●AIM:To compare Visual Functioning Questionnaire-25(VFQ)response data in patients with age-related macular degeneration(AMD)to patients with cataracts.●METHODS:In total 415 individuals with early or intermediate AMD...●AIM:To compare Visual Functioning Questionnaire-25(VFQ)response data in patients with age-related macular degeneration(AMD)to patients with cataracts.●METHODS:In total 415 individuals with early or intermediate AMD and 236 patients with visually significant cataracts who completed the VFQ-25 from a single academic eye center registry between July 2014 and May 2022 were identified.The effect of disease(AMD versus cataract)on VFQ composite score was analyzed using univariate and multivariable linear regression,controlling for age,race/ethnicity,history of transient ischemic attack(TIA)/stroke,and visual acuity.●RESULTS:AMD patients were older than the cataract group(76.0±7.4y vs 73.2±5.9y,P<0.001).There was no difference in sex between groups with both being predominantly female(63%for AMD vs 61%for cataracts,P=0.801).The VFQ composite score was higher in the AMD group(88.8±10.7 vs 82.8±14.5,P<0.001).All vision-related and socio-emotional subscales had significantly higher scores among AMD compared to cataract patients.When adjusted for age,race/ethnicity,history of TIA/stroke,bettereye visual acuity,and worse-eye visual acuity,patients with cataracts had a 3.3-point lower VFQ composite score(95%CI:-5.3 to-1.4,P<0.001)compared to patients with AMD.●CONCLUSION:Patients with early or intermediate AMD report higher visual functioning compared to patients with cataracts in composite score and all VFQ subscale categories in both unadjusted and adjusted analyses.展开更多
This psychobiography aimed to uncover the characteristics of optimal personality functioning(OPF)across the lifespan of Chabani Manganyi(1940–2024),the first Black South African clinical psychologist.The methodology ...This psychobiography aimed to uncover the characteristics of optimal personality functioning(OPF)across the lifespan of Chabani Manganyi(1940–2024),the first Black South African clinical psychologist.The methodology used in this study encompassed an existential Franklian scholarly psychobiography.Sources of data on Manganyi included only publicly available primary and secondary data.Primary sources included Manganyi’s own writings,such as his autobiography,as well as his academic publications,including the biographies he wrote on creative individuals such as Gordimer,Sekoto and Mphahlele.Secondary sources included scholarly publications by academics and colleagues who knew him,as well as tributes,historical accounts,and archival records related to South African psychology scholarship during apartheid and the country’s transition to democracy.The study’s data sources were captured using online research platforms and search engines that included EBSCOhost,ResearchGate,Google Scholar,the University of the Free State’s Kovsie Catalogue and ProQuest.Alexander’s(1988,1990)biographical approach,which lists nine indicators of thematic salience(i.e.,uniqueness,negation,emphasis,primacy,frequency,error or distortion,isolation,incompletion,and omission)were utilized for the identification,extraction and compilation of salient data for analysis,alongside Frankl’s proposed nine characteristics of optimal personality functioning.Findings revealed that Manganyi personified characteristics of self-determining action,which were consistently evident in his pursuit of education,his intellectual independence,and his scholarly innovations in the field of South African psychology.Manganyi also personified a sense of self-transcendence,primarily expressed through his scholarship,mentorship,and social advocacy.He consistently positioned his dedication to work in his search for broader societal comprehension and transformation,beyond his personal advancement.Manganyi also exhibited qualities of future-directedness,work as vocation,and the search for meaning in resilient living,serving as a role model in managing challenging life and historical circumstances in transformative ways.The findings align with Frankl’s existential characteristics of optimal personality functioning as applied within this scholarly psychobiographical approach,highlighting its cross-cultural transportability in studying historical figures.展开更多

