BACKGROUND Spinal cord injury can lead to long-term disability,but current imaging methods are limited in predicting outcomes.Rapid diffusion tensor imaging(DTI)has shown promise,yet its clinical utility remains underexplored.AIM To evaluate the potential applications of a short DTI sequence,incorporated into a cervical spine magnetic resonance imaging(MRI)protocol,for characterizing a range of symptomatic spinal cord pathologies.We propose that cervical spine tractography can provide essential diagnostic information beyond what is currently available from conventional MRI.METHODS We utilized a quick DTI sequence to create tractography models of the cervical spinal cord in four patients with distinct pathologies of various etiologies:Cord contusion,metastasis,myelopathy,and multiple sclerosis.We used DSI Studio software for post-processing of tractography cases.Fiber tract findings for each pathology case were compared to five control cases from the same scanner by looking for individual differences in white matter tract integrity based on the fractional anisotropy(FA)and mean diffusivity(MD)of the regions of interest from controls.These correlated with clinical presentations and conventional MRI findings.RESULTS Control cases showed consistent and intact tract patterns with stable FA and MD values.In pathological cases,abnormalities in fiber orientation and tract continuity correlated with clinical symptoms and lesion locations.CONCLUSION The tractography models can provide additional information on white matter disruption that was not discernible on standard MRI sequences.However,its clinical use remains limited due to the need for specialized imaging protocols and complex post-processing,restricting its use to mostly academic settings. 展开更多

Major ozonated autohemotherapy has been shown to promote recovery of upper limb motor function in patients with acute cerebral infarction, but whether naajor ozonated autohelnotherapy affects remote in)ury remains poorly understood. Here, we assumed that major ozonated autohemotherapy contributes to recovery of clinical function, possibly by reducing remote injury after acute cerebral infarction. Sixty acute cerebral infarction patients aged 30-80 years were equally and randomly allocated to ozone treatment and control groups. Patients in the ozone treatment group received medical treatment and major ozonated autohemotherapy (47 mg/L, 100 mL ozone) for 10 ± 2 days. Patients in the control group received medical treatment only. National Institutes of Health Stroke Scale score, modified Rankin scale score, and reduced degree of fractional anisotropy values of brain magnetic resonance diffusion tensor imaging were remarkably decreased, brain function improved, clinical efficiency significantly increased, and no obvious adverse reactions detected in the ozone treatment group compared with the control group. These findings suggest that major ozonated autohemotherapy promotes recovery of neurological function in acute cerebral infarction patients by reducing re,note injury, and additionally, exhibits high safety. 展开更多

To investigate the feasibility and time window of early detection of Wallerian degeneration in the corticospinal tract after middle cerebral artery infarction, 23 patients were assessed using magnetic resonance diffusion tensor imaging at 3.0T within 14 days after the infarction. The fractional anisotropy values of the affected corticospinal tract began to decrease at 3 days after onset and decreased in all cases at 7 days. The diffusion coefficient remained unchanged. Experimental findings indicate that diffusion tensor imaging can detect the changes associated with Wallerian degeneration of the corticospinal tract as early as 3 days after cerebral infarction. 展开更多

In the current study,we ... Visual field defect is one of the various clinical manifestations in patients with subarachnoid hemorrhage(SAH). Little is known about the pathogenic mechanism of visual field defect in SAH. In the current study,we investigated the diffusion tensor imaging (DTI) finding of the optic radiation in patients with SAH followingrupture of a cerebral artery aneurysm. We recruited 21 patients with aneurismal SAH (12 males, 9 females, mean age, 52.67 years; range, 41–68 years) who showed no definite lesion along the visual pathway.Twenty-one age-and sex-matched normal control subjects were also recruited. DTI data were acquired at an average of 5.9 weeks (range: 3–12 weeks) after onset and reconstruction of the optic radiation was performed using DTI-Studio software. The fractional anisotropy value, apparent diffusion coefficient value,and fiber number of the optic radiation were measured. The fractional anisotropy value of the optic radiation was significantly decreased, and the apparent diffusion coefficient value was significantly increased, in patients with aneurismal SAH than in normal control subjects. However, there was no significant difference in the fiber number of the optic radiation between patients with aneurismal SAH and normal control subjects. The decrement of fractional anisotropy value and increment of apparent diffusion coefficient value of the optic radiation in patients with aneurismal SAH suggest optic radiation injury. Therefore, we recommend a thorough evaluation for optic radiation injury in patient with aneurismal SAH. 展开更多

