences.The article underscored the importance of a mul-tidisciplinary approach in managing POCD,involving the optimization of comor-bidities,depth of anesthesia monitoring,hemodynamic stability,and cerebral oxygenation monitoring.

摘要Cognitive dysfunction often occurs in Alzheimer’s disease,Parkinson’s disease,cerebrovascular disease,or other neurodegenerative diseases,and can significantly impact the life quality of patients and create serious social,psychological,and economic burdens for individuals and their families.Numerous studies have confirmed that exercise can slow the decline in cognitive function through multiple pathways,in which fibronectin type III domain-containing protein 5(FNDC5)plays an important role.However,the current research on the modulation of FNDC5 by exercise and its ability to improve hippocampal cognitive function lacks a systematic and comprehensive understanding.Therefore,this review focuses on the latest research progress regarding the role of exercise-induced FNDC5 in cognitive function,systematically reviews the positive effects of FNDC5 on cognitive function impairment caused by various factors,and clarifies the specific mechanisms by which exerciseinduced FNDC5 improves cognitive function by inhibiting neuroinflammation and improving hippocampal neurogenesis and hippocampal synaptic plasticity.Based on the existing literature,we also identify the areas that require further research in this field.Overall,this review provides a theoretical basis for exercise-based prevention and improvement of cognitive function impairment.

摘要Objective:To identify key factors influencing cognitive function in the elderly,including traditional Chinese medicine(TCM)constitutional classification,and to rank their relative importance.Methods:We used cross-sectional data from seven geographical regions across Chinese mainland.The Changsha version of the Montreal Cognitive Assessment was used to assess cognitive function.A“least absolute shrinkage and selection operator”(LASSO)model,multivariate linear regression analysis,and random forest(RF)model were used.Subgroup analyses were performed to examine the correlation between key TCM constitution types and cognitive function in different population subgroups.Results:A total of 24803 individuals aged 60 and above were included in the study.We selected 18 influential factors using the LASSO model.Higher education,being married,and having insurance were positively correlated with cognitive function in the elderly(all P<0.05).In contrast,poor sleep,vision impairment,hearing impairment,basic activities of daily living disability,instrumental activities of daily living disability,depression,hypertension,coronary heart disease,diabetes,stroke,yang-deficiency constitution(YADC),yin-deficiency constitution(YIDC),qi deficiency constitution(QDC),and blood stasis constitution(BSC)were negatively correlated with cognitive function(all P<0.05).YIDC and BSC affected all dimensions of cognitive function(all P<0.05).YADC mainly affected attention,language,abstraction(verbal analogies),memory,and orientation to time and place dimensions(P<0.001).QDC mainly affected language and abstraction(verbal analogies)dimensions(P<0.05).The negative correlations between BSC,YADC,YIDC,and QDC scores and cognitive function revealed statistically significant dif-ferences across most subgroups.The RF model identified education,BSC,and poor sleep quality as the three most influential factors in our study.Conclusion:BSC,YADC,YIDC,and QDC were associated with cognitive decline in the elderly.Our findings provide new perspectives and significant references for interventions for early-stage cognitive disorders.
摘要Mechanisms of animal learning and memory were traditionally studied without reference to niche-specific functional considerations. More recently, ecological demands have informed such investigations, most notably with respect to foraging in birds. In parallel, behavioural ecologists, primarily concerned with functional optimization, have begun to consider the role of mechanistic factors, including cognition, to explain apparent deviations from optimal predictions. In the present paper we discuss the application of laboratory-based constructs and paradigms of cognition to the real-world challenges faced by avian foragers. We argue that such applications have been handicapped by what we term the 'paradigmatic assumption' - the assumption that a given laboratory paradigm maps well enough onto a congruent cognitive mechanism (or cognitive ability) to justify confiation of the two. We present evidence against the paradigmatic assumption and suggest that to achieve a profitable integration between function and mechanism, with respect to animal cognition, a new conceptualization of cognitive mechanisms - functional cogni- tion - is required. This new conceptualization should define cognitive mechanisms based on the informational properties of the animal's environment and the adaptive challenges faced. Cognitive mechanisms must be examined in settings that mimic the im- portant aspects of the natural environment, using customized tasks designed to probe defined aspects of the mechanisms' opera- tion. We suggest that this approach will facilitate investigations of the functional and evolutionary relevance of cognitive mecha- nisms, as well as the patterns of divergence, convergence and specialization of cognitive mechanisms within and between species .

摘要BACKGROUND Cognitive impairment is one of the common clinical manifestations of depression,causing negative distress to patients.Elevated homocysteine(Hcy)concentrations and gut microbiome dysfunction may be observed in patients with depression.AIM To investigate the relationship between Hcy,microbiome,and cognition in depressive patients.METHODS We recruited 67 patients with major depressive disorder(MDD)(MDD group)and 94 healthy controls(HCs)individuals(HCs group).Serum Hcy levels were determined using the enzyme circulation method.16s rRNA sequencing was used to classify and identify the fecal bacteria.17 Hamilton depression rating scale and MATRICS consensus cognitive battery were used to evaluate mood states and cognition in patients with MDD. Correlation analysis was performed to explore the correlation between fecal flora,Hcy, and depressive cognitive function.RESULTSElevated serum levels of Hcy were seen in patients with MDD compared to healthy individuals. Patients withMDD indicated significant decreases in cognitive scores (P < 0.001) in six modules: Speed of processing, workingmemory, visual learning, reasoning and problem-solving, social cognition, and total scores. Hcy levels showed anegative correlation with processing speed, social cognition, and total MDD scores (P < 0.05). Hcy was alsosignificantly negatively correlated with Alistipes, Ruminococcae, Tenericides, and Porphyromonas (P < 0.05).CONCLUSIONOur results highlight that Hcy was correlated with cognition and gut microbiome in MDD. This interaction may berelated to the physiological and pathological mechanisms underlying cognitive deficits in depression.