In drylands,biocrusts play crucial roles in regulating ecosystem functions.The study was conducted in the hilly rangelands of the semi-arid northern Negev of Israel,where we assessed the visual,morphological,spectral,...In drylands,biocrusts play crucial roles in regulating ecosystem functions.The study was conducted in the hilly rangelands of the semi-arid northern Negev of Israel,where we assessed the visual,morphological,spectral,and soil properties of livestock trampling routes and inter-route spaces in northern and southern facing hillslopes.Overall,both hillslope aspects were visually similar,whereas the ground surface of the routes was brighter(74.4%were characterized as having a'light'color)than the inter-route spaces(86.8%were characterized as having a'dark'color).These observations were supported by morphological identification of biocrust composition,which was dominated by cyanobacteria(67%)in the routes,and by mixed cyanobacteria/moss(56%)in the inter-routes.Mean Normalized Difference Vegetation Index(NDVI)was 24%higher in the inter-routes,while the mean Brightness Index(BI)was 12%higher in the routes.At the same time,the mean Crust Index(CI)was identical in the two microhabitats.Soil quality index(SQI),calculated based on the(pedoderm)soil properties of the two microhabitats,was 6%greater in the inter-routes than in the routes.This study suggests that recurrent trampling exacerbates soil compaction and shearing along the routes,thus preventing the successional development of complex biocrust compositions.展开更多
Objectives:Positive family functioning(FF)is critical for adolescent development,yet only a few studies have examined this developmental trajectory pathway.This study aimed to identify different types of FF developmen...Objectives:Positive family functioning(FF)is critical for adolescent development,yet only a few studies have examined this developmental trajectory pathway.This study aimed to identify different types of FF development trajectories during junior high school students,investigate their influence on social avoidance(SA),and further examine the mediating role of preference for solitude(PS)between them.Methods:A three-wave longitudinal study was used with six-month intervals.Questionnaire data were collected from 436 junior high school students in Jiangxi Province,China.Participants ranged in age from 11 to 14 years old(Mean=12.89 years,SD=1.08;50.2%male).Results:Four heterogeneous types of FF trajectories were identified:(1)a high and increasing group(14.7%);(2)a consistently high group(36.24%);(3)a consistently moderate group(45.86%);and(4)a rapid growth group(3.2%).The developmental trajectories of FF among junior high students significantly varied in their levels of SA(F(3,432)=32.03,p<0.001).Compared to the high and increasing groups,the consistently high,consistently medium,and rapid growth groups exhibited higher levels of SA.PS mediated the association between the developmental trajectory of FF and SA.Conclusion:There was a close relationship between the developmental trajectory of FF and SA.Interventions focusing on family system optimization and solitary preference management could effectively mitigate SA behaviors.These findings are important for promoting healthy socialization in adolescents.展开更多
BACKGROUND Adolescents with an evening chronotype are at higher risk for mental health problems,such as depression and anxiety.However,the mechanisms underlying this relationship are not well understood.Sleep disturba...BACKGROUND Adolescents with an evening chronotype are at higher risk for mental health problems,such as depression and anxiety.However,the mechanisms underlying this relationship are not well understood.Sleep disturbances and impaired social functioning may mediate this association.AIM To examine the mediating roles of sleep quality and social functioning in the relationship between chronotype and mental health in adolescents.METHODS A cross-sectional survey was conducted among 381 adolescents(mean age 14.3 years;61.9%male)using the Morningness-Eveningness Questionnaire,Hamilton Depression Rating Scale,Hamilton Anxiety Scale,Pittsburgh Sleep Quality Index,and Social Disability Screening Schedule.Descriptive statistics,Pearson correlation analyses,and chain mediation analysis using the bootstrap method were performed.RESULTS Chronotype was significantly associated with both depression and anxiety.Sleep quality and social functioning significantly mediated these relationships.The indirect effect on depression was-0.52,accounting for 69.3%of the total effect.For anxiety, the indirect effect was -0.39, accounting for 79.3%. No significant direct effect of chronotype on depressionor anxiety was found after accounting for the mediators.CONCLUSIONSleep quality and social functioning mediate the association between chronotype and adolescent mental health.These findings highlight the importance of addressing sleep disturbances and social functioning impairments ininterventions aimed at improving mental well-being among adolescents with evening chronotypes.展开更多
We sought to identify the differences between adolescents left behind in their home villagesowns(LBA)and non-left behind adolescents(NLB)on subjective well-being and family functioning due to parental migration in sou...We sought to identify the differences between adolescents left behind in their home villagesowns(LBA)and non-left behind adolescents(NLB)on subjective well-being and family functioning due to parental migration in south China.We used a stratified cluster sampling method to recruit middle school students in a city experiencing population-emigration in Jiangxi Province in 2010.Participants included adolescents from families with:(1)one migrant parent,(2)both parents who migrated,or(3)non-left behind adolescents(i.e.,no migrant parent).To determine predictors of subjective well-being,we used structural equation models.Adolescents left behind by both parents(LBB)were less likely to express life satisfaction(P=0.038)in terms of their environments(P=0.011)compared with NLB.A parent or parents who migrated predicts lower subjective well-being of adolescents(P=0.051)and also lower academic performance.Being apart from their parents may affect family functioning negatively from an adolescent's viewpoint.Given the hundreds of millions of persons in China,many who are parents,migrating for work,there may be mental health challenges in some of the adolescents left behind.展开更多
After stroke,even high-functioning individuals may experience compromised bimanual coordination and fine motor dexterity,leading to reduced functional independence.Bilateral arm training has been proposed as a promisi...After stroke,even high-functioning individuals may experience compromised bimanual coordination and fine motor dexterity,leading to reduced functional independence.Bilateral arm training has been proposed as a promising intervention to address these deficits.However,the neural basis of the impairment of functional fine motor skills and their relationship to bimanual coordination performance in stroke patients remains unclear,limiting the development of more targeted interventions.To address this gap,our study employed functional near-infrared spectroscopy to investigate cortical responses in patients after stroke as they perform functional tasks that engage fine motor control and coordination.Twenty-four high-functioning patients with ischemic stroke(7 women,17 men;mean age 64.75±10.84 years)participated in this cross-sectional observational study and completed four subtasks from the Purdue Pegboard Test,which measures unimanual and bimanual finger and hand dexterity.We found significant bilateral activation of the sensorimotor cortices during all Purdue Pegboard Test subtasks,with bimanual tasks inducing higher cortical activation than the assembly subtask.Importantly,patients with better bimanual coordination exhibited lower cortical activation during the other three Purdue Pegboard Test subtasks.Notably,the observed neural response patterns varied depending on the specific subtask.In the unaffected hand task,the differences were primarily observed in the ipsilesional hemisphere.In contrast,the bilateral sensorimotor cortices and the contralesional hemisphere played a more prominent role in the bimanual task and assembly task,respectively.While significant correlations were found between cortical activation and unimanual tasks,no significant correlations were observed with bimanual tasks.This study provides insights into the neural basis of bimanual coordination and fine motor skills in high-functioning patients after stroke,highlighting task-dependent neural responses.The findings also suggest that patients who exhibit better bimanual performance demonstrate more efficient cortical activation.Therefore,incorporating bilateral arm training in post-stroke rehabilitation is important for better outcomes.The combination of functional near-infrared spectroscopy with functional motor paradigms is valuable for assessing skills and developing targeted interventions in stroke rehabilitation.展开更多

There is a lack of evidence in literature tho...BACKGROUND Open ureteric reimplantation by cross trigonal technique described by Cohen is considered a common surgical option for correction of vesicoureteral reflux(VUR). There is a lack of evidence in literature though for what happens to such kidneys, in the long run, particularly those which are poorly functioning.AIM To assess the long-term outcomes of ureteric reimplantation in poorly functioning kidneys in children with unilateral primary VUR.METHODS Children with unilateral primary VUR and a relative renal function of less than 35% who underwent open or laparoscopic ureteric reimplantation between January 2005 and January 2017 were included in the study. Patients who had a follow up of less than five years were excluded. Preoperative evaluation consisted of a voiding cystourethrogram and Dimercaptosuccinic acid(DMSA) scan. In the follow-up period, patients underwent a diuretic scan at 6 weeks and 6 months.Follow up ultrasound was done for change in grade of hydronephrosis and retrovesical ureteric diameter. Subsequent follow up was done at 6 monthly intervals with evaluation for proteinuria and hypertension and any recurrent urinary tract infection(UTI). For assessment of cortical function, DMSA was repeated annually for 5 years after surgery. A paired-samples t-test was used to test the mean difference of DMSA between pre-post observations.RESULTS During this period, 36 children underwent ureteric reimplantation for unilateral primary VUR. After excluding those with insufficient follow-up, 31 were included in the analysis. Most of the patients were males(n = 26/31, 83.8%). Patient’s age(mean ± SD, range) was 5.21 ± 3.71, 1-18 years. The grades of VUR were grade Ⅱ(1patient), grade Ⅲ(8 patients), grade Ⅳ(10 patients), and grade Ⅴ(12 patients). The pre and postoperative DMSA was 24.064 ± 12.02 and 24.06 ± 10.93, which was almost the same(statistically equal, paired-samples t-test: P = 0.873). The median(range) follow-up duration was 82(60-120)mo. One patient had persistent reflux after surgery(preoperative: grade Ⅳ, postoperative: grade Ⅲ), and the very same patient developed recurrent UTI. The difference in the preoperative and postoperative DRF was less than 10% in 29 patients. In one patient, the DRF decreased by 17%(22% to 05%) while in another patient, the DRF increased by 12%(25% to 37%) after surgery. None of the patients had an increase in scarring after surgery. 15% of patients were hypertensive before surgery and all of them continued to be hypertensive after surgery while none developed hypertension after surgery. None of the patients had significant proteinuria(> 150 mg/d) during the follow-up period.CONCLUSION Children with unilateral primary VUR and poorly functioning kidney maintain the renal function over the long term in most cases. Hypertension and proteinuria do not progress over time in these patients.展开更多
We describe the case of a 73-year-old man with left shoulder paresis caused by a herpes zoster infection of the left C5 dermatomes.The patient had been affected by pain for 10 days,a skin rash on his left shoulder and...We describe the case of a 73-year-old man with left shoulder paresis caused by a herpes zoster infection of the left C5 dermatomes.The patient had been affected by pain for 10 days,a skin rash on his left shoulder and back for 5 days,and weakness of his left shoulder for 2 days before admission.Eiectromyography revealed denervation discharges from the left supraspinatus,infraspinatus and deltoid muscles,which was compatible with radiculopathy showing after zoster infection.The patient was examined in accordance with the International Classification of Functioning,Disability and Health,and treated with range-of-movement and strengthening exercises as well as activities of daily living and social participation.At 14 months after the onset of the condition,muscle strength had returned to normal.Electromyography revealed that motor unit action potentials were largely normal.These results indicate that the rehabilitation of paresis caused by herpes zoster can obtain positive results with suitable movement training.展开更多