AIM: To evaluate the value of quantitative diffusion tensor imaging (DTI) in assessing the axonal and myelin damage of the optic nerves and optic radiations in patients with chronic primary angle -closure glaucoma (PACG) by using high -field magnetic resonance (MR) imaging (3T). METHODS: Twenty patients with bilateral chronic PACG and twenty age - and sex matched disease -free control subjects were enrolled. Conventional MRI and DTI were performed on all subjects using 3T MR scanner. Mean diffusivity (MD), fractional anisotropy (FA), axial diffusivities (AD) and radial diffusivities (RD) of each optic nerve and each optic radiation were measured by using post -processing software of DTI studio 2.3, and then compared between left eyes and right eyes and between patients group and control group. The pairedsample t- test were used. RESULTS: There was no abnormality in the shape and signal intensity of the optic nerves and optic radiations in patients group and control group on the conventional MRI. No significant differences were observed in the FA, MD, AD and RD between the right and left optic nerves and optic radiations within patients group and control group (P>0.05). The optic nerves and optic radiations of patients with chronic PACG, as compared with control subjects, had significantly higher MD, AD, RD and significantly lower FA (P<0.05). CONCLUSION: The diffusivity of optic nerves and optic radiations in chronic PACG group showed abnormal and diffusivity parameters could be used markers of axonal and myelin injury in glaucoma. 展开更多

Exploring the relationship between different structure of the spinal cord and functional assessment after spinal cord injury is important. Quantitative diffusion tensor imaging can provide information about the microstructure of nerve tissue and can quantify the pathological damage of spinal cord white matter and gray matter. In this study, a custom-designed spinal cord contusion-impactor was used to damage the T_(10) spinal cord of beagles. Diffusion tensor imaging was used to observe changes in the whole spinal cord, white matter, and gray matter, and the Texas Spinal Cord Injury Score was used to assess changes in neurological function at 3 hours, 24 hours, 6 weeks, and 12 weeks after injury. With time, fractional anisotropy values after spinal cord injury showed a downward trend, and the apparent diffusion coefficient, mean diffusivity, and radial diffusivity first decreased and then increased. The apparent diffusion-coefficient value was highly associated with the Texas Spinal Cord Injury Score for the whole spinal cord(R = 0.919, P = 0.027), white matter(R = 0.932, P = 0.021), and gray matter(R = 0.882, P = 0.048). Additionally, the other parameters had almost no correlation with the score(P 〉 0.05). In conclusion, the highest and most significant correlation between diffusion parameters and neurological function was the apparent diffusion-coefficient value for white matter, indicating that it could be used to predict the recovery of neurological function accurately after spinal cord injury. 展开更多

At present,predicting the severity of brain injury caused by global cerebral ischemiaeperfusion injury(GCI/RI)is a clinical problem.After such an injury,clinical indicators that can directly reflect neurological dysfunction are lacking.The change in hippocampal microstructure is the key to memory formation and consolidation.Diffusion tensor imaging is a highly sensitive tool for visualizing injury to hippocampal microstructure.Although hippocampal microstructure,brain-derived neurotrophic factor(BDNF),and tropomyosin-related kinase B(Trk B)levels are closely related to nerve injury and the repair process after GCI/RI,whether these indicators can reflect the severity of such hippocampal injury remains unknown.To address this issue,we established rat models of GCI/RI using the four-vessel occlusion method.Diffusion tensor imaging parameters,BDNF,and Trk B levels were correlated with modified neurological severity scores.The results revealed that after GCI/RI,while neurological function was not related to BDNF and Trk B levels,it was related to hippocampal fractional anisotropy.These findings suggest that hippocampal fractional anisotropy can reflect the severity of hippocampal injury after global GCI/RI.The study was approved by the Institutional Animal Care and Use Committee of Capital Medical University,China(approval No.AEEI-2015-139)on November 9,2015. 展开更多

