Background Effective public health emergency management(PHEM)is crucial for safeguarding population health and societal resilience in the face of escalating global threats.However,standardized tools for comprehensively assessing emergency readiness across diverse contexts are lacking,particularly in resource-constrained settings like Ethiopia.Existing assessment approaches have demonstrated limitations,including narrow scope focused on specific hazards or sectors,over-emphasis on implementation processes rather than programmatic outcomes,and lack of empirical grounding in tool development and validation.This study aimed to develop and validate a standardized tool to assess PHEM programs.Methods This study employed a sequential exploratory mixed methods design.Relevant assessment domains were identified through a literature review,stakeholder consultations,and expert consultations conducted at a workshop.This study utilizes Donabedian's structure-process-outcome framework to guide the assessment of public health emergency management.A questionnaire containing 60 items was then generated and underwent translation,face validation,and content validity assessment.Construct validity was evaluated using exploratory factor analysis with responses from 260 professionals.Internal consistency reliability was assessed utilizing Cronbach's alpha.Results A 45-item tool for assessing PHEM programs in diverse contexts in Ethiopia was developed and validated.The tool demonstrated high content validity(CVIs>0.83),good construct validity(15-factor structure explaining 74.8%variance),and excellent reliability(overall α=0.863,subscales>0.70).The final tool covers domains such as multi-sectoral coordination,resource allocation,transparency/accountability,workforce capacity,and provision of essential supplies.Conclusions This study developed a comprehensive tool to assess emergency management programs across diverse contexts.The validation revealed critical preparedness factors like multi-sector coordination,funding transparency and workforce strength.The mixed-methods approach proved effective for crafting contextually appropriate evaluation methods in low-resource settings with infrastructure barriers.By standardizing measurement of capacities and gaps,this validated tool can guide strategic policy planning to bolster resilience nationwide.Ongoing monitoring of progress using this model can help prioritize investments and direct coordinated responses to future crises. 展开更多

Characterization of soil hydraulic properties is important to environment management; however, it is well recognized that it is laborious, time-consuming and expensive to directly measure soil hydraulic properties. This paper reviews the development of pedotransfer functions (PTFs) used as an alternative tool to estimate soil hydraulic properties during the last two decades. Modern soil survey techniques like satellite imageryemote sensing has been used in developing PTFs. Compared to mechanistic approaches, empirical relationships between physical properties and hydraulic properties have received wide preference for predicting soil hydraulic properties. Many PTFs based on different parametric functions can be found in the literature. A number of researchers have pursued a universal function that can describe water retention characteristics of all types of soils, but no single function can be termed generic though van Cenuchten (VG) function has been the most widely adopted. Most of the reported parametric PTFs focus on estimation of VG parameters to obtain water retention curve (WRC). A number of physical, morphological and chemical properties have been used as predictor variables in PTFs. Conventionally, regression algorithmsechniques (statisticaleurM regression) have been used for calibrating PTFs. However, there are reports of utilizing data mining techniques, e.g., pattern recognition and genetic algorithm. It is inferred that it is critical to refine the data used for calibration to improve the accuracy and reliability of the PTFs. Many statistical indices, including root mean square error (RMSE), index of agreement (d), maximum absolute error (ME), mean absolute error (MAE), coefficient of determination (r2) and correlation coefficient (r), have been used by different researchers to evaluate and validate PTFs. It is argued that being location specific, research interest in PTFs will continue till generic PTFs are developed and validated. In future studies, improved methods will be required to extract information from the existing database. 展开更多

Understanding the cellular origins and early evolutionary dynamics that drive the initiation of carcinogenesis is critical to advancing early detection and prevention strategies.By characterizing key molecular,cellular and niche events at the precancerous tipping point of early gastric cancer(EGC),we aimed to develop more precise screening tools and design targeted interventions to prevent malignant transformation at this stage.We utilized our AI models to integrate spatial multimodal data from nine EGC endoscopic submucosal dissection(ESD)samples(covering sequential stages from normal to cancer),construct a spatial-temporal profile of disease progression,and identify a critical tipping point(PMC_P)characterized by an immune-suppressive microenvironment during early cancer development.At this stage,inflammatory pit mucous cells with stemness(PMC_2)interact with fibroblasts via NAMPT→ITGA5/ITGB1 and with macrophages via AREG→EGFR/ERBB2 signaling,fostering cancer initiation.We established gastric precancerous cell lines and organoids to demonstrate that NAMPT and AREG promote cellular proliferation in vitro.Furthermore,in the transgenic CEA-SV40 mouse model,targeting AREG and/or NAMPT disrupted key cell interactions,inhibited the JAK-STAT,MAPK,and NFκB pathways,and reduced PD-L1 expression,which was also confirmed by western blot in vitro.These interventions delayed disease progression,reversed the immunosuppressive microenvironment,and prevented malignant transformation.Clinical validation was conducted using endoscopically resected EGC specimens.Our study provides a precise spatiotemporal depiction of EGC development and identifies novel diagnostic markers and therapeutic targets for early intervention. 展开更多