The aim of this concept analysis was...Although the concept of family functioning has gained recent popularity, the terms "family functioning" and "family resilience" are sometimes confused and used interchangeably. The aim of this concept analysis was to clarify what is meant by family functioning in the context of diabetes self-management by assessing specific attributes, antecedents, and consequences. A concept analysis model by Walker and Avant was applied. The identified attributes of family functioning in a diabetes self-management context included problem-solving, communication, roles, affective responsiveness, affective involvement, and behavioral control. Antecedents included family structure, socioeconomic status, family functioning relationships, family stage, and life events. Consequences included family satisfaction, family cohesion, and family relationships. This analysis provided a deeper understanding of a family functioning concept within a diabetes self-management context. It is recommended that health care providers should be aware of antecedent factors that could inhibit outcome improvement. Further research is needed to explain family functioning attributes in relation to antecedents and potential consequences.展开更多
BACKGROUND Long-term treatment of attention deficit/hyperactivity disorder(ADHD)is associated with adverse events,such as nausea and vomiting,dizziness,and sleep disturbances,and poor maintenance of late ADHD medicati...BACKGROUND Long-term treatment of attention deficit/hyperactivity disorder(ADHD)is associated with adverse events,such as nausea and vomiting,dizziness,and sleep disturbances,and poor maintenance of late ADHD medication compromises treatment outcomes and prolongs the recovery of patients’social functioning.AIM To evaluate the effect of non-pharmacological treatment on the full recovery of social functioning in patients with ADHD.METHODS A total of 90 patients diagnosed with ADHD between May 2019 and August 2020 were included in the study and randomly assigned to either the pharmacological group(methylphenidate hydrochloride and tomoxetine hydrochloride)or the non-pharmacological group(parental training,behavior modification,sensory integration therapy,and sand tray therapy),with 45 cases in each group.Outcome measures included treatment compliance,Swanson,Nolan,and Pelham,Version IV(SNAP-IV)scores,Conners Parent Symptom Questionnaire(PSQ)scores,and Weiss Functional Impairment Rating Scale(WFIRS)scores.RESULTS The non-pharmacological interventions resulted in significantly higher compliance in patients(95.56%)compared with medication(71.11%)(P0.05).Patients with non-pharmacological interventions showed higher WFIRS scores for family,daily life skills,and self-concept than those in the pharmacological group(P<0.05).CONCLUSION Non-pharmacological interventions,in contrast to the potential risks of adverse events after longterm medication,improve patient treatment compliance,alleviate patients’behavioral symptoms of attention,impulsivity,and hyperactivity,and improve their cognitive ability,thereby improving family relationships and patient self-evaluation.展开更多
BACKGROUND Tourette syndrome(TS)is recognized as a neurodevelopmental disorder profoundly influenced by familial factors,particularly family functioning.However,the relationship among family functioning,tic severity,a...BACKGROUND Tourette syndrome(TS)is recognized as a neurodevelopmental disorder profoundly influenced by familial factors,particularly family functioning.However,the relationship among family functioning,tic severity,and quality of life in individuals with TS during childhood and adolescence remains unclear.We hypothesized that family functioning plays a role in the association between the severity of TS and quality of life in children.AIM To determine the role of family functioning in the relationship between TS severity and quality of life.METHODS This study enrolled 139 children(male/female=113/26)with TS.We assessed tic severity using the Yale Global Tic Severity Scale,quality of life via the Tourette Syndrome Quality of Life Scale,and family functioning through the Family Assessment Device.Our analysis focused on correlating these measures and exploring the mediating role of family functioning in the relationship between tic severity and quality of life.Additionally,we examined if this mediating effect varied by gender or the presence of comorbidity.RESULTS We found that family communication dysfunction had a significant mediating effect between tic severity and both psychological symptoms(indirect effect:Β=0.0038,95%confidence interval:0.0006-0.0082)as well as physical and activities of daily living impairment(indirect effect:Β=0.0029,95%confidence interval:0.0004-0.0065).For vocal tic severity,this mediation was found to be even more pronounced.Additionally,in male participants and those without attention deficit hyperactivity disorder,the mediating effect of family communication dysfunction was still evident.CONCLUSION Our study highlights the impact of family functioning on the tic severity and the quality of life in children.This relationship is influenced by gender and comorbid conditions like attention deficit hyperactivity disorder.展开更多
The present study aimed to investigate the efficacy of adenotonsillectomy(AT) for children with obstructive sleep apnea syndrome(OSAS) and the improvement of their cognitive function. Studies on cognitive performance ...The present study aimed to investigate the efficacy of adenotonsillectomy(AT) for children with obstructive sleep apnea syndrome(OSAS) and the improvement of their cognitive function. Studies on cognitive performance of OSAS children treated with or without AT were identified by searching the Pubmed, EMBASE and Cochrane library. A meta-analysis was conducted to analyze the literature. The random-effects model was used to evaluate 11 eligible studies using an inverse-variance method. The neuropsychological test results of 4 cognitive domains(general intelligence, memory, attention-executive function and verbal ability) were obtained and analyzed. By comparison of cognitive function between OSAS children and healthy controls, the effect sizes of each domain were achieved as follows: general intelligence, –0.5(P<0.0001); memory, –0.18(P=0.02); attention-executive function, –0.21(P=0.002); and verbal ability, –0.48(P=0.0006). The effect sizes of general intelligence, memory, attention-executive function, and verbal ability after AT compared to baseline level were –0.37(P=0.008), –0.36(P=0.0005), –0.02(P=0.88), and –0.45(P=0.009), respectively. Comparing the cognitive ability between OSAS children after AT and healthy controls showed that the effect sizes were –0.54(P=0.0009), –0.24(P=0.12), –0.17(P=0.35), and –0.45(P=0.009) in general intelligence, memory, attention-executive function, and verbal ability, respectively. Our results confirmed that OSAS children performed worse than healthy children in terms of the 4 cognitive domains investigated. After 6–12 months of observation, significant improvement in attention-executive function and verbal ability were found in OSAS children treated with AT compared to their baseline level; restoration of attention-executive function and memory were observed in OSAS children after AT in comparison to healthy controls. Further rigorous randomized controlled trials should be conducted to obtain definitive conclusions.展开更多