Objective To distinguish the edema, injury, or rupture in the traumatic skeletal muscle fiber in vivo using diffusion tensor imaging (DTI) and tractography on magnetic resonance imaging (MRI). Methods The skeletal muscle trauma models were made in 4 rabbits ( eight hindlimbs) by iron discus ( weight 1. 0 kg, diameter 6 cm) falling down vertically from 45 cm height to rabbits' thighs. Conventional sequences and two-dimensional (2D) diffusion-weighted (DW) spin-echo (SE) echo planar imaging (EPI) sequence with fat suppression ( b = 600 s/mm^2 ) were performed on 1.5T MRI scanner. The grading of edema, injury, and fiber rupture in the damaged muscle were made according to their histopathological views, which was consistent with the images. The mean apparent diffusion coefficient (ADC) values and fractional anisotropy (FA) values were measured from the region of interests (ROIs) of all groups on 2D DW images used for tractography. Analysis of variance test was performed to analyze all data. Results ADC values of the areas in normal muscle, edema muscle, injury muscle, and ruptured muscle were (6. 12±1.34 ) × 10^-3, ( 6.38±1.30 ) × 10^-3, ( 8.06±0. 97 ) × 10^-3, and ( 9. 57±0. 93 ) × 10^-3 mm^2/s, respectively. There was significant difference among groups (P 〈0. 001 ), but no difference between edema muscle and normal muscle group (P 〉0. 05). The FA values of normal muscle, edema muscle, injury muscle, and ruptured muscle were 0. 42±0. 12, 0. 36±0. 12, 0. 26±0. 09, 0. 12±0. 08, respectively, with a significant difference among groups ( P 〈 0. 001 ). In the edema muscle, the tracking cross-fiber could be seen but it decreased slightly. In the injury muscle, the tracking fiber decreased markedly. In the ruptured muscle, the transverse-orientation tracking fiber vanished, yet some interrupted longitudinal-orientation tracking fiber could be found. Conclsion The edema, injury, and rupture of muscle fiber in rabbit damaged skeletal muscle can be verified according to the ADC and the FA on DTI and tractography. 展开更多

AIM:To investigate the visual pathway in normal subjects and patients with lesion involved by diffusion tensor imaging(DTI)and diffusion tensor tractography(DTT).METHODS:Thirty normal volunteers,3 subjects with orbital tumors involved the optic nerve(ON)and 33 subjects with occipital lobe tumors involved the optic radiation(OR)(10 gliomas,6 meningiomas and 17 cerebral metastases)undertook routine cranium magnetic resonance imaging(MRI),DTI and DTT.Visual pathway fibers were analyzed by DTI and DTT images.Test fractional anisotropy(FA)and mean diffusivity(MD)values in different part of the visual pathway.RESULTS:The whole visual pathway but optic chiasm manifested as hyperintensity in FA maps and homogenous green signal in the direction encoded color maps.The optic chiasm did not display clearly.There was no significant difference between the bilateral FA values and MD values of normal visual pathway but optic chiasm,which the FA values tested were much too low(all P>0.05).The ONs of subjects with orbital tumors were compressed and displaced.Only one subject had lower FA values and higher MD values.OR of 9 gliomas subjects were infiltrated,with displacement in 2 and disruption in 7 subjects.All OR in 6 meniongiomas subjects were displaced.OR in 17 cerebral metastases subjects all developed displacement while 7 of them had disruption also.CONCLUSION:MR-DTI is highly sensitive in manifesting visual pathway.Visual pathway can be analyzed quantitatively in FA and MD values.DTT supplies accurate three dimensional conformations of visual pathway.But optic chiasm's manifestation still needs to improve. 展开更多

Cerebral small vessel disease(CSVD)is a leading cause of age-related microvascular cognitive decline,resulting in significant morbidity and decreased quality of life.Despite a progress on its key pathophysiological bases and general acceptance of key terms from neuroimaging findings as observed on the magnetic resonance imaging(MRI),key questions on CSVD remain elusive.Enhanced relationships and reliable lesion studies,such as white matter tractography using diffusion-based MRI(dMRI)are necessary in order to improve the assessment of white matter architecture and connectivity in CSVD.Diffusion tensor imaging(DTI)and tractography is an application of dMRI that provides data that can be used to non-invasively appraise the brain white matter connections via fiber tracking and enable visualization of individual patient-specific white matter fiber tracts to reflect the extent of CSVD-associated white matter damage.However,due to a lack of standardization on various sets of software or image pipeline processing utilized in this technique that driven mostly from research setting,interpreting the findings remain contentious,especially to inform an improved diagnosis and/or prognosis of CSVD for routine clinical use.In this minireview,we highlight the advances in DTI pipeline processing and the prospect of this DTI metrics as potential imaging biomarker for CSVD,even for subclinical CSVD in at-risk individuals. 展开更多

After baseline MRI, an intravenous bolus of 20% mannitol solution was given over 10 min in controls as well as in patients with ALF and ACLE Repeat MRI for the same position was acquired 30 rnin after completing the rnannitol injection. RESULTS: No statistically significant difference was observed between controls and patients with ALF and ACLF in metabolite ratios, DTI metrics and brain volume or CSF volume following 45 rain of mannitol infusion. There was no change in clinical status at the end of post-mannitol imaging. CONCLUSION: The osmotic effect of mannitol did not result in significant reduction of brain water content, alteration in metabolite ratios or any change in the clinical status of these patients during or within 45 min of mannitol infusion. 展开更多

Diffusion tensor MRI (DT-MRI or DTI) is emerging as an important non-invasive technology for elucidating intemal brain structures. It has recently been utilized to diagnose a series of diseases that affect the integrity of neural systems to provide a basis for neuroregenerative studies. Results from the present study suggested that neural tissue is reconstructed with multiple diffusion-weighted gradient directions DTI, which varies from traditional imaging methods that utilize 6 gradient directions. Simultaneously, the diffusion tensor matrix is obtained by multiple linear regressions from an equation of echo signal intensity. The condition number value and standard deviation of fractional anisotropy for each scheme can be used to evaluate image quality. Results demonstrated that increasing gradient direction to some extent resulted in improved effects. Therefore, the traditional 6 and 15 directions should not be considered optimal scan protocols for clinical DTI application. In a scheme with 20 directions, the condition number and standard deviation of fractional anisotropy of the encoding gradients matrix were significantly reduced, and resulted in more clearly and accurately displayed neural tissue. Results demonstrated that the scheme with 20 diffusion gradient directions provided better accuracy of structural renderings and could be an optimal scan protocol for clinical DTI application. 展开更多

We report a patient with severe ataxia due to Dandy-Walker malformation, who showed functional recovery over 10 months corresponding to a change in a cerebellar peduncle lesion. A 20-month-old female patient who was diagnosed with Dandy-Walker syndrome and six age- and sex-matched healthy control subjects were enrolled. The superior cerebellar peduncle, the middle cerebellar peduncle, and the inferior cerebellar peduncle were evaluated using fractional anisotropy and the apparent diffusion coefficient. The patients' functional ambulation category was 0 at the initial visit, but improved to 2 at the follow-up evaluation, and Berg's balance scale score also improved from 0 to 7. Initial diffusion tensor tractography revealed that the inferior cerebellar peduncle was not detected, that the fractional anisotropy of the superior cerebellar peduncle and middle cerebellar peduncle decreased by two standard deviations below, and that the apparent diffusion coefficient increased by two standard deviations over normal control values. However, on follow-up diffusion tensor tractography, both inferior cerebellar peduncles could be detected, and the fractional anisotropy of superior cerebellar peduncle increased to within two standard deviations of normal controls. The functional improvement in this patient appeared to correspond to changes in these cerebellar peduncles. We believe that evaluating cerebellar peduncles using diffusion tensor imaging is useful in cases when a cerebellar peduncle lesion is suspected. 展开更多