Self-regulation processes, such as goal-setting, may become disordered after traumatic brain injury, particularly when the frontal regions of the brain and their connections are involved. Such impairments reduce injured veterans' ability to return to work or school and to regain satisfactory personal lives. Understanding the neurologically disabling effects of brain injury on executive function is necessary for both the accurate diagnosis of impairment and the individual tailoring of rehabilitation processes to help returning service members recover independent function.Methods/design: The COMPASSgoal(Community Participation through Self-Efficacy Skills Development) program develops and tests a novel patient-centered intervention framework for community reintegration psychosocial research in veterans with mild traumatic brain injury. COMPASSgoal integrates the principles and best practices of goal self-management. Goal setting is a core skill in self-management training by which persons with chronic health conditions learn to improve their status and decrease symptom effects. Over a three-year period, COMPASSgoal will recruit 110 participants with residual executive dysfunction three months or more post-injury. Inclusion criteria combine both clinical diagnosis and standardized scores that are >1 SD from the normative score on the Frontal Systems Rating Scale. Participants are randomized into two groups: goal-management(intervention) and supported discharge(control). The intervention is administered in eight consecutive, weekly sessions. Assessments occur at enrollment, post-intervention/supported discharge, and three months post-treatment follow-up.Discussion: Goal management is part of the "natural language" of rehabilitation. However, collaborative goal-setting between clinicians/case managers and clients can be hindered by the cognitive deficits that follow brain injury. Re-training returning veterans with brain injury in goal management, with appropriate help and support, would essentially treat deficits in executive function. A structured approach to goal self-management may foster greater independence and self-efficacy, help veterans gain insight into goals that are realistic for them at a given time, and help clinicians and veterans to work more effectively as true collaborators.展开更多

Through the interactions among biotic and abiotic components, and encompassing physical environments, ecosystem forms a dissipative struc- ture that allows it to dissipate energy continuously and thereby remains functional over time. Biotic regulation of energy and material fluxes in and out of the ecosystem allows it to maintain a homeostatic state which corresponds to a self-organized state emerged in a non-equilibrium thermodynamic system. While the associated self-organizational processes approach to homeostatic state, entropy (a measure of irre- versibility) degrades and dissipation of energy increases. We propose here that at a homeostatic state of ecosystem, biodiversity which includes both phenotypic and functional diversity, attains optimal values. As long as biodiversity remains within its optimal range, the corresponding homeostatic state is maintained. However, while embedded environmental conditions fluctuate along the gradient of accelerating changes, phenotypic diversity and functional diversity contribute inversely to the associated self-organizing proc- esses. Furthermore, an increase or decrease in biodiversity outside of its optimal range makes the eco- system vulnerable to transition into a different state.展开更多
Objectives:The arrival of cancer in adolescents and young adults(aged 15 to 24 years)-Adolescents and young adults(AJA)-corresponds to a fragile period during which the adulthood of the young person and the evolution ...Objectives:The arrival of cancer in adolescents and young adults(aged 15 to 24 years)-Adolescents and young adults(AJA)-corresponds to a fragile period during which the adulthood of the young person and the evolution of family ties mobilize the family as a whole.Therefore,cancer,beyond its individual traumatic dimension,affects the whole family,which can modify family ties and family functioning.Our objective is to evaluate family functioning from the complex model evaluating cohesion and adaptability when an adolescent or young adult has cancer.Methods:Adolescents and young adults with cancer(n=41),mothers(n=41),and fathers(n=13)participated in this study.They completed the Family Adaptation and Cohesion Scales(FACES Ⅲ)questionnaire.Family functioning when an aya is ill has been compared to that of families without any disease.Results:a comparison of the mean scores of perceived cohesion and adaptability of face Ⅲ indicates no significant difference for cohesion.In contrast,the averages of the adaptability scores of our sample with those of the general population indicate that families with cancer hais generally feel more“adaptable”than the non-clinical population.These results are statistically significant for AJA,but also for mothers and fathers.Regarding the mean scores of ideal cohesion and ideal adaptability,there are no significant differences between fathers in our sample and fathers in the general population.In contrast,mothers in our sample had less ideal adaptability than those in the general population.In aya patients with cancer,the scores of both adaptability and cohesion were significantly different from those of non-diseased adolescents.Conclusion:Cancer leads to many changes in family relationships,making it difficult to empower the young patient and latent the evolution of the relationship.展开更多

Functional diversity is of high ecological importance because it is capable of influencing several aspects of ecosystem functioning like ecosystem dynamics, stability, nutrient availability, etc. Functional diversity of a community can be measured by functional richness and evenness. Functional richness refers to the number of species inhabiting a particular niche and functional evenness reveals how evenly the species are being distributed. Increase or decrease in functional richness and evenness simultaneously increases and decreases the functional diversity respectively. Decrease in functional richness and evenness decreases the ecosystem productivity and stability which ultimately decreases functional diversity of the same ecosystem. The effects of functional diversity on the productivity of an ecosystem can be quantitatively explained by the sampling effect model and the niche differentiation model. There are other proposed mechanisms like Niche complementarity and species redundancy relating functional diversity with ecosystem functioning. Rivets and idiosyncratic models relate functional diversity and species richness with ecosystem functioning. By considering the above proposed models on ecosystem functioning, it can be considered that functional diversity is a principal component of ecosystem functioning. So it can be assumed that, knowledge about a particular ecosystem reveals its richness and evenness which enable an individual knowing about the diversity of functional traits prevailing in the ecosystem. Methods The thyroid samples from 14 cases of diagnosed AFTAs were analyzed, with norma...Objective To determine the relationship between TSH receptor gene mutations and autonomously functioning thyroid adenomas (AFTAs). Methods The thyroid samples from 14 cases of diagnosed AFTAs were analyzed, with normal thyroid specimens adjacent to the tumors as controls. The 155 base pairs DNA fragments which encompassed the third cytoplasmic loop and the sixth transmembrane segments in the TSH receptor gene exon 10 were amplified by Polymerase chain reaction (PCR) and analyzed by the single-strand conformation polymorphism (SSCP). Direct sequencing of the PCR products was performed with Prism Dye Terminator Cycle Sequencing Core Kit. Results 6 of 14 AFTA specimens displayed abnormal migration in SSCP analysis. In sequence analysis of 3 abnormally migrated samples, one base substitution at nucleotide 1957 (A to C) and two same insertion mutations of one adenosine nucleotide between nucleotide 1972 and 1973 were identified. No mutations were found in controls. Conclusion This study confirmed the presence of TSH receptor gene mutations in AFTAs; both one-point substitution mutation and one-base insertion mutation were found to be responsible for the pathogenesis of AFTAs.展开更多