Conventional imaging technology does not measure white matter lesions of the brain or cognitive impairment. Diffusion tensor imaging is able to detect changes in the microstructure of white matter, and correlating vascular risk factors with cognitive dysfunction seems feasible using this technique. A total of 60 leukoaraiosis patients and 30 healthy elderly controls received diffusion tensor imaging examination to detect the average diffusion coefficient and fractional anisotropy of leukoaraiosis lesion and normal white matter. Leukoaraiosis severity positively correlated with average diffusion coefficient but negatively correlated with fraction anisotropy. Neuropsychology tests (mini-mental state examination) were related to average diffusion coefficient and fraction anisotropy of normal white matter in leukoaraiosis patients, especially in the anterior horn and centrum semiovale. The average diffusion coefficient and fraction anisotropy of leukoaraiosis displayed characteristic changes. Therefore, diffusion tensor imaging can detect the macrostructural changes in white matter with normal magnetic resonance imaging, and these changes are related to cognitive function. Application of diffusion tensor imaging techniques to analyze the correlation between vascular risk factors and cognitive dysfunction is feasible. 展开更多

The corticospinal tract (CST) is one of the most important neural tracts for motor function in the human brain. Little is known about age-related changes of the CST. tn this study, we tried to evaluate age-related changes of the CST using diffusion tensor imaging in 60 healthy subjects. The diffusion tensor imaging result revealed that the tract number and fractional anisotropy value were decreased, and the apparent diffusion coefficient (ADC) value was increased with aging. The distribution showed a semilog pattern for tract number, fractional anisotropy and ADC of the CST, and the pattern of each graph was near-linear. When compared with the diffusion tensor imaging parameters of subjects in the 20 s age group, tract number and fractional anisotropy values were significantly decreased in the 50 s-70 s age groups. Likewise, the ADC value was significantly higher in the 50 s-70 s age groups. The CST in the brain of normal subjects degenerated continuously from the 20 s to the 70 s, with a near-linear pattern, and degeneration of the CST began to manifest significantly in the subjects in their 50 s, compared with the subjects in their 20 s. 展开更多