As nurses tend to obtain family-related information from particular family members in the clinical setting, when assessing families, they often confront the issue of the differences between the theoretical level and methodological level. Although this issue needs resolving for evidence-based family nursing practice, sufficient research is lacking on the methodology related to family assessment. The present study aimed to clarify the factors that affected evaluation of family functioning among couples. Semi-structured interviews were conducted with 10 child-rearing families (couples) using the Survey of Family Environment (SFE) as a family functioning scale. Content analysis identified 12 factors that affected discrepancies in the couples’ evaluations and eight factors that affected agreement in those evaluations. These factors were classified into three categories: factors concerning family or family members;factors concerning questions related to the SFE;and factors concerning the view of the family as a whole. The results of this study should contribute to the development of family assessment tools and effective methods for evaluation of family care.展开更多

摘要●AIM:To compare Visual Functioning Questionnaire-25(VFQ)response data in patients with age-related macular degeneration(AMD)to patients with cataracts.●METHODS:In total 415 individuals with early or intermediate AMD and 236 patients with visually significant cataracts who completed the VFQ-25 from a single academic eye center registry between July 2014 and May 2022 were identified.The effect of disease(AMD versus cataract)on VFQ composite score was analyzed using univariate and multivariable linear regression,controlling for age,race/ethnicity,history of transient ischemic attack(TIA)/stroke,and visual acuity.●RESULTS:AMD patients were older than the cataract group(76.0±7.4y vs 73.2±5.9y,P<0.001).There was no difference in sex between groups with both being predominantly female(63%for AMD vs 61%for cataracts,P=0.801).The VFQ composite score was higher in the AMD group(88.8±10.7 vs 82.8±14.5,P<0.001).All vision-related and socio-emotional subscales had significantly higher scores among AMD compared to cataract patients.When adjusted for age,race/ethnicity,history of TIA/stroke,bettereye visual acuity,and worse-eye visual acuity,patients with cataracts had a 3.3-point lower VFQ composite score(95%CI:-5.3 to-1.4,P<0.001)compared to patients with AMD.●CONCLUSION:Patients with early or intermediate AMD report higher visual functioning compared to patients with cataracts in composite score and all VFQ subscale categories in both unadjusted and adjusted analyses.

摘要This psychobiography aimed to uncover the characteristics of optimal personality functioning(OPF)across the lifespan of Chabani Manganyi(1940–2024),the first Black South African clinical psychologist.The methodology used in this study encompassed an existential Franklian scholarly psychobiography.Sources of data on Manganyi included only publicly available primary and secondary data.Primary sources included Manganyi’s own writings,such as his autobiography,as well as his academic publications,including the biographies he wrote on creative individuals such as Gordimer,Sekoto and Mphahlele.Secondary sources included scholarly publications by academics and colleagues who knew him,as well as tributes,historical accounts,and archival records related to South African psychology scholarship during apartheid and the country’s transition to democracy.The study’s data sources were captured using online research platforms and search engines that included EBSCOhost,ResearchGate,Google Scholar,the University of the Free State’s Kovsie Catalogue and ProQuest.Alexander’s(1988,1990)biographical approach,which lists nine indicators of thematic salience(i.e.,uniqueness,negation,emphasis,primacy,frequency,error or distortion,isolation,incompletion,and omission)were utilized for the identification,extraction and compilation of salient data for analysis,alongside Frankl’s proposed nine characteristics of optimal personality functioning.Findings revealed that Manganyi personified characteristics of self-determining action,which were consistently evident in his pursuit of education,his intellectual independence,and his scholarly innovations in the field of South African psychology.Manganyi also personified a sense of self-transcendence,primarily expressed through his scholarship,mentorship,and social advocacy.He consistently positioned his dedication to work in his search for broader societal comprehension and transformation,beyond his personal advancement.Manganyi also exhibited qualities of future-directedness,work as vocation,and the search for meaning in resilient living,serving as a role model in managing challenging life and historical circumstances in transformative ways.The findings align with Frankl’s existential characteristics of optimal personality functioning as applied within this scholarly psychobiographical approach,highlighting its cross-cultural transportability in studying historical figures.
摘要As global strategies shift toward expanding treatment eligibility for chronic hepatitis B,ensuring long-term adherence is critical.Block et al recently published a study in World Journal of Virology,challenging the conventional focus on economic barriers and revealing that compromised physical functioning is a significantly more potent predictor of non-adherence than financial affordability.This finding exposes a profound“clinical discordance”:Patients may achieve viral suppression yet suffer from residual fatigue and functional impairment,potentially driven by immune exhaustion or metabolic comorbidities.We argue that this symptom burden serves as a functional blockade to daily persistence.Therefore,clinical care must evolve from“adherence policing”to active symptom management.Integrating patient-reported outcomes and addressing physical deficits are essential strategies to safeguard the efficacy of antiviral regimens in the elimination era.
摘要In drylands,biocrusts play crucial roles in regulating ecosystem functions.The study was conducted in the hilly rangelands of the semi-arid northern Negev of Israel,where we assessed the visual,morphological,spectral,and soil properties of livestock trampling routes and inter-route spaces in northern and southern facing hillslopes.Overall,both hillslope aspects were visually similar,whereas the ground surface of the routes was brighter(74.4%were characterized as having a'light'color)than the inter-route spaces(86.8%were characterized as having a'dark'color).These observations were supported by morphological identification of biocrust composition,which was dominated by cyanobacteria(67%)in the routes,and by mixed cyanobacteria/moss(56%)in the inter-routes.Mean Normalized Difference Vegetation Index(NDVI)was 24%higher in the inter-routes,while the mean Brightness Index(BI)was 12%higher in the routes.At the same time,the mean Crust Index(CI)was identical in the two microhabitats.Soil quality index(SQI),calculated based on the(pedoderm)soil properties of the two microhabitats,was 6%greater in the inter-routes than in the routes.This study suggests that recurrent trampling exacerbates soil compaction and shearing along the routes,thus preventing the successional development of complex biocrust compositions.

摘要Objectives:Positive family functioning(FF)is critical for adolescent development,yet only a few studies have examined this developmental trajectory pathway.This study aimed to identify different types of FF development trajectories during junior high school students,investigate their influence on social avoidance(SA),and further examine the mediating role of preference for solitude(PS)between them.Methods:A three-wave longitudinal study was used with six-month intervals.Questionnaire data were collected from 436 junior high school students in Jiangxi Province,China.Participants ranged in age from 11 to 14 years old(Mean=12.89 years,SD=1.08;50.2%male).Results:Four heterogeneous types of FF trajectories were identified:(1)a high and increasing group(14.7%);(2)a consistently high group(36.24%);(3)a consistently moderate group(45.86%);and(4)a rapid growth group(3.2%).The developmental trajectories of FF among junior high students significantly varied in their levels of SA(F(3,432)=32.03,p<0.001).Compared to the high and increasing groups,the consistently high,consistently medium,and rapid growth groups exhibited higher levels of SA.PS mediated the association between the developmental trajectory of FF and SA.Conclusion:There was a close relationship between the developmental trajectory of FF and SA.Interventions focusing on family system optimization and solitary preference management could effectively mitigate SA behaviors.These findings are important for promoting healthy socialization in adolescents.
摘要BACKGROUND Adolescents with an evening chronotype are at higher risk for mental health problems,such as depression and anxiety.However,the mechanisms underlying this relationship are not well understood.Sleep disturbances and impaired social functioning may mediate this association.AIM To examine the mediating roles of sleep quality and social functioning in the relationship between chronotype and mental health in adolescents.METHODS A cross-sectional survey was conducted among 381 adolescents(mean age 14.3 years;61.9%male)using the Morningness-Eveningness Questionnaire,Hamilton Depression Rating Scale,Hamilton Anxiety Scale,Pittsburgh Sleep Quality Index,and Social Disability Screening Schedule.Descriptive statistics,Pearson correlation analyses,and chain mediation analysis using the bootstrap method were performed.RESULTS Chronotype was significantly associated with both depression and anxiety.Sleep quality and social functioning significantly mediated these relationships.The indirect effect on depression was-0.52,accounting for 69.3%of the total effect.For anxiety, the indirect effect was -0.39, accounting for 79.3%. No significant direct effect of chronotype on depressionor anxiety was found after accounting for the mediators.CONCLUSIONSleep quality and social functioning mediate the association between chronotype and adolescent mental health.These findings highlight the importance of addressing sleep disturbances and social functioning impairments ininterventions aimed at improving mental well-being among adolescents with evening chronotypes.

摘要We sought to identify the differences between adolescents left behind in their home villagesowns(LBA)and non-left behind adolescents(NLB)on subjective well-being and family functioning due to parental migration in south China.We used a stratified cluster sampling method to recruit middle school students in a city experiencing population-emigration in Jiangxi Province in 2010.Participants included adolescents from families with:(1)one migrant parent,(2)both parents who migrated,or(3)non-left behind adolescents(i.e.,no migrant parent).To determine predictors of subjective well-being,we used structural equation models.Adolescents left behind by both parents(LBB)were less likely to express life satisfaction(P=0.038)in terms of their environments(P=0.011)compared with NLB.A parent or parents who migrated predicts lower subjective well-being of adolescents(P=0.051)and also lower academic performance.Being apart from their parents may affect family functioning negatively from an adolescent's viewpoint.Given the hundreds of millions of persons in China,many who are parents,migrating for work,there may be mental health challenges in some of the adolescents left behind.

摘要After stroke,even high-functioning individuals may experience compromised bimanual coordination and fine motor dexterity,leading to reduced functional independence.Bilateral arm training has been proposed as a promising intervention to address these deficits.However,the neural basis of the impairment of functional fine motor skills and their relationship to bimanual coordination performance in stroke patients remains unclear,limiting the development of more targeted interventions.To address this gap,our study employed functional near-infrared spectroscopy to investigate cortical responses in patients after stroke as they perform functional tasks that engage fine motor control and coordination.Twenty-four high-functioning patients with ischemic stroke(7 women,17 men;mean age 64.75±10.84 years)participated in this cross-sectional observational study and completed four subtasks from the Purdue Pegboard Test,which measures unimanual and bimanual finger and hand dexterity.We found significant bilateral activation of the sensorimotor cortices during all Purdue Pegboard Test subtasks,with bimanual tasks inducing higher cortical activation than the assembly subtask.Importantly,patients with better bimanual coordination exhibited lower cortical activation during the other three Purdue Pegboard Test subtasks.Notably,the observed neural response patterns varied depending on the specific subtask.In the unaffected hand task,the differences were primarily observed in the ipsilesional hemisphere.In contrast,the bilateral sensorimotor cortices and the contralesional hemisphere played a more prominent role in the bimanual task and assembly task,respectively.While significant correlations were found between cortical activation and unimanual tasks,no significant correlations were observed with bimanual tasks.This study provides insights into the neural basis of bimanual coordination and fine motor skills in high-functioning patients after stroke,highlighting task-dependent neural responses.The findings also suggest that patients who exhibit better bimanual performance demonstrate more efficient cortical activation.Therefore,incorporating bilateral arm training in post-stroke rehabilitation is important for better outcomes.The combination of functional near-infrared spectroscopy with functional motor paradigms is valuable for assessing skills and developing targeted interventions in stroke rehabilitation.
摘要BACKGROUND Open ureteric reimplantation by cross trigonal technique described by Cohen is considered a common surgical option for correction of vesicoureteral reflux(VUR). There is a lack of evidence in literature though for what happens to such kidneys, in the long run, particularly those which are poorly functioning.AIM To assess the long-term outcomes of ureteric reimplantation in poorly functioning kidneys in children with unilateral primary VUR.METHODS Children with unilateral primary VUR and a relative renal function of less than 35% who underwent open or laparoscopic ureteric reimplantation between January 2005 and January 2017 were included in the study. Patients who had a follow up of less than five years were excluded. Preoperative evaluation consisted of a voiding cystourethrogram and Dimercaptosuccinic acid(DMSA) scan. In the follow-up period, patients underwent a diuretic scan at 6 weeks and 6 months.Follow up ultrasound was done for change in grade of hydronephrosis and retrovesical ureteric diameter. Subsequent follow up was done at 6 monthly intervals with evaluation for proteinuria and hypertension and any recurrent urinary tract infection(UTI). For assessment of cortical function, DMSA was repeated annually for 5 years after surgery. A paired-samples t-test was used to test the mean difference of DMSA between pre-post observations.RESULTS During this period, 36 children underwent ureteric reimplantation for unilateral primary VUR. After excluding those with insufficient follow-up, 31 were included in the analysis. Most of the patients were males(n = 26/31, 83.8%). Patient’s age(mean ± SD, range) was 5.21 ± 3.71, 1-18 years. The grades of VUR were grade Ⅱ(1patient), grade Ⅲ(8 patients), grade Ⅳ(10 patients), and grade Ⅴ(12 patients). The pre and postoperative DMSA was 24.064 ± 12.02 and 24.06 ± 10.93, which was almost the same(statistically equal, paired-samples t-test: P = 0.873). The median(range) follow-up duration was 82(60-120)mo. One patient had persistent reflux after surgery(preoperative: grade Ⅳ, postoperative: grade Ⅲ), and the very same patient developed recurrent UTI. The difference in the preoperative and postoperative DRF was less than 10% in 29 patients. In one patient, the DRF decreased by 17%(22% to 05%) while in another patient, the DRF increased by 12%(25% to 37%) after surgery. None of the patients had an increase in scarring after surgery. 15% of patients were hypertensive before surgery and all of them continued to be hypertensive after surgery while none developed hypertension after surgery. None of the patients had significant proteinuria(> 150 mg/d) during the follow-up period.CONCLUSION Children with unilateral primary VUR and poorly functioning kidney maintain the renal function over the long term in most cases. Hypertension and proteinuria do not progress over time in these patients.
摘要We describe the case of a 73-year-old man with left shoulder paresis caused by a herpes zoster infection of the left C5 dermatomes.The patient had been affected by pain for 10 days,a skin rash on his left shoulder and back for 5 days,and weakness of his left shoulder for 2 days before admission.Eiectromyography revealed denervation discharges from the left supraspinatus,infraspinatus and deltoid muscles,which was compatible with radiculopathy showing after zoster infection.The patient was examined in accordance with the International Classification of Functioning,Disability and Health,and treated with range-of-movement and strengthening exercises as well as activities of daily living and social participation.At 14 months after the onset of the condition,muscle strength had returned to normal.Electromyography revealed that motor unit action potentials were largely normal.These results indicate that the rehabilitation of paresis caused by herpes zoster can obtain positive results with suitable movement training.