BACKGROUND: There is a growing research focus on the combination of blood oxygenation level dependent functional magnetic resonance imaging (BOLD-fMRI) and diffusion tensor imaging (DTI) to evaluate visual cortical function and structural changes in the cerebrum, as well as morphological changes to the white matter fiber tracks, after visual pathway lesions. However, the combined application of BOLD-fMRI and DTI in treating of visual pathway abnormalities still requires further studies. OBJECTIVE: To observe and evaluate the effects of hyperbaric oxygen on visual pathway abnormalities, and to evaluate the characteristics of cerebral function and anatomic structural changes by using BOLD-fMRI combined with DTI technique. DESIGN: Case contrast observation. SETTING: Fuzhou General Hospital of Nanjing Military Area Command of Chinese PLA. PARTICIPANTS: Sixteen patients (9 males and 7 females, 15-77 years old) with lateral or bilateral visual disorder induced by visual pathway lesions were selected from the Department of Neurology, Fuzhou General Hospital of Nanjing Military Area Command of Chinese PLA from January 2006 to May 2007. These patients comprised the lesion group. Measures of interventional therapy: hyperbaric oxygen of two normal atmospheres for three courses (10 d/course) and routine internal medicine treatment. In addition, 12 healthy subjects of similar sex and age to the lesion group were regarded as the control group. The control group underwent routine ophthalmological and ocular fundus examinations; diagnostic results were normal. The experiment received confirmed consent from the local ethic committee, and all patients provided informed consent. METHODS: BOLD-fMRI and DTI manifestations in the lesion group were observed before and after hyperbaric oxygen intervention, and the results were compared with the control group. The subjects were positioned on their back, and BOLD-fMRI images were collected with the following GRE EPI sequence: TR = 2 000 ms, TE = 40 ms, layer thickness = 5 mm, and 20-layer successive scanning to cover the whole brain. While, DTI images were collected with SE EPI sequence of single excitation: TR=10 000 ms, TE = 112 ms, layer thickness = 4 mm, layer spacing = 0.5 mm, and a 30-layer successive scanning, matrix = 128×128. A diffusion gradient was applied at 13 directions, and one layer without diffusion weighted imaging was collected at b =1 000 s/mm^2. Numbers of activated voxels in cortical-activated regions, and fractional anisotropy in bilateral cerebral optical radiation regions, were calculated. Displacement, continuity, and destruction of fibrous tracts were analyzed. MAIN OUTCOME MEASURES: Results of BOLD-fMRI and DTI examinations. RESULTS: All 16 patients and 12 controls were included in the final analysis. ① Numbers of activated voxels through the use of BOLD-fMRI: prior to hyperbaric oxygen therapy, the number of activated voxels in the bilateral cortex of occipital lobe were significantly less in the lesion group than in the control group (t =3.23, P 〈 0.01). In addition, the number of activated voxels significantly increased after treatment compared to before treatment (t = 2.46, P 〈 0.05). ② Fractional anisotropy in optical radiation regions of bilateral cerebrum: fractional anisotropy in the lesion group was significantly less than the control group (t =2.89, P 〈 0.05). In addition, fractional anisotropy after treatment was significantly higher than before treatment (t = 2.48, P 〈 0.05). Moreover, fractional anisotropy of optical neuropathy was significantly higher in 6 patients in the lesion group than the occipital lobe optical central lesion (t = 2.35, P 〈 0.05). CONCLUSION: BOLD-fMRI combined with DTI can indicate the occurrence, development, and therapeutic course of action for optical pathway lesions. The results acquired from these methods can provide information for function and structure, which can provide reliable verification in the treatment of cerebral function. 展开更多

Based on the Wallerian degeneration in the spinal cord pathways,the changes in synaptic connections,and the spinal cord-related cellular responses that alter the cellular structure of the brain,we presumed that brain diffusion tensor imaging(DTI)parameters may change after spinal cord injury.However,the dynamic changes in DTI parameters remain unclear.We established a Beagle dog model of T10 spinal cord contusion and performed DTI of the injured spinal cord.We found dynamic changes in DTI parameters in the cerebral peduncle,posterior limb of the internal capsule,pre-and postcentral gyri of the brain within 12 weeks after spinal cord injury.We then performed immunohistochemistry to detect the expression of neurofilament heavy polypeptide(axonal marker),glial fibrillary acidic protein(glial cell marker),and NeuN(neuronal marker).We found that these pathological changes were consistent with DTI parameter changes.These findings suggest that DTI can display brain structure changes after spinal cord injury. 展开更多

Compared with the control group,the mild depressed patients had:(1)a higher choline/creatine ratio and a negative correlation between the choline/creatine ratio and the average diffusion coefficient in the hippocampus;(2)a lower choline/creatine ratio and a higher fractional anisotropy in the striatum;(3)a lower fractional anisotropy and a positive correlation between the fractional anisotropy and the choline/creatine ratio in the prefrontal deep white matter;and(4)a higher average diffusion coefficient and a positive correlation between the choline/creatine ratio and the N-acetylaspartate/creatine ratio in the thalamus,as well as positive correlation between the choline/creatine ratio and Hamilton depression scores.These data suggest evidence of abnormal connectivity in neurofibrotic microstructures and abnormal metabolic alterations in the limbic-cortical-striatal-pallidal-thalamic neural circuit in patients with mild depression. 展开更多