摘要Although the concept of family functioning has gained recent popularity, the terms "family functioning" and "family resilience" are sometimes confused and used interchangeably. The aim of this concept analysis was to clarify what is meant by family functioning in the context of diabetes self-management by assessing specific attributes, antecedents, and consequences. A concept analysis model by Walker and Avant was applied. The identified attributes of family functioning in a diabetes self-management context included problem-solving, communication, roles, affective responsiveness, affective involvement, and behavioral control. Antecedents included family structure, socioeconomic status, family functioning relationships, family stage, and life events. Consequences included family satisfaction, family cohesion, and family relationships. This analysis provided a deeper understanding of a family functioning concept within a diabetes self-management context. It is recommended that health care providers should be aware of antecedent factors that could inhibit outcome improvement. Further research is needed to explain family functioning attributes in relation to antecedents and potential consequences.

摘要BACKGROUND Long-term treatment of attention deficit/hyperactivity disorder(ADHD)is associated with adverse events,such as nausea and vomiting,dizziness,and sleep disturbances,and poor maintenance of late ADHD medication compromises treatment outcomes and prolongs the recovery of patients’social functioning.AIM To evaluate the effect of non-pharmacological treatment on the full recovery of social functioning in patients with ADHD.METHODS A total of 90 patients diagnosed with ADHD between May 2019 and August 2020 were included in the study and randomly assigned to either the pharmacological group(methylphenidate hydrochloride and tomoxetine hydrochloride)or the non-pharmacological group(parental training,behavior modification,sensory integration therapy,and sand tray therapy),with 45 cases in each group.Outcome measures included treatment compliance,Swanson,Nolan,and Pelham,Version IV(SNAP-IV)scores,Conners Parent Symptom Questionnaire(PSQ)scores,and Weiss Functional Impairment Rating Scale(WFIRS)scores.RESULTS The non-pharmacological interventions resulted in significantly higher compliance in patients(95.56%)compared with medication(71.11%)(P0.05).Patients with non-pharmacological interventions showed higher WFIRS scores for family,daily life skills,and self-concept than those in the pharmacological group(P<0.05).CONCLUSION Non-pharmacological interventions,in contrast to the potential risks of adverse events after longterm medication,improve patient treatment compliance,alleviate patients’behavioral symptoms of attention,impulsivity,and hyperactivity,and improve their cognitive ability,thereby improving family relationships and patient self-evaluation.

摘要BACKGROUND Tourette syndrome(TS)is recognized as a neurodevelopmental disorder profoundly influenced by familial factors,particularly family functioning.However,the relationship among family functioning,tic severity,and quality of life in individuals with TS during childhood and adolescence remains unclear.We hypothesized that family functioning plays a role in the association between the severity of TS and quality of life in children.AIM To determine the role of family functioning in the relationship between TS severity and quality of life.METHODS This study enrolled 139 children(male/female=113/26)with TS.We assessed tic severity using the Yale Global Tic Severity Scale,quality of life via the Tourette Syndrome Quality of Life Scale,and family functioning through the Family Assessment Device.Our analysis focused on correlating these measures and exploring the mediating role of family functioning in the relationship between tic severity and quality of life.Additionally,we examined if this mediating effect varied by gender or the presence of comorbidity.RESULTS We found that family communication dysfunction had a significant mediating effect between tic severity and both psychological symptoms(indirect effect:Β=0.0038,95%confidence interval:0.0006-0.0082)as well as physical and activities of daily living impairment(indirect effect:Β=0.0029,95%confidence interval:0.0004-0.0065).For vocal tic severity,this mediation was found to be even more pronounced.Additionally,in male participants and those without attention deficit hyperactivity disorder,the mediating effect of family communication dysfunction was still evident.CONCLUSION Our study highlights the impact of family functioning on the tic severity and the quality of life in children.This relationship is influenced by gender and comorbid conditions like attention deficit hyperactivity disorder.

摘要The present study aimed to investigate the efficacy of adenotonsillectomy(AT) for children with obstructive sleep apnea syndrome(OSAS) and the improvement of their cognitive function. Studies on cognitive performance of OSAS children treated with or without AT were identified by searching the Pubmed, EMBASE and Cochrane library. A meta-analysis was conducted to analyze the literature. The random-effects model was used to evaluate 11 eligible studies using an inverse-variance method. The neuropsychological test results of 4 cognitive domains(general intelligence, memory, attention-executive function and verbal ability) were obtained and analyzed. By comparison of cognitive function between OSAS children and healthy controls, the effect sizes of each domain were achieved as follows: general intelligence, –0.5(P<0.0001); memory, –0.18(P=0.02); attention-executive function, –0.21(P=0.002); and verbal ability, –0.48(P=0.0006). The effect sizes of general intelligence, memory, attention-executive function, and verbal ability after AT compared to baseline level were –0.37(P=0.008), –0.36(P=0.0005), –0.02(P=0.88), and –0.45(P=0.009), respectively. Comparing the cognitive ability between OSAS children after AT and healthy controls showed that the effect sizes were –0.54(P=0.0009), –0.24(P=0.12), –0.17(P=0.35), and –0.45(P=0.009) in general intelligence, memory, attention-executive function, and verbal ability, respectively. Our results confirmed that OSAS children performed worse than healthy children in terms of the 4 cognitive domains investigated. After 6–12 months of observation, significant improvement in attention-executive function and verbal ability were found in OSAS children treated with AT compared to their baseline level; restoration of attention-executive function and memory were observed in OSAS children after AT in comparison to healthy controls. Further rigorous randomized controlled trials should be conducted to obtain definitive conclusions.

摘要Background: Traumatic brain injury is a major health problem that frequently leads to deficits in executive function. Self-regulation processes, such as goal-setting, may become disordered after traumatic brain injury, particularly when the frontal regions of the brain and their connections are involved. Such impairments reduce injured veterans' ability to return to work or school and to regain satisfactory personal lives. Understanding the neurologically disabling effects of brain injury on executive function is necessary for both the accurate diagnosis of impairment and the individual tailoring of rehabilitation processes to help returning service members recover independent function.Methods/design: The COMPASSgoal(Community Participation through Self-Efficacy Skills Development) program develops and tests a novel patient-centered intervention framework for community reintegration psychosocial research in veterans with mild traumatic brain injury. COMPASSgoal integrates the principles and best practices of goal self-management. Goal setting is a core skill in self-management training by which persons with chronic health conditions learn to improve their status and decrease symptom effects. Over a three-year period, COMPASSgoal will recruit 110 participants with residual executive dysfunction three months or more post-injury. Inclusion criteria combine both clinical diagnosis and standardized scores that are >1 SD from the normative score on the Frontal Systems Rating Scale. Participants are randomized into two groups: goal-management(intervention) and supported discharge(control). The intervention is administered in eight consecutive, weekly sessions. Assessments occur at enrollment, post-intervention/supported discharge, and three months post-treatment follow-up.Discussion: Goal management is part of the "natural language" of rehabilitation. However, collaborative goal-setting between clinicians/case managers and clients can be hindered by the cognitive deficits that follow brain injury. Re-training returning veterans with brain injury in goal management, with appropriate help and support, would essentially treat deficits in executive function. A structured approach to goal self-management may foster greater independence and self-efficacy, help veterans gain insight into goals that are realistic for them at a given time, and help clinicians and veterans to work more effectively as true collaborators.

摘要Ecosystem stays far from thermodynamic equilibrium. Through the interactions among biotic and abiotic components, and encompassing physical environments, ecosystem forms a dissipative struc- ture that allows it to dissipate energy continuously and thereby remains functional over time. Biotic regulation of energy and material fluxes in and out of the ecosystem allows it to maintain a homeostatic state which corresponds to a self-organized state emerged in a non-equilibrium thermodynamic system. While the associated self-organizational processes approach to homeostatic state, entropy (a measure of irre- versibility) degrades and dissipation of energy increases. We propose here that at a homeostatic state of ecosystem, biodiversity which includes both phenotypic and functional diversity, attains optimal values. As long as biodiversity remains within its optimal range, the corresponding homeostatic state is maintained. However, while embedded environmental conditions fluctuate along the gradient of accelerating changes, phenotypic diversity and functional diversity contribute inversely to the associated self-organizing proc- esses. Furthermore, an increase or decrease in biodiversity outside of its optimal range makes the eco- system vulnerable to transition into a different state.
摘要Objectives:The arrival of cancer in adolescents and young adults(aged 15 to 24 years)-Adolescents and young adults(AJA)-corresponds to a fragile period during which the adulthood of the young person and the evolution of family ties mobilize the family as a whole.Therefore,cancer,beyond its individual traumatic dimension,affects the whole family,which can modify family ties and family functioning.Our objective is to evaluate family functioning from the complex model evaluating cohesion and adaptability when an adolescent or young adult has cancer.Methods:Adolescents and young adults with cancer(n=41),mothers(n=41),and fathers(n=13)participated in this study.They completed the Family Adaptation and Cohesion Scales(FACES Ⅲ)questionnaire.Family functioning when an aya is ill has been compared to that of families without any disease.Results:a comparison of the mean scores of perceived cohesion and adaptability of face Ⅲ indicates no significant difference for cohesion.In contrast,the averages of the adaptability scores of our sample with those of the general population indicate that families with cancer hais generally feel more“adaptable”than the non-clinical population.These results are statistically significant for AJA,but also for mothers and fathers.Regarding the mean scores of ideal cohesion and ideal adaptability,there are no significant differences between fathers in our sample and fathers in the general population.In contrast,mothers in our sample had less ideal adaptability than those in the general population.In aya patients with cancer,the scores of both adaptability and cohesion were significantly different from those of non-diseased adolescents.Conclusion:Cancer leads to many changes in family relationships,making it difficult to empower the young patient and latent the evolution of the relationship.
摘要Functional diversity is a component of biodiversity that generally covers the range of functional traits of microorganisms prevailing in an ecosystem. Functional diversity is of high ecological importance because it is capable of influencing several aspects of ecosystem functioning like ecosystem dynamics, stability, nutrient availability, etc. Functional diversity of a community can be measured by functional richness and evenness. Functional richness refers to the number of species inhabiting a particular niche and functional evenness reveals how evenly the species are being distributed. Increase or decrease in functional richness and evenness simultaneously increases and decreases the functional diversity respectively. Decrease in functional richness and evenness decreases the ecosystem productivity and stability which ultimately decreases functional diversity of the same ecosystem. The effects of functional diversity on the productivity of an ecosystem can be quantitatively explained by the sampling effect model and the niche differentiation model. There are other proposed mechanisms like Niche complementarity and species redundancy relating functional diversity with ecosystem functioning. Rivets and idiosyncratic models relate functional diversity and species richness with ecosystem functioning. By considering the above proposed models on ecosystem functioning, it can be considered that functional diversity is a principal component of ecosystem functioning. So it can be assumed that, knowledge about a particular ecosystem reveals its richness and evenness which enable an individual knowing about the diversity of functional traits prevailing in the ecosystem. Thus, it opens up a new way in understanding and carrying out ecology related studies more efficiently and precisely in ecosystem.
摘要Objective To determine the relationship between TSH receptor gene mutations and autonomously functioning thyroid adenomas (AFTAs). Methods The thyroid samples from 14 cases of diagnosed AFTAs were analyzed, with normal thyroid specimens adjacent to the tumors as controls. The 155 base pairs DNA fragments which encompassed the third cytoplasmic loop and the sixth transmembrane segments in the TSH receptor gene exon 10 were amplified by Polymerase chain reaction (PCR) and analyzed by the single-strand conformation polymorphism (SSCP). Direct sequencing of the PCR products was performed with Prism Dye Terminator Cycle Sequencing Core Kit. Results 6 of 14 AFTA specimens displayed abnormal migration in SSCP analysis. In sequence analysis of 3 abnormally migrated samples, one base substitution at nucleotide 1957 (A to C) and two same insertion mutations of one adenosine nucleotide between nucleotide 1972 and 1973 were identified. No mutations were found in controls. Conclusion This study confirmed the presence of TSH receptor gene mutations in AFTAs; both one-point substitution mutation and one-base insertion mutation were found to be responsible for the pathogenesis of AFTAs.
摘要In family healthcare nursing, the family system unit (i.e., a group in which the members, seen as a whole, mutually interact) is the target of care. As nurses tend to obtain family-related information from particular family members in the clinical setting, when assessing families, they often confront the issue of the differences between the theoretical level and methodological level. Although this issue needs resolving for evidence-based family nursing practice, sufficient research is lacking on the methodology related to family assessment. The present study aimed to clarify the factors that affected evaluation of family functioning among couples. Semi-structured interviews were conducted with 10 child-rearing families (couples) using the Survey of Family Environment (SFE) as a family functioning scale. Content analysis identified 12 factors that affected discrepancies in the couples’ evaluations and eight factors that affected agreement in those evaluations. These factors were classified into three categories: factors concerning family or family members;factors concerning questions related to the SFE;and factors concerning the view of the family as a whole. The results of this study should contribute to the development of family assessment tools and effective methods for evaluation of family care.
